    One important element of the work gynecologist is counseling for the control of fertility. For the majority of patients the most important criterion for choosing a contraceptive method is its effectiveness. Therefore the individual choice of contraceptive method should first of all take into account the performance of the methods for the prevention of unwanted pregnancy. The real effectiveness of each method of contraception is the result of the effectiveness of the method and the exact compliance with medical recommendations as to its application and the factors independent of patient (eg antibiotic vomiting diarrhea). To reduce the risk of contraceptive failure should consider recommending methods whose effectiveness is very high even in the case of learning the context of misuse.

    AIM The aim of the study was to evaluate the usefulness of CA125 measurements in ovarian cancer diagnostics in pre- versus postmenopausal women. MATERIAL AND METHODS The study group consisted of 99 serum specimens of women diagnosed with ovarian cancer (28 pre- and 71 postmenopausal). The control group consisted of 86 specimens collected from women without any ovarian pathology (34 pre- and 52 postmenopausal). CA 125 measurements were performed with Zymed Laboratories CA 125 ELISA Kit. Descriptive statistics, including mean values, standard deviation (SD) and 95CI of CA 125 among pre- and postmenopausal women, were calculated. The usefulness of CA 125 measurements in ovarian cancer diagnostics was assessed by calculating specificity sensitivity positive and negative predictive rates among pre- versus postmenopausal women.     In 2004 the first livebirth after cryopreserved ovarian tissue transplantation has been reported which is a turning point in fertility preservation after anticancer treatment and a result of almost 50 years of research. However transplantation of cryopreserved ovary is still experimental method and further study needs to be done on the risk of cancer cell reimplantation, this method is very promising for cancer patients who wants to preserve fertility after cancer treatment. This is a review of methods used for cryopreserved ovarian tissue.

    Numerous data indicate that polymorphism of estrogen receptor alpha (ERalpha) may predict lipid levels, lipid response to hormone replacement therapy (HRT), myocardial infarction risk, bone fracture risk, bone mineral density (BMD) and changes in BMD over time. In this study we aimed to evaluate distribution of ERalpha PvuII and XbaI genotypes in population of Polish postmenopausal women qualified to different protocols of HRT. Subject of the study were 64 consecutive postmenopausal women aged from 45 to 65 years (mean 56.6) assigned to HRT. ERalpha PvuII and XbaI polymorphism was determined by PCR-restriction fragment length polymorphism (RFLP). The absence of PvuII and XbaI restriction sites were indicated by "P" and "X" and presence by "p" and "x", respectively.     A group of 59 twin pregnant women who gave birth at the Department of Obstetrics and Gynecology Central Clinical Hospital of Ministry of Interior and Administration in Warsaw in the years 2005-2006. The patients have been divided into four groups: spontaneous twin pregnant women (n=16), twin pregnant women after in- vitro fertilization (IVF) (n=11), twin pregnant women after in-vitro fertilization with intracytoplasmic sperm injection (ICSI) (n=29) and twin pregnant women after ICSI and transfer of frozen embryos (n=3). In one case intrauterine death of one of twins in the 34th week of gestation has been noticed. The cause of the death was umbilical cord wrapped around his neck. The gestation was ended with cesarean section and Apgar score of the second twin was 8 in the fifth minute.     The serum LH concentration was analyzed in 47 patients of reproductive age with diagnosed hypergonadotropic hypogonadism based on clinical status, pituitary hormone and sex steroids levels, karyotype, ultrasound scan, CT scan, laparoscopy and ovarian biopsy. The following groups were categorized: 46XY, 45X0, premature ovarian failure, WHO III group and iatrogenic symptoms after surgery. The mean serum FSH level was similar in each study group. The serum LH concentration was the lowest in 45X0 group compared to the highest values in 46XY and surgically treated groups. The LH/FSH ratio was close to normal in 46XY and surgically treated groups. We conclude the syndrome should be recognized mainly on basis of FSH level since the LH level shows heterogenic fluctuations. The serum LH concentration is close to normal for physiological conditions (postcastrated menopause, male genotype) but significantly lower in ovarian disorders and gonadal dysgenesis.

