& Tryout Procedure

Objective and scope
This procedure establishes the minimum requirements for the
lockout of energy isolating devices whenever maintenance or
servicing is done on machines or equipment. It shall be used to
ensure that
 The machine or equipment is stopped,
 Isolated from all potentially hazardous energy sources.
 Locked out before employees perform any servicing or
maintenance where the unexpected energization or start-
up of the machine or equipment or release of stored
energy could cause injury.
Fundamental Purpose
• Safeguard employees.
• Protect Farm equipment.
• Ensure work is carried out under the safest possible

conditions.
• Effective coordination.
• Cover special risks through LOTO & close Supervision.

Responsibilities of Authorized Person
LOTO:
To make sure that Electrical lockout / tag out & tryout procedure is
followed whenever a Maintenance / Electrical job is to be carried
out on equipment which requires Electrical Isolation.
Responsibilities
Overall responsibility of the Lock Out / Tag Out & Try Out
procedure lies with the area owner to ensure that all jobs are
carried out only after proper locks & tags have been applied.
Responsibilities
Locks will be installed by all work permit signing authorities (Departments) in
case of equipment requiring Lockout / Tag out & Tryout Procedure.
If job is performed on energized Circuit work permit needs co-signing by

What is a Lockout Device?
A device that positively:
Prevents a machine from being started

up or turned on,
Prevents a machinery part from moving,
Prevents electrical energizing,
Blocks a pipeline, steam line, Caustic

Acid line or air line
Personal Lockout System
–Each EFL employee must apply his own lock, tag and then
TRY the effectiveness of the lockout when doing
maintenance, or repair work. (If 10 EFL employees from
same or different interfaces are working on a machine at any
one time and there is only one lockout point, each employee
must apply his own lock (and tag) using a multiple locking
device.
• AM/ Area Operator
• The locks and tags of the Production interface/Area owner (i.e., the Primary
Authorized Employee) will be applied first and removed last.
• The lock and the local ON / OFF switch will then be tagged with a red and white
danger tag. This tag must provide the following information:
– Name of the equipment locked out
– Date and time of lock application.
– Reason for lockout
– Single or “Multiple Lockout” if multiple locks are being installed.
– Name of the Production/Area owner AM or Area Operator and his
signature.
Note: The lock number of Primary Authorized Person and Principle Authorized
Person receiving the permit will be mentioned on the Work Permit.
• Maintenance Person (Principle Authorized Employee)
• When the Production interfaces lock and tag has been installed
on the breaker/multiple locking device, the “Principle Authorized
Employee” to whom the permit has been issued will install his
lock and tag on the breaker/multiple locking device. He must
determine to his own satisfaction that the equipment is safe for
work by trying/checking that the relevant equipment has been
de-energized. ‘TRY’ step must be carried out in the presence of
the Post Operator
• It will be the responsibility of the “Principle Authorized Employee”
to ensure the safety of all the “Affected Personnel” who will be
working on the equipment

What is Tag-out?
Tags are warning devices only
They don’t provide the same level of protection as lockout devices.

We only use for information, along with a locked device.
They can only be removed by an authorized person.

Tag out
Danger tags should NOT be used

for other purpose. Their use
should be reserved for situations
where a possible danger to
personnel exists.
Work Permit Extended
• In case the Work Permit is extended, the name and

lock number of incoming maintenance technician
(Principle Authorized Employee) has to be
mentioned on the card copy of the permit and the
Principle Authorized Employee has to install his
personal lock and tag on the breaker in addition to
signing the card copy of the permit.
Locking Facility Not Available
– Any point where no Locking facility is available
due to design feature of the breaker, proceed as
follows
» Put off the relevant breaker & put a tag on it.
» Issue work permit to electrical department & get the
leads disconnected from the nearest point to the
equipment.
• If a Breaker cannot be locked out then Supply cable of
the breaker should be disconnected and fuse removed (if
applicable) prior to start the job.
Removing Locks and Tags
• Each individual working on the equipment will remove his
lock and tag from the breaker/multiple locking device when
the job is complete. In case he is reassigned to another job or
leaves the facility, he will remove his personal lock only. The
tag will remain installed and he must mention on the tag the
reason for removal of his lock while the job is incomplete.
Removing Locks and Tags
• Maintenance Person:
• The “Principle Authorized Employee” to whom the
permit was issued will remove his lock and tag.
• AM or his designated Post Operator
• Production/Area owner AM or his designated Post
Operator will ensure that all work on the equipment
is complete and locks have been removed before
removing his lock and tag.
Removal of Personal Lock by Supervisor
• In case hard copy of the work permit issued is not available,
Production AM/Area owner shall call the area Team Leader
and Maintenance Team Leader at site and carry out a joint
survey of the equipment that will be energized by the breaker.
Each Team Leader shall physically check and verify the safe
operating conditions and that no job is pending.
• The authorized employee’s Supervisor can remove the lock

using the spare key. No area Supervisor can remove the lock
of a person who does not report to his line organization. In
addition, the following must be recorded: ( may be on e-mail
in the incident report to the safety section).
Relamping within Offices, Halls, and
Operating Areas
• Lockout for re-lamping in offices, halls, and operating areas is not required.
The ON/ OFF push button should be switched off and tagged by Production
(or other area owner in case of non-operating areas) and I&E interface.
After completion of the job, the tags should be removed. However, if the
lockout facility is available, proper lockout / tag out must be carried out.
– For repair of the lighting circuit including bulb holders, following two
options should be used:
– Total power to the panel should be put OFF and locked / tagged at main
panel.
– The circuit leads should be disconnected from the output of the switch at
the panel
– For re-lamping at boundary wall and street lights etc., the relevant breaker
should be locked and tagged by I&E interface.
What kind of injuries can happen?
Electrocution from live parts
Burning from steam or hot liquids
Chemical burns or injuring
From machinery:
- Deep cuts and wounds
- Serious injuries
- All of these can be fatal when
severe
Work Permit Rule
Work permit must be written/ filled by Assistant Area
Manager or Team Leader in his own handwriting. No
Maintenance employee / area operator is allowed to write /
fill work permit checklist.
Only AM / Team Leader of Issuing area is allowed to write / fill the work permit as
per input / discussion with the maintenance person.
Electrical energy is a good servant but a
bad master if handled carelessly.
