• Direct the Population Health Improvement Program, a five year New York State health systems
transformation initiative designed to support shared measurement as well as assessment practices in the
six-county Capital Region which includes Albany, Columbia, Greene, Rensselaer, Saratoga and
Schenectady. The PHIP supports and advances the agenda of the State Health Innovation Plan, the
Prevention Agenda and the Design Systems Reform Incentive Payment programs. 50% NYSDOH

• Managed team implementation for accredited Second Annual Dental Medical Conference, a forum for
135 academic, medical and dental providers identifying leading edge models that integrate the practice of

alone division with a $3.2 million budget and 9 staff. Provided oversight and technical management of

appropriate permanent executive placement.
• Innovated and leveraged best practices for six community and faith based organizations in different states
to provide mentoring and social resilience skills for 1200 at risk Latino youth over two years and five
community and faith based organizations to provide HUD-approved housing counseling services to over
300 client families over two years in under-resourced communities in the multiple cities across the country.