    The COVID-19 pandemic has challenged health systems around the world. Maternal-foetal medicine, which has been particularly affected, must consider scientific data on the physiological processes occurring in the pregnant woman’s body to develop relevant standards of care. Our study retrospectively compared the clinical and laboratory characteristics of 52 COVID-19 pregnant patients with 53 controls. Most of the pregnant patients required medical attention during the third trimester and therefore we propose that vaccination is needed prior to the 30th week of pregnancy. We found no differences between the 2 groups in the course of illness classification system, days of hospital stay, need for oxygen supplementation, need for mechanical ventilation, and ICU admission.     The COVID-19 pandemic outbreak influenced general and mental health worldwide. The objective of this study was to assess the anxiety level during the COVID-19 pandemic among pregnant women and compare it between COVID-infected and non-infected groups. We prospectively assessed the daily routine and anxiety level using a bespoke questionnaire and GAD-7 scale validated for pregnant women. With logistic regression, we established possible risk factors of generalized anxiety disorder spectrum and main causes of concern. The dataset included 439 responders of our survey. Of which, 21% had COVID-19 infection during pregnancy; 38% were screened for possible generalized anxiety disorder and the proportion was higher in women who suffered from COVID-19 (48% vs. 35%, p = 0.03).     Meconium concentrations of naturally accumulated ceruloplasmin (CER), lactoferrin (LF), and neutrophil gelatinase‐associated lipocalin (NGAL) and their relationships considered as a panel of acute phase proteins could be used for the assessment of fetal homeostasis. CER, LF and NGAL concentrations were measured using enzyme‐linked immunosorbent assay kits in meconium portions (n = 80) collected from 19 healthy neonates. The coefficients of variation (CV) of the meconium LF (1.77) and NGAL (1.26) were about two‐fold higher than that of CER (0.73) with significant (P < 0.05) correlations between all three parameters. The LF to NGAL ratio (CV = 0.67) correlated strongly with the CER concentrations (r = 0.39, P < 0.01). These correlations between CER, LF and NGAL concentrations suggest their combined involvement in the metabolic processes in the developing fetus.

    Dysfunctional uterine bleeding (DUB) occurs frequently in women at the reproductive age and is unrelated to structural uterine abnormalities. It significantly impairs the quality of life for many otherwise healthy women. Evaluation of patients with abnormal uterine bleeding and identification of those with DUB is based on medical records, physical examination, laboratory tests, uterine imaging and endometrial sampling. Surgical treatment options include hysterectomy and conservative surgery (endometrial resection or ablation). Medical therapy, with the avoidance of possibly unnecessary surgery, is an attractive treatment option. However, there is considerable variation in practice and lack of consensus regarding the most effective therapy.

    The ovarian hyperstimulation syndrome (OHSS) is still a difficult diagnostic and therapeutic problem. OHSS is associated with significant hypertrophy of the ovaries associated with the loss of the intravascular fluid to the third space which results in hypovolaemia, oliguria, electrolyte imbalance, and a rise in haematocrit. The endogenous OHSS is rare. Most often OHSS appears as a complication of induction of ovulation. The fundamental issue in pathophysiology of OHSS is an increase of capillary permeability which results in the leakage of fluid to the third space. The vascular endothelial growth factor--VEGF--is considered to be the factor directly responsible for the processes involved. The most common are the mild and moderate forms of the syndrome. The severe form of OHSS is a life-threatening condition. The following symptoms may be present: ascites, pleural and pericardial effusion, oliguria, dyspnoea with tachypnoe, tachycardia, adult respiratory distress syndrome, renal failure, venous thrombosis, ischaemic stroke, haemorrhage from a ruptured ovary. Therapy should be based on the correction of hypovolaemia, hypotension and oliguria. Antithrombotic prophylaxis is an integral part of the OHSS management. Some interesting attempts have been undertaken to re-infuse the protein-rich ascites fluid directly to the systemic circulation, so called continuous auto-transfusion system of the ascites (CATSA).

    Polycystic ovarian syndrom (PCOS) affects 5% to 10% of women of reproductive age, and is the most common reason of anovulation in infertile women. It is multi-symptom disease. A drug which is used to induce ovulation in women with PCOS is clomiphene citrate. After clomiphene treatment, ovulation is achieved in 56%-73% of women. The rest of them has different level of opportunity to therapy. Clomiphene does not have influence on elementary factor in PCOS which seems to be opportunity to insulin. The drug which can have positive effect is metformin, derivative from biguanid. Its influence on percent of ovulation, pregnancy and live births was evaluated based on randomized trials. Many trials have reserched this so far, but their results are divergent. In this article, we are aiming to systemize results of randomized trials concerning the role of metformin in PCOS therapy.

    Polycystic ovary syndrome (PCOS) is the most common reproductive endocrine disorder in women of childbearing age. Estimates of incidence based on ultrasound scans of normal populations suggest that up to 23% of normal women may have polycystic ovaries (1); however, not all of these women are anovulatory or require medical treatment. The majority of the women with ultrasound evidence of PCOS have irregular menstrual cycles (75%) and 75% of the women with regular cycles are hirsute. This is in contrast to the women with normal ovaries (no ultrasound evidence of polycystic ovaries) of whom only.9% have irregular cycles (2). Within the infertile population, approximately three quarters of women with anovulatory infertility have PCO, thus accounting for approximately one third of women with secondary amenorrhea and approximately 90% of women with oligomenorrhea (3). Clearly PCOS affects a large number of women and is associated with menstrual irregularities in a large proportion of women.

    Ovarian tissue preservation and transplantation are intended for women undergoing aggressive regimens of chemical and/or radiological therapy, bone marrow transplantation or stem cell transplantation. Main indications for the procedure are neoplastic diseases and autoimmune disorders. The first live human birth after ovarian tissue autotransplantation was successfully done in 2002. Cryopreserved ovarian tissue can be autografted either orthotopically or heterotopically. Neovascularization of the implanted tissue is essential for the procedure. Vascular transplantation seems to be the best approach for avoiding follicular loss and extending the lifespan of the ovarian grafts. The procedure, regardless of whether ortho- or heterotopic, is connected with a risk of reimplantation of neoplastic cells. This can be minimized by multiple ovary biopsies, thorough histological examination and molecular genetic techniques. Introducing ovarian tissue transplantation into clinical practice requires many problems to be solved. Standardization of the freeze-thaw protocol is one of the most important issues. Solving the problem of transient graft ischemia is also essential. Eventually, the future safety of the method requires the development of efficient tests to detect the presence of neoplastic cells in the transplanted tissue.

