ESSENTIAL INTRAPARTUM NEWBORN CARE

(Linear, Sterile, Perineum, Time, Dry, Breathing, Initiate, Remove, Clamp, Oxytocin, Placenta, Breastfeed, Administer, Measure)

Introduction: Goodmorning, ma’am, I am student nurse ______________, and I will be the


one to deliver your baby. So how are you, ma’am?

1. Lays out material in linear manner.


2. Wears sterile gloves. (double gloving)
Nurse: First, I am just transferring the kidney basin to the sterile area.
Nurse: I am putting on double gloves to minimize infection risk, then I will lay out my
materials in a linear manner.

3. Supports the perineum.


Nurse: I am now supporting the perineum with gauze pad to prevent tears.

4.Calls out the time of birth and sex of the baby


Nurse: Okay! BABY GIRL/BOY OUT ___ : ___ AM/PM.

5. Dries thoroughly the baby for full 30 seconds using the 1st towel
5.1. Follows an organized sequence: Wipe eyes, face, head, front and back, arms
and legs.
5.2. Wipes gently, do not wipe off the vernix
5.3. Removes the wet cloth, replace with a dry one
Nurse: I am now drying the baby while it is prone on the mother’s abdomen.
Do not wipe the vernix.
Nurse: Eyes, face, head, front, back, arms, and legs.
Nurse: I’ll cloth the baby with a dry one

6. Performs a rapid assessment of the baby's breathing


(Nurse rubs baby’s back)
Nurse: Baby’s breathing is stable.

7. Initiates immediate skin-skin contact.


7.1. Positions the newborn prone on the mother's abdomen or chest
7.2. Covers the newborn's back with a dry blanket
7.3. Covers the newborn's head with a bonnet
Nurse: Initiating skin contact to promote bonding. Put the bonnet on the baby’s
head.

8. Removes the 1st set of gloves prior to cord clamping and cutting.
9. Clamps and cuts properly timed cord, between 1-3 minutes.
Nurse: I am removing my first glove for the cord and clamping. Preparing to clamp
and cut the cord after 1-3 minutes or until pulse stops.

10. Injects Oxytocin 10 IU IM to the mother's deltoid.


Nurse: Injecting Oxytocin 10 IU IM into the mother’s deltoid to prevent hemorrhage.

11. Checks on the mother's condition and delivers the placenta.


11.1. Checks how heavy her bleeding is and examine the perineum, vagina and
vulva for tears.
11.2. Cleans the mother and keep her comfortable
11.3. Checks the perineum of the mother to assess for bleeding and tears
Nurse: Now assessing the mother's condition of bleeding and delivering the
placenta.

12. Initiates breast feeding for the 1st 30-60 minutes.


12.1. Encourages the mother to latch her newborn towards her breast
12.2. Counsels on attachment and positioning.
Nurse: I am now positioning the baby to latch onto the mother’s breast for
feeding, at least 30 to 60 minutes. The chin should be touching the breast and
the baby should breathe properly. The mouth should occupy the whole areola

13. Administers eye ointment, Vitamin K, Hepatitis B, and BCG, after the baby
completes her breastfeeding.
Nurse: After breastfeeding, I am going to transfer the baby on another table to
administer erythromycin, Vit K, Hep B, and BCG vaccines.

14. Performs anthropometric measurements.


14.1 Head circumference: 33-35 cm.
14.2 Chest circumference: 30-33 cm
14.3 Abdomen: 27-30cm
14.4 Arm:
14.5 Thigh:
14.6 Length: 47-50 cm
14.7 Weight: 2,500-4,000 grams
Nurse: I will perform the anthropometric measurements.
Nurse: Head is 34, Chest 31, Abdomen 28, Arm 11, Thigh 12, Length 49,
Weight 3 kg.
