During the acute stage of a Wallenberg's syndrome ipsilateral appendicular dysmetria is frequently seen. The dysmetria is more apparent in the ipsilateral upper extremity. These patients also have a peculiar type of dysmetric eye movements that are characterized by hypermetric saccades toward the side of the lesion and hypometric saccades to the opposite side. We examined four patients with acute Wallenberg's syndrome and found horizontal dysmetria of the affected extremity. Hypermetric arm and hand movements were present to the side of the lesion and hypometric movements toward the opposite side. This type of dysmetria is probably related to the same patophysiological mechanism that underlies dysmetric eye movements in the Wallenberg's syndrome. The dysmetria tends to disappear with time although it continues to be present in some patients six months after the ischemic damage.

Patients with cerebral cysticercosis (CC) have specific antibodies of the IgG type in blood and cerebrospinal fluid (CSF). In order to determine if the specific antibodies present in the CSF come from blood or synthetized in the subarachnoid space, we studied the integrity of the blood brain barrier (BBB) and the intrathecal synthesis of specific antibodies. We found a ruptured BBB with more frequency and in a higher intensity in patients with CC and hydrocephalus (malignant CC) than in patients with parenchymal CC (benign CC). The patients with malignant CC have a greater number of inflammatory cells, a higher albumin index (serum albumin/CSF albumin), a higher level of protein and a higher titre of specific antibodies in the CSF than those with benign CC. In 15 patients in both groups we could demonstrate intrathecal synthesis of antibodies. We conclude that in CC the specific antibodies are produced intrathecally, and in a lesser amount come from the blood through a ruptured blood-brain barrier.

A 24 year-old woman with miliary tuberculosis of the lungs, developed a diffuse encephalopathy with signs of increased intracraneal pressure and without meningitis. A CAT scanner disclosed decreased density of the white matter and multiple nodular lesions enhanced with the infusion of contrast media. To our knowledge, this is the first description of the findings in computerized axial tomography of a patient with miliary tuberculosis and hematogenous dissemination associated to diffuse encephalopathy without meningitis.

When we move our head with our eyes open the optokinetic and the vestibular systems are simultaneously activated. The optokinetic and the vestibular systems interact in a complex way. A full field optokinetic system can suppress or enhance vestibular reflexes according to the needs of the organism. It is likely that the summation of optokinetic and vestibular nystagmus is not linear, rather, the organism encodes retinal slip velocity as the important input.

This study expresses a theory designed to associate the fundamental thermodynamic concepts to the electrophysiology of the nervous system. An analogy is drawn between the thermodynamic and electrical events of the myocardial cell during its mechanical phases with those thermodynamic and electrical events of the nerve cell during conduction. Although the myocardial and nerve cells are designed to perform different functions, their processes of membrane potential activation and corresponding directions of thermodynamic and entropy changes are basically the same. A thermodynamic view of nerve cell "active" potential corresponds to a passive downhill process during which the entropy of the system (nerve cell) increases. In contrast, the so called "resting" membrane potential corresponds to an active uphill process. Thus, there is a paradox because the nerve cell is thermodynamically passive during the "active" membrane potential, whereas it is thermodynamically active during the "resting" membrane potential. It is suggested that inhibition in the nervous system may be considered a thermodinamically active process. If the negentropic process fails, the result is the malfunctioning or the final destruction of the system.

The formation and absorption capacity of CSF in the isolated spinal subarachnoid space was investigated in seven dogs by using a perfussion of a solution similar to the cerebrospinal fluid labeled with I 131. It was found that there was no CSF production in the spinal canal; however, there was an important absorption rate with an average of 0.2695 ml/min when the intraspinal pressure was of 10 centimeters of water. The clinical implications of this findings are discussed.

Environmental factors as well as different modes of inheritance has been suggested to explain the etiology of left-handedness. In order to improve knowledge of this problem, manual skill (MS) of parents (n = 234), siblings (n = 506) and children (n = 126) of 60 right-handed (RHI) and 60 left-handed (LHI) index cases (IC), born in Mexico City, were studied. Parents and siblings of both IC had similar frequencies of left-handedness. Quite the contrary, 36.7% of children of LHI were left-handed, while 7.3% children of RHI happen to be left-handed (P < 0.00025). No differences were found in the appearance of perinatal environmental factors. These findings are explained in part according to the pressure exerted by parents and/or teachers for dextrality. The impact of this influence modifies the effect of several unknown genes (multifactorial inheritance). The understanding of the above mechanisms in the etiology of MS is relevant not only for academic purposes, but for the educational sphere as well.

Two cases of cryptococcal meningitis and increased intracranial pressure in patients with acquired immunodeficiency are described. Both patients presented high intracranial pressure that persisted despite optimal antifungal treatment (amphotericin B, 5-flucytosine initially, and fluconazole posteriorly). The elevated intracranial pressure produced headache, seizures, and reduced visual and auditory acuity. CAT scan demonstrated absence of ventricular dilatation or focal lesions. Both cases were treated with adequate antifungal therapy, as well as with repeated lumbar punctures and placement of a lumboperitoneal shunt due to the persistence of elevated intracranial pressure. One patient presented with unilateral loss of vision due to optic nerve atrophy. After one year of follow-up, one patient died due to progression of his disease, while the other is still alive and without evidence of neurological disease. Intracranial hypertension is a frequent clinical manifestation of cryptococcal meningitis in patients with acquired immunodeficiency syndrome (AIDS) that requires adequate diagnosis and management. Treatment should be directed towards the reduction of intracranial pressure though repeated lumbar punctures and, in some cases, with lumboperitoneal or ventricular-peritoneal shunts.

A twenty year old man presented an acute hematomyelia at T2-T3 level and had electrocardiographic changes suggesting subendocardial and subepicardial ischemia; he also had precordial pain and elevation of the MB fraction of creatine phosphokinase. Neurons providing heart inervation are located at the T2-T3 spinal level. The electrocardiographic changes observed were considered neurogenic in origin and were transient. Although there are experimental reports showing electrocardiographic changes associated with compression of the upper part of thoracic spinal cord, this is the first report to our knowledge, in which an acute spinal injury is shown to be associated with neurogenic changes in ventricular repolarization simulating acute myocardial ischemia.
