Safety Observation Reporting Form
OBSERVATION DETAILS
Date and time of the OCTOBER 31,2025
Observation : 7:45 AM
Location of the Observation : WTG ACCESS ROAD /ALABAT
During the transportation of the Cylindrical Tower S4 along the
WTG access road, it was observed that the trailer truck driven by
Mr. Diao and logistics team encountered difficulty while
approaching a steep section of the road. Suddenly, the joint pin
connecting the truck head and the multi-axle trailer bed snapped,
resulting in the trailer rolling backward at high speed.
The driver immediately applied emergency braking and steering
control, while the assisting crew alerted nearby workers to move to
Describe the observed safety. The trailer came to a stop after being blocked by a mound of

soil, preventing it from falling into the adjacent embankment.
Personnel on site demonstrated quick response, situational
awareness, and effective communication, which helped prevent
: potential injury or major equipment damage.

DESCRIPTION OF OBSERVATION
During transportation of WTG components to the wind farm area, the
Provide a detailed description trailer truck carrying the Cylindrical Tower S4 was approaching a steep
of the observed situation or
section of the access road when the joint pin connecting the truck head
and multi-axle trailer bed suddenly snapped. This caused the trailer to
behavior
roll backward at high speed, but was fortunately stopped by a mound of
: soil, preventing a potential fall over the embankment.
POSITIVE ASPECTS
The driver and crew responded quickly and remained calm during the
incident, avoiding panic.
The presence of soil embankment acted as a natural barrier that
prevented the trailer from toppling over.
Identify any positive aspects or Immediate coordination and communication with the HSE team and
safe practices observed logistics supervisor were carried out.
Safety awareness and quick decision-making prevented further damage
or injury.

AREAS FOR IMPROVEMENT
[Link]-Trip Equipment Inspection:
Strengthen inspection procedures for all trailer connection
components, especially joint pins, couplers, and locking mechanisms
before dispatch.
Require dual verification and documentation by both the driver and
mechanic prior to movement of heavy loads.
2. Maintenance Program:
Implement a more frequent preventive maintenance schedule for high-
load transport equipment to detect early signs of wear and tear.
Identify any areas for
improvement or potential 3. Route Assessment and Planning:
safety hazards observed
Reassess and improve the WTG access road conditions, particularly
steep and uneven sections, to ensure safe navigation for heavy
vehicles.
Include risk control measures such as barriers, soil embankments, and
slope reinforcements.
4. Emergency Preparedness:
Conduct regular emergency response drills specific to transport-related
mechanical failures and brake loss scenarios.
RECOMMENDATIONS
[Link] Pre-Dispatch Inspections:
Conduct a comprehensive inspection of all trailer coupling
components, including joint pins, bolts, and locking systems before
every trip.
Provide recommendations for
addressing the identified areas Implement a checklist-based approval system to ensure mechanical
for improvement or potential readiness prior to loading and dispatch.
safety hazards
2. Upgrade Equipment Components:
Replace existing joint pins and connectors with high-grade, load-rated
materials suitable for steep and uneven terrain.
:
Coordinate with the equipment supplier for engineering review and
possible design improvement of the coupling assembly.
3. Improve Route Safety Controls:
Conduct a technical assessment of the WTG access road and enhance
safety barriers, slope stability, and traction control.
Install warning signage, wheel chocks, and slope indicators at critical
sections.
4. Enhance Personnel Competency:
Schedule refresher training for drivers, riggers, and transport crew on
defensive driving, load management, and emergency response.
Encourage a proactive safety culture by recognizing crews that identify
and report potential hazards before accidents occur.
5. Strengthen Monitoring and Oversight:
Assign an HSE or logistics representative during all heavy-lift
transport operations for real-time supervision.
Review and update the Transport and Lifting Safety Procedures to
include lessons learned from this incident.
ACTION TAKEN
[Link] trailer truck was immediately secured and stabilized to prevent
further backward movement.
2. The driver and crew were safely evacuated from the area, and no
injuries were reported.
Describe any immediate action 3. The incident site was cordoned off, and warning signs were placed

be taken based on the
4. The HSE Officer and Maintenance Team conducted an on-site
observation
inspection to assess the extent of the mechanical failure.
5. The damaged joint pin was removed and replace by new pin.
6. Transport operations were temporarily hold until the equipment was
inspected and cleared for use.
:
7. A toolbox meeting and safety briefing were conducted with all
logistics personnel to discuss the incident and reinforce preventive
measures.
8. The incident was formally reported to the Project Management and
HSE Department for documentation and review.
FOLLOW-UP
Will a follow-up be conducted
to ensure that the
recommended actions are
implemented? : YES NO
If yes, specify the person(s)
responsible for the follow-up : Health Safety and Environment (HSE) OFFICERS
ADDITIONAL COMMENTS
The quick response and situational awareness of the driver and support
crew were commendable, preventing what could have been a serious
accident.
The incident highlights the critical importance of mechanical integrity
and the need for strict compliance with pre-trip inspection protocols.
Continuous improvement in route maintenance and equipment
reliability is necessary, especially in challenging terrain such as the

information or comments you
would like to include? The lessons learned from this event should be shared across all
transport teams to strengthen safety culture and prevent recurrence.
Management support is recommended for procurement of upgraded
trailer coupling components and for the implementation of enhanced
: HSE monitoring during transport operations.
