UNSAFE ACT

1. Several dry dock employees observed crossing over mooring lines.


All personnel informed not to cross the moorings and red tape barricades in place to warn
personnel of the same.
Ramp will be rigged by the yard safety officer to provide safe access over the moorings.

Gas hose observed routed with high voltage power cable on main deck stbd. Side various
location.
Onboard foreman and service watchman were informed to arrange separately.

Open paint bucket observed on main deck under pipe rack and poop deck without barricading &
signage.
Foreman informed to provide barricading and signage.

Electrical socket damaged safety flap cover observed on main deck various location.
Service watchman informed to rectify.

Garbage, mud and rubbish bags were observed on main deck and poop deck.

Steel materials temporary storage on main deck without any barricading and signage.
Trades foreman informed.

Staging materials storage various location on main deck without barricading and signage.
Onboard stager informed accordingly.

In engine room generator platform access observed fully obstructed by electrical motor and
material box.

There are four skip were observed overflow on main deck, forecastle deck and poop deck.
Crane section was contacted and skips were cleared and empty garbage is provided.

2. Fire extinguisher near boiler platform on E/R 2nd Deck found obscured and obstructed by metal
plates and piping.
2nd Eng. Notified and area cleared of all obstructions.
All ship staff notified in tool box meeting to ensure FFA and LSA onboard the vessel to be clear
obstructions at all times.
3. Empty plastic bottles were lying in the foam room
All Plastic bottles removed and foam room cleaned
Al staff members instructed to dispose off the plastic bottles after use and comply the shipboard
garbage management plan.

4. Lots of garbage found scattered in the Fwd section of the vessel even though garbage bags are
provided in various locations.
All garbage cleared
The same brought up with the safety officer and DD project manager to inform their staff to
maintain good housekeeping at all times.

5. Lots of wooden waste and shavings left behind after completion of ply wood laying on the
flooring in the accommodation and engine room.
Areas cleaned and carpenters advised to maintain good housekeeping.
The same brought to the attention of the yard safety officer and the ship repair manager to
follow up with their staff.

6. During safety walk around with yard staff it was noticed that several accesses in engine room
near incinerator and bottom platform were obstructed by pipes and waste materials.
Yard personnel informed to clear the accesses and keep free from obstruction at all times.
The same brought to the attention of the yard safety officer and the ship repair manager to
follow up with their staff.

7. Too much congestion observed in various spaces on the deck especially on the port quarter due
to continuous landing of objects by the shore crane.
Yard staff cleared all areas and is free of obstructions.
Decision made by ship repair manager to barricade some areas and prevent access of personnel
due to congestion and to highlight emergency exits away from such congested spaces.

8. Damage was caused to an external tube light casing due to miscommunication between the
signal man and the crane operator. The load of the crane swung and hit the funnel casing side
where the tube light got damaged.
Signal man informed to be vigilant and to give clear instructions to the crane operator to ensure
safety. Tube light casing renewed with spare by ETO.
The same brought to the attention of the Ship repair manager and the Yard safety officer to
follow up with their staff. Also, ship’s crew advised to keep a close eye on the yard staff with
respect to crane operations.

9. Many cables observed obstructing the access on the Bosun store staircase.
Hatch opened and cables re-routed to clear staircase of obstructions.
The same brought to the attention of the Yard safety officer to follow up with their staff.
10. One yard personnel was carrying out chipping on the dresser couplings of the C.O.W. line on
deck. He left during tea time leaving his tools pressurized and eventually the air hose burst from
the equipment violently swaying in the area.
The air supply was stopped and the air hose connected back to the equipment by the person
responsible.
The person responsible notified and the same brought to the attention yard safety officer to
follow up.

11. During lunch time it was observed that too many members of the yard staff were using the
gangway simultaneously. The max. Persons allowed on the shore gangway is 40 persons
however, more than 50 persons were using it at the time.
All persons stopped by the safety officer and asked to disembark the vessel in a single file so that
there is no congestion on the shore gangway.
The same brought to the attention of the Yard safety officer and the SRM to follow up with their
staff.

12. COT 1S manholes were found left open and unattended by the yard staff. The wooden covers
and barricades for the same were shifted out of position and no warning to passersby.
Safety officer and Bosun put back the wooden covers and barricades.
The same brought to the attention of the Yard safety officer and the SRM to follow up with their
staff and discussed in daily meeting.

