    Time in Therapeutic Range (TTR) is a value used to assess the efficacy of Warfarin treatment. The aim of our study is to determine the effective INR levels and the rate of TTR in patients on Warfarin regimen due to Atrial Fibrillation (AF) or Mechanical Prosthetic Valve (MPV). A total of 94 patients (58 female, and 36 male, mean age: 64.9±11years) on Warfarin treatment due to AF or MPV with at least 10 INR levels measurements in the last 6 months were included in this retrospective study. The patients were divided into 2 groups. Group 1 consisted of the patients with Valvular AF (n=47); Group 2 included the patients with Non-Valvular AF (n=47); TTR and INR levels were compared. The average of INR values were found as 2,4 (min: 1,3, max: 4,3) in all patients; 2,3 (min: 1,3, max: 4,2) in Group 1; 2,6 (min: 1,3, max: 4,3) in Group 2. The average of TTR values was found 40.3% (min: 10%, max: 80%) in all patients; 43.8% (min: 10%, max: 80%) in Group 1; 36,8% (min: 10%, max: 80%) in Group 2. INR and TTR values are needed to assess the effectiveness of the Warfarin treatment. The patients in treatment with Warfarin should be well trained and frequently monitored. On the other hand, the underlying factors of the TTR values being determined as lower in the Turkish patient population might be due to the lower socio-economic and socio-cultural status, inadequate education levels, and the insufficient information on use of the medication provided by the doctors to the patients.

Bu listeler baz alinarak yasli hastalarin kullanmasi uygun olmayan ilaclar tespit edilerek, daha az yan etkisi olan daha uygun ilaclar ile degistirilmektedir. PRISCUS ilac listesinde ilk kez 2010 yilinda Almanya’da kullanilmaya baslanan daha sonrada revize edilen yasli hastalarda kullanimi uygun olmayan ilaclari gosteren bir listedir. Biz bu derlememizde PRISCUS listesi, yaslilarda coklu ilac ve uygunsuz ilac kullanimi ve u hakkinda bilgi vermeyi amacladik.

    Background: The pathogenesis of irritable bowel syndrome (IBS) has not been fully elucidated. The gastrointestinal tract have a well-differentiated intrinsic nervous system and also this system is connected with nervous system. The symptoms of IBS are related with autonomic nervous system (ANS). It was also possible to see cardiovascular symptoms due to this link. This link can influence QT dispersion (QTd).     Aims: It is known that the angle between the aorta and the septum on the long axis in two-dimensional echocardiography is different between individuals in the community. The relationship between aortoseptal angle (AoSA), age and diastolic dysfunction has been mentioned in a couple of articles. We aimed to investigate if this angle is directly related to duration of hypertension (HT), regardless of age factor. Method: The data of 1294 patients who applied to the cardiology outpatient clinic and whose AoSAs were recorded and analyzed retrospectively. SPSS 20 was entered and the correlation of AoSA with age, duration of HT and other data was investigated. Results: A significant correlation was found between AoSA, duration of HT, age, and diameter of the ascending aorta.     Pericardial effusion and cardiac tamponade is a complication often seen in patients with advanced lung disease. However, pericardial effusion and cardiac tamponade is a rare form of presentation of lung cancer as the first finding. 52-year-old male patient presented with a sudden onset dyspnea to emergency service. There was a significant cardiomegaly in patient’s telecardiography and was showed an image compatible with massive pericardial effusion and cardiac tamponade in echocardiography. The patient's condition improved with emergency pericardiocentesis. Biopsies were taken from the neck lymph nodes seen on thorax CT. Biopsy was consistent with large cell lung cancer. Malignancy should be considered in patients with pericardial effusion and cardiac tamponade. For the first time in this case, lung cancers are discussed in a patient with massive pericardial effusion and cardiac tamponade.

    Patients with carotid disease are frequently referred for carotid artery stenting based on the results of carotid duplex studies. During carotid artery stenting, the stent is usually extended into the common carotid artery, thereby crossing the external carotid artery. Previous studies have shown conflicting results regarding internal carotid stenting and external carotid artery flow velocities, but the effect of stenting on ipsilateral superior thyroid artery velocities has not been defined. This study examined the effect of internal carotid angioplasty and stenting on the ipsilateral superior thyroid artery Doppler-derived flow parameters. We prospectively studied preinterventional and postinterventional duplex scans obtained from 41 patients (mean age ± SD, 64 ± 10 years) who underwent carotid artery stenting. The Doppler-defined preprocedural peak systolic velocity (PSV) end-diastolic velocity (EDV), resistive index (RI), and pulsatility index (PI) in the ipsilateral external carotid and superior thyroid arteries were compared with postprocedural values. Among patients with stenting, the preprocedural PSV, EDV, RI, and PI in the ipsilateral superior thyroid artery were 30 ± 11 cm/s, 13 ± 6 cm/s, 0.62 ± 0.11, and 1.04 ± 0.28,respectively; after stenting, they were 36 ± 8 cm/s, 14 ± 9 cm/s, 0.71 ± 0.07, and 1.11 ± 0.19. The preprocedural PSV, EDV, RI, and PI in the ipsilateral external carotid artery were 79 ± 24 cm/s, 17 ± 7 cm/s, 0.77 ± 0.26, and 1.27 ± 0.22; after stenting, they were 94 ± 31 cm/s, 20 ± 6 cm/s, 0.80 ± 0.4, and 1.25 ± 0.31. Despite a slight increase in superior thyroid and external carotid artery flow, there was no statistically significant change from before to after stenting. This study showed no differences in blood velocity profiles in the ipsilateral superior thyroid and external carotid arteries after stenting.

    To evaluate the left (LV) and right (RV) ventricular function in euthyroid Hashimoto's thyroiditis (eHT) patients. Forty five (45) patients diagnosed with euthyroid Hashimoto's thyroiditis (eHT) and age and gender-matched forty five (45) control subjects were enrolled in this study. Echocardiographic parameters reflecting RV and LV functions such as; chamber dimensions, ejection fraction, fractional shortening, conventional and tissue Doppler derived early and late filling velocities (E, A, E', A'), isovolumic relaxation (IVRT) and contraction (IVCT) times, ejection time (ET), deceleration time (DT), Tei index, pulmonary acceleration time (PAcT) and tricuspid annular plane systolic excursion (TAPSE) of patients with eHT and were compared to control subjects using paired-samples t-test or Wilcoxon signed rank test. Regarding the LV function, patients with eHT compared to controls, had higher LV-Tei index (0.6±0.2 vs 0.4±0.1, p<0.001), DT(p<0.001), IVRT(p<0.001), and E/E' ratio (p=0.04). In contrast, the peak E wave velocity (p=0.02), E/A ratio (p=0.01) and ejection time (p=0.02) were significantly lower in the eHT group than control. For the RV, Tei index (0.40±0.11 vs 0.28±0.07, p<0.001), TAPSE (2.0±0.3 vs 2.2±0.2 mm, p<0.001), PAcT (124.3±22.6 vs 149.4±18.3 msec, p<0.001), A' (p=0.007) and IVCT (p=0.001) were significantly higher in patients with eHT than control. However, tricuspid E/A ratio (p=0.01), E' (p=0.03) and E'/A' ratio (p=0.001) were significantly lower in the eHT than control group. This study demonstrated that both RV and LV functions were impaired in patients with eHT.
