Policy Guidelines to DepEd-Region I Employees who are
invited as a Lecturer/ Resource Person/ Facilitators/ Judge
on Seminars/Trainings/Workshops/Contests/
Conducted by Private Entities

Section 4 p#3:
As a general rule, all DepEd Region I officials and employees may opt
to attend and be a part of these seminars/ trainings / workshops/
contests conducted by PRIVATE ENTITIES. However, they are NOT
ALLOWED to attend these activities on OFFICIAL BUSINESS or
OFFICIAL TIME

Section 4p#4:
Employees who are invited and are interested to partake in this private
group discussions and events are advised to file their CSC-Form 6
( Application Leave) before attending.

Section 4p#5:
Employees are also
requested to prepare letter of confirmation addressed to that particular
private entities notifying them of their confirmation to attend the said
event, likewise, a letter of regret shall be prepared if the employee
cannot attend and participate in this event.

Rules on Correction of Entries in School Records
Rule 1:
Only requests for correction of entry from graduates may be processed by
the Regional Office.
Requests from currently enrolled pupils/ students may be processed
directly by the school.
Rule 2:
The following may be corrected in the school level without need of a
resolution from the Regional Office:
1. Error of entry in a school record brought about by manifest clerical
error/ typographical error;

2. Omitted middle name or suffix, i.e., “Jr.”, so long as it
appears in the certified true copy of the graduate’s birth
certificate;
3. Abbreviation of names or vice – versa like in the case of
“Ma.” to “Maria” or vice versa, from “ De La Cruz” to
“Dela Cruz” and vice-versa. Provided, that what should be
followed is how the name is spelled in the birth certificate;
4. Error in the day and month of birth;
5. Error in the sex/ gender;
6. Error in the name of parent/s; and
7. Error in the place of birth;

The following may only be corrected upon a


resolution issued by the Regional Office:

1. Correction in the spelling or change of the


graduate’s family name , first name, and/ or
middle name; and
2. Correction in the year of birth.

Rule 4

In any case, if a student/graduate is requesting a


correction of an entry by virtue of a correction of
his or her birth certificate affected by judicial order
or by a decision from the civil registrar, such
correction/s must already be annotated in the birth
certificate at the time of the request.

Rule 5
In any case, the personal appearance of the student/ graduate is
required together with a certified true copy of his or her birth certificate.
For graduates appearing before the Regional Office, the following
documents are also required:
1. A letter-request signed by the requester explaining the
circumstances of the discrepancy:
2. A duly notarized affidavit of two disinterested persons, not related
to the requester within the 4th degree of consanguinity , attesting to
the fact that the requester whose name appears on the birth
certificate is one and the same person as the appearing in the
school record;
3. A photocopy of the school record ( Form 137’ Official Transcript of
Record, and/ or Diploma) sought to be corrected;

4. Two valid identification cards;
5. For private school graduates, a certified true copy of the
Special order of graduation reflecting their name, and
6. For alien graduates or foreign-born graduates, they shall be required
to submit a certified true copy of their passport or any document issued
by the Bureau of Immigration or by the Department of Foreign Affairs.
Rule 6
If the request is meritorious, a Resolution shall be issued by the Office
ordering the school concerned to make the correction. A copy of the
Resolution shall be attached as an annex to to the school record
corrected.

Rule 5

REMINDERS OF THE FOLLOWING TIMELINES

Government Permit / Recognition
1. Request for Opening of New School or Operation of New
Grade Level by existing PE, E, JHS shall be submitted one
(1) year prior to its operation

a. Division Office- on or before June 30


b. Regional Office- on or before August 30

2. Request for Renewal of Permit and Application for


Recognition shall be submitted to DO for initial
processing and inspection before it is indorsed to the RO

a. For renewal permit- on or before January 2nd


b. For Recognition- on or before February 1st

3. Private School shall submit a Child Protection Policy

4. New Schools should pay school bond and inspection fee


at the Cash Section, DepEd Regional Office I.

5. The registered name of school with the Securities and


Exchange Commission (SEC) should be strictly followed.

6. Updated Retirement Plan

7. Certificate of Fire Inspection

TUITION AND OTHER
SCHOOL FEES
Tuition and School Fees

1. Separate application for new and increase of tuition and


other school fees with the requirements shall be
submitted to the Regional Office not later than May 15 of
every year.

2. Schools which do not increase their tuition fees and other


school fees shall inform the DepEd Regional Office I not
later than May 15 of every year. It should be
accompanied with a photocopy of the latest approved
fees.

Tuition and School Fees
3. The application for new and increase of tuition and other
school Fees shall include the following requirements:
 Notarized notice of advise on intent containing the schedule of fees.
 Photocopy of latest approved fees;
 Notarized Certificate of compliance on the incremental proceeds in
preceding school year;
 Notarized certification of consultation with duly organize student
government with PTA supported by minutes of the meeting;
 Financial Statement duly certified by Certified Public Accountant
 A copy of the last Tax Return filed with the Bureau of Internal
Revenues.

Tuition and School Fees

4. Before the application shall be indorsed to the Regional


Office, the Division Office should check the
completeness of the requirements.

5. The Regional Director will approve tuition and other school fees until
June 30 of every school year. Beyond the deadline the said
application will no longer be considered.

School Calendar

1. A Private School shall submit to the Regional Office


fifteen (15) days before the opening of classes of every
school year copy of the School Calendar indicating
therein the number of contact days per month and the
activities to be undertaken, based on the standard
school calendar issued by the DepEd Central Office.

2. School Calendar should be submitted to the Division


Office for Initial Processing.

SPECIAL ORDER (Dep Ed Order No. 10, s. 2018)

“GUIDELINES ON THE ISSUANCE OF SPECIAL ORDERS FOR THE


APPROVAL OF THE ELIGIBILITY FOR GRADUATION FROM SENIOR HIGH
SCHOOL OF GRADE 12 LEARNERS ENROLLED IN PRIVATE SCHOOLS/
COLLEGES AND TECHNICAL- VOCATIONAL INSTITUTIONS FOR
S.Y 2017-2018”

No. 4 # Application for special orders of qualified Grade 12 learners for SY


2017-2018 shall be filed by the school through the schools division office(SDO)
at least 15 calendar days before the end of the current academic year. This
shall be processed by the RO and returned to the school through the SDO
within seven(7) calendar days from the receipt of application. For PSO’s, the
same timeline applies for the application and the subsequent issuance of the
special order with the Bureau of Curriculum Development processing the said
application

SPECIAL ORDER (Dep Ed Order No. 88, s. 2018)

1. The Special Orders shall be required for the graduation of students from the
formal secondary level and the SO number is included/ written in the
Diploma. No special orders will be required for elementary school
graduates.

2. Application for special orders of graduating students from schools with


college offerings should be submitted to Regional Office at least sixty (60)
calendar days before the end of the academic year.

3. Application for special orders of graduating students from schools with


basic education offering only should be submitted to the Division Office and
approved by the Schools Division Superintendent.

4. The Division Office shall furnish the Regional Office copy of the approved
Special Orders.

Classes During Summer”

The school shall submit a letter of request to offer
remedial or advancement classes during summer to the
Schools Division Office( SDO) not later than the 3rd
week of March that precedes the summer term.
The SDS shall approve the request based on the results
of the evaluation. Approval of classes during sumer
shall be issued for the term or year applied for.