13. Welding being carried out on A-Deck drain lines without any supervision by fire man and the
sparks were falling down onto the upper deck where there was no barricade or warning to the
people passing underneath.
Job stopped by the safety officer. Barricades in place to restrict the area. Fire man stationed to
monitor the area.
The same brought to the attention of the Yard safety officer and the SRM to follow up with their
staff.

14. Welding transformer left switched on and unattended during lunch time by yard staff.
Transformer switched off by ships staff.
The same brought to the attention of the Yard safety officer and the SRM to follow up with their
staff.

15. Lots of loose items such as pipes, nut-bolts and other spares were noticed lying around in
various levels of the engine room. These were either tripping hazards or obstructing access to
certain spaces.
All items cleared by dry dock staff and all accesses kept clear.
The matter was raised in the daily safety and production meeting and all trades were asked to
clear up the work areas upon completion of their jobs.

16. One member of the dry dock staff was found standing right below a lift which was being handled
by the E.R. Crane.
The craneaghe was stapped and the person in question was asked to keep clear of the area.
All personnel (Yard and Ships staff) were briefed on the matter and have been instructed to
comply with the C.O.S.W.P at all times.

17. One member of the dry dock staff was observed crossing into an area which was barricaded by
red tape. The area was roped off because testing of boilers was in progress and area was to be
clear of all personnel.
The person in question was immediately notified. He was explained that the area had been
roped off for a reason and that he must at all times respect the barricades or use another route
to reach his destination.
The matter was discussed in the daily safety and production meeting, the yard safety officer had
a safety meeting with all yard staff regarding the same.

18. One member of ships staff observed carrying out fabric maintenance of deck pipelines with
safety harness, however the harness had been attached with a shock arrestor of 2m length,
incase of a fall the person in question would not be protected by the harness.
the job was stopped and the shock arrestor was removed to shorten the length of the safety
harness.
All crew members briefed in the morning toolbox meeting regarding the correct use of safety
harness and working aloft procedures.

19. Technicians who were appointed for installation of the new CCTV camera had removed the
power supply for NO.2 ECDIS station and replaced it with the CCTV monitor power supply
without informing the ETO or the 2nd Officer. When the 2nd officer came to do the weekly
update of the ECDIS the next day he found that the NO.2 ECDIS was not powering on and even
the UPS was completely drained out.
The technicians were instructed to inform the ships officers or the ETO regarding any changes
they require to be made in the system. If they had informed the ETO, he would have provided
them with an extension for connecting the CCTV power supply.
The matter was brought up with the yard safety officer and the ship repair manager, they had a
meeting with all the contractors and technicians and informed them to take permission from
ship staff prior to commencing any job.

20. One yard employee observed using hammer and chisel to cut U-Clamps on the electrical conduit
pipe without using safety gloves.
The job was stopped and the person in question was asked to wear safety gloves before
commencing the job again.
The yard safety officer was informed and the matter was addressed in the morning tool box
meeting. All persons to strictly comply with yard and ships safety policies regarding use of
proper PPE for all jobs.

21. Lots of empty paint and thinner drums were noticed stowed outside of the paint store, this
could be a fire hazard.
Paint drums cleared and disposed off in the garbage skip provided by the yard.
All ships staff advised to inform the chief officer or 2nd engineer of any scrap or garbage items
that are no longer required onboard so that they may arrange for disposal to the yard.

22. During deballasting operations on the repair berth, the vessels draft was decreasing and it was
noticed that the clearance of the shore gangway and the ships railings was decreasing. If not
adjusted in time the gangway would rest on the railing and might damage it as well.
The yard safety officer informed immediately and the gangway adjusted to increase the
clearance of the gangway and the ships railings.
Deck watch advised to monitor the situation regularly and to inform the duty officer so that
proper action can be taken in good time to avoid any accident or damage to ship or personnel.

23. Very poor housekeeping of the public toilet on upper deck observed during vessel stay in the dry
dock. People were noticed entering with their dirty safety shoes, floor was wet and dirty, tissue
paper lying on the ground and whole toilet smelling of urine. Poor hygienic conditions
maintained.
The public toilet was cleaned by ship staff and sanitized with bleach.
The public toilets onboard were locked on masters instructions and the yard staff were informed
to use only the public urinals which had been provided on the deck by the dry dock.

