The American Association of endodontics defines five type of tooth fracture. CRAZE LINE,
FRACTURED CUSP, CRACKED TEETH ,SPLIT TEETH and VERTICAL ROOT FRACTURE .
Craze line and fractured cusp usually only require minimal or preventive clinical intervention; the
planning and prognosis are straightforward.
split teeth and vertical root fractures, although they require extraction because of their hopeless
prognosis are similarly clear-cut in understanding the tooth’s fate once diagnosed correctly.
crack teeth, unlike the previous mentioned type of fracture pose a unique problem in both diagnosis
and prognosis.
A cracked tooth is defined as a incomplete fracture initiated from the ground and extending sublingually
Even though the incidence of crack teeth varies among studies, there are some common factor that
increase the likelihood of having crack .
for example large amalgam restoration of mandibular molar are frequently associated with crack in the
past few decades. Multiple treatment have been advocated for managing cracked teeth, but an accurate
and reproducible method for diagnosing the extent or severity of the crack still lacking.
Accordingly, the treatment decision for cracked teeth is often based on personal experience and
anecdotal experience. In pioneering study by krell and Rivera
Cracked teeth that were diagnosed with reversible pulpi‘tis were treated by placing a crown without
endodontic intervention
About 20% of the samples progressed into irreversible pulpitis or pulpal necrosis within a sixth month
period

however, an exception was made to include any immediate failure that occurred within one year. The

endodontic treatment was only recommended when the crack was limited to the pulp chamber. In

medical record system. 64 were found to have clear description and observation visible with high

examination record.

expansion of a previous radiolucency at the

RESULT
62 sample met the inclusion criteria and were included in the study. The mean age of the collected sample
was 51 year old consisting of 27 men and 35 women. The most commonly observed teeth were molar 85%.
Most of the sample were previously restored with amalgam in the most common restorative material. The
most common and peripheral diagnosis in the correct teeth was pulp necrosis with symptomatic apical
periodontitis, 38%. This was followed by a visible colitis with symptomatic apical Peone -itis 616%
percussion sensitivity was the most frequently observed symptom in both diagnosis group and was present
in 60% of all sample more than 1/ third 39% had biting pain, and 27% were palpation sensitive. The overall
tooth success rate was 75.8% with a 23.3 month mean follow-up period . And minimum follow-up time was
one month, which accounted for two immediate failure cases.
SUMMARY
Although a crack has a negative impact on root canal treated teeth, the
endodontic treated teeth with definitive full coverage restoration had a
promising two year success rate
