HARP Care Coordinator/ Link Clinician
The Care Coordinator role provides support to patients, carers and other professionals with the aim of building
the capacity of the individual, to improve their experience of chronic disease with the aim of reducing
unnecessary or inappropriate Emergency Department visits. Responsible for providing comprehensive care
coordination to clients referred to the service, including undertaking assessments, designing intervention
strategies, implementing care plans and providing education for clients and families in a variety of community
settings. In conjunction with the Harp Link role the Care Coordination role was mainly short term in nature and
aimed predominantly to coordinate the services already working with the consumer or to redirect consumers
frequently presenting to the Emergency Dept at Rosebud Hospital, to more appropriate community supports.
The HARP Link Clinician conducts initial needs assessments and referral for patients eligible for HARP Care
Coordination who present to the Emergency Departments of Peninsula Health. This role also assisted in
identifying the frequently presenting consumers who may benefit from a Patient Management Plan (PMP).

Alcohol and other Drug Telephone Intake and Assessment Clinician
Provide telephone and face-to-face intake and assessments for consumers voluntarily accessing various AOD
services in regional Victoria. Develop comprehensive assessments and refer on to appropriate services, take
referrals from various community agencies including the forensic system, Gp’s, employment agencies, housing