24. it was noticed that someone had eaten their dinner in the duty mess room and had kept their
plate of leftovers uncovered which was attracting flies.
the plate was cleared and the leftovers disposed off into the garbage bin.
All personnel especially galley staff advised not to leave any food uncovered in the mess rooms
and ti maintain good housekeeping at all times to ensure a safe and hygienic vessel.

25. Spare Magnetic compass was stored in normal working condition not in up side down position.

26. Tools and equipment’s observed on the top of the EEBD box in the E/Room work shop
All tools and equipment’s removed and EEBD was made clear and free from all obstructions
All Staff members instructed that all LSA & FFA equipments to be kept free from all obstructions
and ready for immediate use.

27. Empty plastic bottles were lying in the foam room


All Plastic bottles removed and foam room cleaned
Al staff members instructed to dispose off the plastic bottles after use and comply the shipboard
garbage management plan.

28. Fire door of the laundry was observed open during night hours safety round
Fire door of the laundry was closed immediately
All staff members instructed to keep the laundry fire door closed

29. All slots of the ashtrays were full in the smoke room.
All the cigarettes disposed off and astray cleared
All staff members instructed to clear the ashtrays and keep them ready for use

30. Coffee cups were observed dirty lying on the bridge


All the cups were cleaned and secured neatly
All bridge team members instructed to clean their coffee cups after use and maintain the good
hygiene.

31. Unsecured empty paint drums and buckets lying outside the paint store.
All paint drums and buckets secured in the store
Deck staff instructed to maintain the good housekeeping practices and not to leave the
unsecured paint drums outside the store

32. Two personnel’s observed giving signal to the crane operator


Person stopped immediately and instructed that the person in charge of the operation will only
give signal to the crane operator
All deck staff instructed that only one person should signal to the crane operator

33. Some pieces of card board, plastics and garbage observed in the foam room
Foam room cleaned and all garbage removed.
All staff members instructed not to leave any garbage in the foam room and keep the area
cleaned.

34. MSDS for low & High Expansion foam were missing from the foam room
A copy of MSDS for low & High Expansion foam posted in the foam room
All staff members instructed not to remove MSDS from its locations

35. Coffee spill observed in the crew recreation room - a slip hazard
All spill cleaned immediately
All personnel instructed not to cause any coffee spill, if caused clean it properly

36. One technician seen walking within the accommodation with his dirty safety shoes.
The technician was told to remove his dirty shoes in the changing locker on sunken deck and use
only clean shoes within the accommodation.
All crew advised to maintain good housekeeping at all times.

37. Foam was observed leaking out of the washing machine in the ships laundry. Upon opening, it
was discovered that the load was very small and excessive soap was used for cleaning.
Soapy water drained from the washing machine and area cleaned.
All crew advised only to use small quantities of detergent and to use the washing machine only
when there are sufficient amount of clothes to wash in the interest of conserving energy and
water.

38. Clothes iron found switched on and left unattended in the ships laundry.
Iron switched off and allowed to cool down before securing.
All crew advised to switch off electrical equipment after completion of the job at hand,
especially if it can be a fire hazard.

39. Some WBT manhole cover handles rusted or broken making it difficult to lift / handle.
New handles fabricated in Dry Dock.
Increase frequency of inspection and maintenance.

40. IMO sign for "wear face shield" in engine room workshop found in poor condition.
IMO sign replaced with new one.
Increase frequency of inspection by responsible officer.

41. Several wheel spanners for fire / foam hydrants on deck we found missing from their location.
All wheel spanners found and stowed in the proper locations near each foam / fire hydrant and
ready for immediate use in an emergency.
All crew advised to put tools back in proper location after use and responsible officer to increase
frequency on inspection.

42. One crew member did not report to the bridge after exiting from the pump room.
OOW contacted the said crew member by Walkie talkie and confirmed his location.
All crew members advised to comply with enclosed space entry procedures at all times when
entering / exiting pump room.

43. One crew member observed mixing paint without wearing goggles.
Crew member stopped and asked to wear goggles before proceeding with job at hand.
All crew members advised to wear proper PPE and to comply with the COSWP at all times.

44. Mid ship store door not secured with locking pin and was swinging during rolling of vessel.
Mid ship door secured with locking pin
All crew advised to secure doors with locking pins when opened even if vessel is not rolling.

