背景技术

发明内容

图2展示了根据某些实施方案的用于治疗腹壁缺损的一种装置200。在某些实施方案中,装置200包括第一合成聚合物材料210和附着到该合成聚合物材料210的整个外周边界230上的脱细胞组织基质220。在使用中,脱细胞组织基质220可以固定到围绕在体腔中的缺损140的组织上从而闭合该体腔(例如,腹部)而没有将该第一合成聚合物材料附着到组织上。例如,当一个腹部切口形成时(或者正中线抑或其他位置),完全闭合该切口可能是困难的。这可能是由于腹部内容物的膨胀、大的病人尺寸、和/或由于先前的外科手术、外伤或疾病造成的组织损失。另外,在一些情况下,再次进入该外科手术部位,例如,以便进行另外的外科手术,可能是令人希望的。装置200可以帮助闭合一个切口或其他缺损,并且可以用来再次进入该外科手术部位和/或在阻止正常闭合的问题减轻(例如,膨胀减轻或继后的外科手术步骤完成)之后闭合该缺损。
如在此使用的,术语“合成聚合物材料”包括由人类生产的任何聚合物材料薄片,该薄片或者来自化学反应,抑或通过装配一种天然材料来生产。例如,由人类生产的聚合物可以包括聚乙烯类或聚酰胺类。通过装配一种天然材料生产的材料可以包括,例如,用蚕丝生产的薄片。
在初期植入期间,具有附着到其外周边界230上从而形成一个接头235的脱细胞组织基质220的合成聚合物材料210(参见图5A-5D)被定位于腹壁中的缺损内。然后,该脱细胞组织基质附着到围绕腹壁缺损的一个外周边界的组织上从而闭合该缺损。通常,对于一个正中切口而言,脱细胞组织基质220将被固定到腹部筋膜上(例如,直肌鞘),由此作为该直肌鞘的一个延伸部分,该部分通常被用来闭合正中腹部切口。如本领域已知的,使用典型缝线、手术钉(surgicalstaple)、或回形针、其他适合的连接机构,该脱细胞组织基质可以被附着到组织上。在某些实施方案中,脱细胞组织基质220可以通过使缝线穿过脱细胞组织基质220而连接。在某些实施方案中,这些缝线可以穿过多个预先形成的开口295,这些开口可能被加固(或开口360,如在图3中所示的)。
可以用不同的材料来生产合成聚合物材料210和脱细胞组织基质220(共同称为“材料”)通常,这两种材料都应当是无菌的或消毒的,并且应当具有适合的生物力学特性以防止在使用中破裂或撕裂。另外,在某些实施方案中,该材料的机械特性是相容的以便提供相对于不同材料的均匀应力分布从而预防失效,如以下更详细说明的。另外,该合成材料通常应当是惰性的或生物相容的从而预防过度的炎症。适合的合成材料可以包括,例如,(或其他聚四氟乙烯材料)、(高密度聚乙烯)、或聚丙烯缝线(prolene)。在某些实施方案中,这些合成材料可以包括合成的可再吸收材料,遍及它们的尺寸的部分或全部。另外,这些材料可以被涂覆有治疗剂,(例如,抗粘涂层,抗微生物剂等)。
该脱细胞组织基质可以被选择为提供多种不同的生物学特性和机械特性。例如,该脱细胞组织基质可以被选择为允许组织向内生长和重塑,以允许组织再生正常发生在该基质被植入的部位。例如,当该脱细胞组织基质被植入到筋膜上或筋膜中时,该基质可以被选择为允许筋膜再生而没有过度纤维化或瘢痕形成。另外,该脱细胞组织基质不应当引出过度的炎症反应并且应当最后被重塑,以产生与最初的宿主组织相似的组织。在某些实施方案中,该脱细胞组织基质可以包括或StratticeTM,它们分别为人脱细胞真皮基质和猪脱细胞真皮基质。可替代地,可以使用其他适合的脱细胞组织基质被使用,如以下进一步说明的。
