Required Documents to Submit on or before March 22, 2024  School Structural/Operational Plan (3 copies) for the approval of SDS (schools with Hot meals only)  Signed WFP and SOB (3 Copies) with ORS/DV  APP  MOA of cook/workers (notarized) 3 sets each  PDS and Medical Cert and Health Card of helpers 3 sets