45. One of the fire hose being use in the E/Room for routine washing and cleaning

46. Safety torches/Lamps observed missing from fire locker

47. Cardboard boxes and other garbage observed to be collected and stored in the C- Deck
stairways-obstruction to the safe passage.

48. Nozzles and F-Keys observed missing from the fire hose boxes on deck
All the missing items from fire hose boxes located and placed

49. One of the Eye wash observed missing from the station near the boiler platform
A New Eye wash bottle placed at the station.
All personnel instructed to report immediately if any of the eye wash being used

50. One of the person observed to be entering in the Bosun store without running the exhaust fan
of the store.
Person stopped immediately and instructed to run the fan prior entering in to Bosun store

entering in to space

51. One of the person fails to report the bridge after exiting the pump room

53. Bosun store weather tight door observed open during night hours

54. Wheel type portable trolley was observed unsecured on E/R 1st deck.
Trolley was secured immediately by the observer.
All personnel were instructed to keep the wheel type trolley secured.

55. Accommodation Entrance weather tight door port side observed open during night hours
Accommodation Entrance weather tight door port side closed immediately
All Staff members instructed to keep all the weather tight doors closed during night hours

56. Upper deck port side accommodation entrance was obstructed by some tools and equipment’s

57. During ums period found wet rags (in diesel oil) in the bucket in ER workshop.

58. ER railings on stairs between ER 2nd Floor and ER 3rd Floor found oily.
Oily railings cleaned by dry rags.

59. One of the crew member observed carrying out cleaning with compressed air without proper
eye protection.

60. Paint drums observed kept open on E/R 2nd deck.


All paint drums properly closed and transferred to the paint locker & secured properly.

61. One of the crew member observed entering E/Room without proper ear protection
Person stopped immediately and instructed to use proper ear protection prior entering the
E/Room.

62. Pumproom entrance weather tight door observed to be open during night hours

64. Small Water spill was observed on the A-Deck alleyway near water fountain - A Slip hazard
Immediately wet floor sign was posted and floor dried up.
All personnel were instructed not to cause any water spill on the alleyways and clean
immediately if noticed.

65. E/Room floor was painted but notice sign was not displayed to avoid the fresh painted area

66. Unattended plastic water bottles observed on deck after working hours - a threat to

67. Pumproom fan observed to be running during night hours when it was not required at sea

68. C-Deck Port side outer passage exit way observed to be obscured by Basket ball Net
Basket Ball net removed imediately and Emergency Exit cleared.
All staff members instructed to remove the basket ball net when not in use.

69. One of the crew member observed using grinder and buffing machine while wearing cotton
gloves
Person instructed to remove the cotton gloves and use proper PPE suitable for the machine
All personnel instructed to use proper PPEs at all time.

70. One of the crew member observed walking with shoe laces open.
Person instructed to tie up the shoe laces immediately
All personnel instructed to wear the PPEs properly

71. One of the person observed working near hot machinery without gloves.

All personnel instructed to keep the paint drums always in the paint room only upon completion
of job.

76. Welding was being carried out in the E/Room shop with curtain open
Curtains were closed immediately
All staff instructed that curtains to be used while being carried out in E-Room W/Shop
77. One of the crew member observed cleaning the surface with compressed air without proper eye
protection
Person stopped immediately and instructed to use proper PPEs
All person instructed to use proper PPEs at all times

78. Unattended and open paint drum observed in E/room 2nd platform.
Paint drum was transferred to the paint locker and secured properly.
All E/R staff were instructed not to leave the paint open and unattended in machinery spaces
and all the paints to be transferred to the designated paint store upon completion of job.

79. ER railings and stairs between 2nd and 3rd floor observed wet due to condensation from vents -
A slip hazard
All the stairs and railings cleaned immediately
All E/room staff instructed to clean the wet area immediately and maintain safe working
condition at all times

80. One of the crew members observed on the stairs with both his hands engaged carrying some
equipment and tools.
Person stopped immediately and assistance rendered as required
All staff members instructed that three-point contacts should always be maintained while on the
stairs-Comply COSWP and maintain safe working environment at all times

81. One of the staff member observed opening M/Eng. indicator cocks for cleaning before
performance, without using proper face /eye protection.
Person stopped immediately and instructed to use proper PPEs, face shield/eye protection.
All personnel instructed to use proper PPEs at all times appropriate to the work