    This case report refers to a 53-year-old female patient who deliberately misinformed the staff of the in vitro fertilization (IVF) clinic using counterfeit documents concerning her real age and underwent the IVF procedure. During the pregnancy it was discovered accidentally that the patient was not telling the whole truth about her real age. In the 34(th) week of pregnancy two healthy twins have been delivered by a cesarean section. Somehow the trust between the patient and the doctor was compromised, however we can acknowledge this case of postmenopause pregnancy as a therapeutical success for the clinic and for the patient herself. The mother invested her time, emotions and financial resources, and risked her health undergoing IVF procedures and carrying the pregnancy to fulfill her wish and deliver a child. Regardless of her intentions, we can argue about her knowledge concerning this procedure and awareness of threats to pregnancy resulting from her age. The age of childbearing ...

Cortisol concentration in amniotic fluid was much higher in group PG. The mode of action of prostaglandin on fetal lung had been analyzed.     The free androgen index (FAI) was estimated and compared with intensiveness of hirsutism in ten anovulatory women of reproductive age referred for polycystic ovarian syndrome with LH/FSH ratio greater than 3.0. The control group consisted of five ovulatory women. The serum testosterone, SHBG, LH and FSH were measured by radioimmunoassay. The mean plasma level of hormones in the study group was following: T-5.8 +/- 1.9 nmol/l, SHBG - 51.0 +/- 18.0 nmol/l, LH - 18.5 +/- 6.0 mIU/ml, FSH - 5.0 +/- 1.2 mIU/ml, in comparison with the control group: T - 1.4 +/- 0.4 nmol/l, SHBG - 90.0 +/- 11.2 nmol/l, LH - 9.4 +/- 4.8 mIU/ml, FSH - 7.3 +/- 0.9 mIU/ml. FAI was calculated as T/SHBG ratio multiplied by 100. In contrast to the control group the seven times higher FAI did not accompany the intensiveness of hirsutism.

    Leptin, a product of adipocytes, is a cytokine with multiple effects on the reproductive axis. Leptin causes the activation of STAT proteins within target cells. The aromatase gene promoter in adipose stromal cells contains a functional STAT binding region, leading to the hypothesis that leptin may regulate aromatase activity in fat tissue. To test this hypothesis, adipose stromal cells were isolated from subcutaneous abdominal fat or breast fat then placed into tissue culture. The cells were treated for three days with increasing concentrations of recombinant human leptin. Aromatase activity in the stromal cells was measured by the release of 3H2O from radiolabeled androstenedione precursor. Basal aromatase activity varied markedly between, but there were no differences between abdominal fat and breast fat. Leptin concentrations in the physiological range of normal weight or thin women (10 ng/ml) had no effect on aromatase activity. In 2 of 8 abdominal fat cultures and 1 of 2 breast fat cultures, a high obese concentration of leptin (100 ng/ml) stimulated a significant increase in aromatase activity. In the remaining subjects there was no effect of leptin, even at high concentrations. These data demonstrate that in approximately 30 percent of our subject population leptin was able to stimulate aromatase activity in adipose stromal cells at high concentrations. The elevated levels of aromatase activity may contribute to increase circulating estrogen levels in certain obese women and suggest that elevated leptin concentrations in obese women may cause locally elevated estrogen concentrations in the breast and thereby promote tumor formation.

Descriptive statistics, including mean values, standard deviation (SD) and 95CI of CA 125 among pre- and postmenopausal women, were calculated.     Dysfunctional uterine bleeding (DUB) occurs frequently in women at the reproductive age and is unrelated to structural uterine abnormalities. It significantly impairs the quality of life for many otherwise healthy women. Evaluation of patients with abnormal uterine bleeding and identification of those with DUB is based on medical records, physical examination, laboratory tests, uterine imaging and endometrial sampling. Surgical treatment options include hysterectomy and conservative surgery (endometrial resection or ablation). Medical therapy, with the avoidance of possibly unnecessary surgery, is an attractive treatment option. However, there is considerable variation in practice and lack of consensus regarding the most effective therapy.

    The serum concentrations of sex hormones in appropriate days of the menstrual cycle, and of lipids in 15 ovulating women, were measured. The psoriatic patients were compared with a control group of healthy subjects matched for age, sex and body mass index (BMI). In psoriatic patients, the levels of measured hormones were lower than in the controls, with the exception of LH concentrations, which were increased but in a statistically insignificant way. The testosterone levels and LH/FSH ratio were significantly lower in the psoriatic group. The triglyceride concentrations were significantly higher in the patients, and this phenomenon was not related to their body mass.
