Se realizo una comparacion aleatorizada de una Terapia de Esquema de Doble Foco con Asesoramiento Individual sobre Drogas, como mejora para el tratamiento residencial de 105 pacientes dependientes de drogas y con trastornos especificos de la personalidad versus pacientes sin esos trastornos. Ambos tratamientos fueron guiados mediante manual y aplicados durante 6 meses por psicoterapeutas experimentados, intensivamente entrenados y supervisados con evaluacion independiente de la fidelidad. Usando el modelo de riesgos proporcionales de Cox, no se encontraron diferencias psicoterapicas en la retencion (dias de tratamiento). El Modelo Lineal Jerarquico indico que los participantes con trastorno de la personalidad comenzaron con mayores sintomas psiquiatricos, interpersonales y de disforia, y ambas terapias redujeron los sintomas a mas de 6 meses. ABSTRACT Food insecurity (FI) is under-researched among people experiencing substance use disorders (SUD). Given the physical and mental health consequences associated with FI, this study examined the prevalence of FI and its covariates in individuals seeking treatment for SUDs and whether any specific SUD diagnosis was associated with FI. Individuals with polysubstance use disorder (PSUD) were compared to those with a single SUD diagnosis. This study was a cross-sectional chart review at an outpatient substance use disorder treatment unit of 201 treatment-seeking adults with one or more SUD. Of 201 adults, 43.8% reported experiencing FI within the past 12 months. Only cannabis use disorder was associated with FI (p = .025). Those with PSUD had 2.08 times the odds (OR = 2.08, 95% CI = 1.15– 3.76) of FI compared to those diagnosed with one SUD. Policy and practice recommendations are discussed regarding implementation of FI screening and interventions in substance use treatment.

The prevalence of unidentified or untreated unhealthy alcohol use remains high. With the advent of pharmacotherapy and models of counseling appropriate for use in primary care settings as well as in specialty care, clinicians have new tools to manage the range of alcohol problems across the spectrum of health care settings. By extending treatment to primary care, many people who do not currently receive specialty care may have increased access to treatment. In addition, primary care providers, by virtue of their ongoing relationship with patients, may be able to provide continuing treatment over time. Extending the spectrum of care to hazardous drinkers who may not be alcohol dependent could result in earlier intervention and reduce the consequences of excessive drinking.

Drunk-dialing is a term documented in both popular culture and academic literatures to describe a behavior in which a person contacts another individual by phone while intoxicated. In our collective clinical experience we have found that clients drunk-dial their clinicians too, particularly while in substance use treatment, and yet there is a noticeable absence of research on the topic to guide clinical decision-making within a process-based understanding of these events. As the parameters within which psychotherapy takes place become increasingly technologized, a literature base to document clients’ idiosyncratic use of technology will become increasingly necessary and useful. We provide a brief review of the existing research on drunk-dialing and conclude with specific questions to guide future research and practice.

Psychologists' roles within academic medicine have expanded well beyond research and scholarship. They are active as providers of patient care, medical education, and clinical supervision. Although the number of psychologists in academic health centers continues to grow, they represent a small portion of total medical school faculties. However, with the movement toward collaborative care models, emphasis on interprofessional teams, and increased emphasis on psychological science topics in medical curricula, psychologists are well-positioned to make further contributions. Another path through which psychologists can further increase their contributions and value within academic health centers is to aspire to leadership roles. This article describes the first author's reflections on her experiences in a two-year, cohort-based, educational leadership development certificate program in academic medicine. The cohort was comprised largely of physicians and basic scientists, and a small number of non-physician participants of which the first author was the only clinical psychologist. The insights gained from this experience provide recommendations for psychologists interested in leadership opportunities in academic medicine.

Physician shortages in the US are expected to intensify with the implementation of the Patient Protection and Affordable Care Act. These shortages may negatively impact access to care, quality of care, and confidence in the system's ability to adequately provide for health needs in the US. Concerns regarding physician demand underscore how critical Graduate Medical Education funding is to preparing the physician workforce. In 2014 5.6 % of US medical school seniors did not match into residency. Psychology has faced longstanding training imbalance issues with a misalignment between the number of internship positions and the number of applicants. The authors summon attention to the damaging effects a training imbalance poses to a health care profession, its trainees, and ultimately the public it serves.