82. One of the crew member observed operating ER Crane without wearing helmet.
Person stopped immediately and instructed to wear proper PPEs.
All personnel instructed to use proper PPEs at all times appropriate to the job and circumstances

83. One of the staff member observed in E/Room without proper safety shoes

84. One of the crew member observed in the E/Room with open shoe laces.

86. Dedicated firefighting hose from hose reel in ECR deck being used for the cleaning of soot
collection tank

87. Pumproom fan observed running after normal working hours while no one entering the
pumproom
Confirmed with HOD and OOW and Pumproom fan stopped
Al personnel instructed to stop the pumproom fan when not required or after the working hour
when no one is expected to enter the pumproom

88. ONE SHORE PERSON LEFT THE ACCOMMODATION DOOR OPENED DURING CARGO OPERATION.
THE ACCOMMODATION DOOR CLOSED.
ALL SHORE PERSONS INSTRUCTED TO KEEP ALL ACCOMMODATION DOORS CLOSED ALL THE
TIME DURING CARGO OPERATION.

89. One crew member wearing improper foot protection inside the accommodation.
Crew member was stopped and proper foot protection worn.
All crew members instructed to follow company policy all the time.

90. One eye wash bottle in the E/R workshop found opened and still in use.
The eye wash bottle removed and replaced by new one.
All crew members encouraged to report about any defects related to safety to the safety officer.

91. Plastic bottle found thrown in the galley general garbage bin.
Plastic bottle removed and disposed correctly.
All crew members informed and instructed to follow company policy and garbage management
plan all the time.

92. SOME WEIGHTS AND DUMBELLS FOUND NOT IN PLACE AND NOT WELL SECURED INSIDE THE
GYM.
All weights and dumbbells properly placed and secured.
All crew members instructed to keep all weights and dumbbells in its location and properly
secured.

93. One crew member observed walking down the stairs in the E/R with portable blower in both
hands.
He was stopped by the duty engineer and advised to use the hand rail.
All crew members instructed to keep always one hand free to avoid any injuries.

94. One crew member kept paint drum on the bottom platform in the E/R.
The paint drum removed and stowed in the correct location.
All crew members advised to put back any tools/materials used in the correct location.
95. One crew member was sighted walking in the E/R with open shoe laces.
He was informed to tie his shoes laces properly.
All crew members were instructed to follow a safe working practice and were PPE properly in
order to avoid any trip hazard.
96. One bag with oily rags kept near incinerator instead of putting inside a drum.
The oily rags disposed into special drum.
All crew members instructed to pay more attention while using oily rags in order to avoid any
fire hazards.

97. One crew member was standing near handling machinery during crane operation.
He was stopped and advised to keep clear of any handling machinery operation.
All crew members instructed to keep clear from the crane during handling operation in order to
avoid any injuries.

98. Tools found inside one of the fire boxes on deck.


Tools removed from the fire box and stowed in the correct location.
All crew members instructed to keep all safety equipment’s clear from any obstructions and
ready for immediate use.

99. One fire hose nozzle on deck found not moving freely when tested.
The fire hose nozzle replaced by new one and repaired.
All fire hoses and fire nozzles to be checked and maintained at regular interval in order to be
ready for immediate use.

100. One fire hydrant f-key found not in place on deck.


Fire hydrant f-key placed to the hydrant.
All deck crew reminded about the importance of keeping the safety equipment’s in place and
ready for immediate use.
101. BUCKET OF TOOLS FOUND LYING ON THE FLOOR IN THE E/R WORKSHOP.
THE BUCKET HAS BEEN REMOVED AND TOOLS STOWED IN THE CORRECT LOCATION.
ALL ENGINE CREW INFORMED ABOUT THE CASE AND ADVICED TO KEEP ALL AREAS CLEAR FROM
ANY OBSTRUCTIONS.

102. During fire drill, one of the ship staffs observed that not aware of his duties.
Concerned crew was instructed his duties as per muster list.
All ship personnel were instructed to ensure aware and familiarize of their duties.

103. Magnetic compass was not covered after completing magnetic compass curve
adjustment.
104. Gyro repeaters in the port bridge wing was found not covered after taking compass
bearing.

Gyro repeater was covered by the observer.

