CHAPTER 1
Establishing A Relationship
The nurse-patient relationship enables nurses to spend more time, to
connect, to interact with their patients as well as to understand
their patient's needs. It assists nurses to establish a unique perspective
regarding the meaning of the patient's illness, beliefs, and preferences
of patients/families.
Example

Nurse : Good morning.


Mrs Liony : Good morning, Nurse.
Nurse : How are you this morning ?
Mrs Liony : Not very well, I think.
Nurse : I’m Nurse Adi, I will take care of you
today. Your name is Mrs Liony Alexandra, isn’ it? How
can I address you?
Mrs Liony : Please call me Liony.
Nurse : Mrs Liony, If you need my assistance,
please just call me. I will help you.
Mrs Liony : Ok, Thank you nurse.
Nurse : Now, It’s time for me to visit other
patients. I’ve really got to go. See you.
Mrs Liony : See you.

Asking & Showing Rooms In The Hospital
OUTLINE :

Example

Woman : Excuse me. Can you help me,


please?
Nurse : What can I do for you?
Woman : I want to go to the ward room
number 404. But I can’t find it.
Nurse : The ward is in the 4th floor,
Ma’am. The room you’re looking
for is the fourth room
  from the elevator.
Woman : How can I get to the 4th floor?
Can I use the elevator?
Nurse : You can take the stairs or the
elevator for visitor.
Woman : Alright. Thank you.
Nurse : You’re welcome.
CHAPTER 3
Health Problem
Example
Nurse : Did you call me?
Patient : Yes, I did. I’ve body ache and mild fever since I woke up this morning.
Nurse : Let me check.
Nurse : You’ve mild fever, but your blood pressure is normal. Nothing to worry about.
The doctor will come on morning round between 9:30 and 10:00 AM. Tell him about
this when he is here. In the meantime, you can have your breakfast.
Patient : OK.
Attendant : Are there more tests the patient needs to undergo and when will the
patient be discharged?
Nurse : Like I said before, blood test on fasting state will be done tomorrow, and the
patient will be discharged most likely day after tomorrow. However, doctor will take
the final call on the discharge date.

CHECKING VITAL SIGN
Learning Objectives
- To ask permission to take vital signs
- To record vital signs
Vital Signs:
- temperature (oral, armpit,celcius or farenheit)
- pulse (apical, temporal, femoral, radial, carotic, apex of heart, popliteal, dorsalis
pedis, posterior tibialis
-respiration
- BP (Blood Pressure: right arm or left arm)
It is now common practice lor nurses to communicate with thei patients as much
as possible when they are doing routine nursing tasks. If the nurses talk, the
patients become involved in their treatment. As the nurses i take the patients'
vital signs, it will be better if they say what they are going to do explain why they
are doing it and give the patients feedback
Example : conversation
I Need to Take your Vital Signs :
Nurse : "i need to take your vital signs to make sure that your
temperature, pulse, respiration and blood pressure are okay while you're
staying in our hospital"
Patient : "All right."
Nurse : "I'm just going to wrap this cult around your arm and pump
somel air into it so that I can read your blood pressure. Could you please
roll up your sle
Patient: "Right." (Patient rolls up his sleeve) "Like this?"
Nurse: "Yeah, that's fine." (Nurse attaches cuff and starts pumping)
Patient: "it feels , a bit funny"
Nurse: "Don't worry. It won't hurt you."
Patient: "What is my blood pressure?"
Nurse: "Your blood pressure is 120/80."
Patient: "Is it OK?"
Nurse: "Yes. It's normal."

DISCHARGE INSTRUCTION
Learning Objectives

Useful Expressions (Asking common Communicable Disease)
And example :

(Example: are you allergic to penicillin/antibiotic)