All OOWs were advised to cover the gyro repeater after taking compass bearing.
105.While landing scrap item at MAA, one of the ship staff was found standing beneath the
crane hook while transferring heavy items to service boat.
Concerned crew was advised to stand well clear.
All ship personnel were advised that never stand beneath crane hook while any load is
suspended.

105. Messenger rope was kept at the entrance of the forecastle store making access
obstructed.
Messenger rope was removed and made access clear.
All ship staffs were advised to ensure all access is unobstructed while storing / keeping any
things in the location.

106. One of the ship personnel was found not wearing safety goggle while mixing paint with
thinner.
Concerned crew was stooped and told to wear safety goggle.
All ship personnel were instructed to wear proper PPE for the jobs at all times.

107. Air hose was obstructing the safe passage in the E/R floor.
Air hose was removed and made passage clear.
All ship staffs were advised to ensure equipment’s to be secured after completing the jobs and
when storing / keeping any things, check for the passage to be free.

108. While safety round in E/R, one drum filled with water was found unattended on the 3rd Deck.
Water was removed and drum was secured.
All ship staffs were advised to ensure proper housekeeping to be maintained at all times.

109. Inflatable lifejacket was found left on deck exposed after using it for rigging combination
ladder for Marine advisor.
Inflatable lifejacket was put back inside midship store.
All ship staffs were instructed to ensure all LSA/FFA to be stored in safe location and to be
secured after using it.
110. During the Abandon ship drill it was observed one crew member mustered without
immersion suit.
Concerned crew member was told to bring immersion suit.
All ship personnel were instructed to mustered always with proper PPE for the drill.

111. It was observed few plates and mugs are not properly secured in Officer's pantry.
Plate and mugs were secured properly
All crew members were explained to keep everything secured while the vessel is out at sea and
to wash their plates and mugs after use.

112. Nozzles and F-Keys observed missing from the fire hose boxes on deck
All the missing items from fire hose boxes located and placed
All staff members instructed not to remove any item from fire hose boxes as they are dedicated
for firefighting.

113. One of the person observed to be entering in the Bosun store without running the
exhaust fan of the store.
Person stopped immediately and instructed to run the fan prior entering in to Bosun store
All personnel instructed to run the fan provided for the ventilation in the Bosun store prior
entering in to space

114. Banana peel was found in general garbage bin.


Banana peel was segregated and disposed in food waste garbage bin.
All ship staffs were advised to follow garbage management plan at all times.

115. During the rounds on deck before the watch it was observed that some empty mineral
water left lying on deck unattended.
Mineral water bottle was removed by the observer.
All ship personnel were instructed to maintain good housekeeping all around the vessel as due
to wind bottle may fly overboard.

116. During the routine inspection round observed oil can lying on top of EEBD box in engine
room workshop.
Oil can remove and stored appropriately
Matter discussed with everybody during closing toolbox meeting and everybody made aware
not to obstruct any of the safety items.

117. It was observed while removing hot dish from the oven crew member removing
aluminum foil using bare hand. chances of burn due to steam
Crew member was stopped immediately and asked to wear gloves
Matter discussed with everyone during closing toolbox meeting and everybody made aware of
the correct procedure and KOTC safety culture

118. During the routine inspection round it was observed one of the fire extinguisher in
engine room obstructed with garbage bag.
Garbage bag was removed and Fire extinguisher made easily accessible.
All other areas checked. Matter discussed during the tool box meeting and everybody made
aware of importance of keeping LSA and FFA equipment in readiness.

119. Cabinet door for Bridge publications drawer was observed left open whilst vessel was
rolling.
Cabinet door was closed by the observer.
All OOW were advised that ensure cabinet door is closed after taking any publication in order to
avoid damage.
121. Door of the microwave oven was observed open in the crew pantry, vessel was rolling
slightly at that time.
Door was closed by the observer.
All ship staffs were instructed to proper use of the all equipment on board to avoid damage of
the equipment.
120. Telephone receiver was not put properly in the Bosun store after speaking,
consequently auto telephone alarm received on BNWAS panel.
Bosun was contacted and advised to put receiver properly.
All ship personnel were advised ensure to put telephone receiver properly after using it.

One of the spare battery for the safety torch of Firefighting locker was found expired.
Spare expired battery was replaced with new one.
All safety equipment’s to be frequently checked and any expired items to be replaced
immediately.