通常,合成聚合物材料210和脱细胞组织基质220都应当具有这样的机械特性,使得这些材料在使用中不会失效(如,破裂或撕裂)。另外,这些材料应当具有足够的柔性和弹性,当它们被植入时将由外科医生处理,将被成型为允许覆盖底层结构,并且在病人活动过程中允许伸缩从而向临近组织提供均匀的应力分布而没有撕裂。应当理解的是,这些特性可以通过改变普通材料特性连同这些材料的结构特征(例如,厚度)而改变(例如,拉伸强度和弹性特性)。在某些实施方案中,这些材料将被这样选择,使得这些材料可以经受住至少20N的拉力而不失效。在某些实施方案中,这些材料可以经受住每单位宽度的最小力,如至少20N/cm,至少24N/cm,或更高,这取决于病人。另外,在某些实施方案中,这些材料被选择用来允许缝线保留。在一些实施方案中,这些材料具有至少20N的一个缝线保留强度。
在某些实施方案中,材料210、20可以被选择并且其大小被这样确定,使得跨过这些材料的应力分布在使用过程中保持相对均匀。例如,在不同的实施方案中,合成聚合物材料210和脱细胞组织基质220可以被这样选择,使得超出典型操作范围的极限拉伸强度和/或弹性特性是相对相等的,或在彼此的一个特定范围之内。另外,位于合成聚合物材料210与脱细胞组织基质220之间的接头235的机械性能可以类似地与合成聚合物材料210和/或脱细胞组织基质220的那些机械特性相匹配。例如,在某些实施方案中,合成聚合物材料210的极限强度与脱细胞组织基质220的极限强度差异为小于20%,小于15%,小于10%,小于5%,或任何这些百分比之间的值。在某些实施方案中,合成聚合物材料210的弹性模量与脱细胞组织基质220的弹性模量差异为小于20%,小于15%,小于10%,小于5%,或任何这些百分比之间的值。
合成聚合物材料210和脱细胞组织基质220可以通过使用许多装置和技术而彼此附着。例如,材料210、220可以通过使用不同的缝线、钉、大头钉或包括永久缝线(如聚丙烯缝线)的粘合剂而连接。在许多配置中,材料210、220可以彼此连接。图5A-5D是根据不同示例性实施方案的图2的装置的截面图。正如所展示的,这些材料可以在一个端对端接头235(图5A)处通过一个重叠接头235’(图5B)而附着,其中合成材料210形成一个分叉接头235’’(图5C),或该脱细胞组织基质形成一个分叉接头235’’’(图5D)。
在某些实施方案中,这些材料可以通过将这些材料中的一种或两种编织到另一种上被附着。例如,图6展示了在一个接头250处附着到一种编织的合成材料211上的脱细胞组织基质220。在其他实施方案中,生物材料220可以被编织,或材料220、221两者都被编织,以产生一个具有足够的机械特性的接头250从而在使用过程中防止失效,同时允许相对均匀的应力分布。
如上所述,脱细胞组织基质220可以被固定到围绕一个体腔中的缺损140的组织上以闭合该缺损而不将该第一合成聚合物材料附着到组织上。这样,被选择为允许组织向内生长和重塑的脱细胞组织基质220是被连接到、附着到、和/或锚定到组织上的唯一材料(缝线或其他连接装置除外)。进一步地,在附着之后,筋膜或其他组织可以开始向内生长以及重塑。
另外,如上所提及的,在一些实施方案中,多次进入一个外科手术部位/切口、和/或在完成随后的治疗之后或一个病人的情况变化(例如,腹部内容物的膨胀减轻)之后,最终永久闭合该切口可能是令人希望的。因此,在一些实施方案中,该合成聚合物材料可以包括一个开口240或可以被切割,而无需切割临近组织,以允许重复进入。然后,该开口240可以用缝线260或其他多个装置重新密封。在一些实施方案中,该合成聚合物材料(由椭圆形250定界的)的一部分可以被移除,并且合成聚合物材料210可以被缩短,从而在切口边缘上提供另外的张力或移除过剩的或受污染的材料。
在一些情况下,完全移除合成聚合物材料210同时使该脱细胞组织基质220仍然附着到组织上可能是令人希望的。例如,合成聚合物材料210可能在稍后的时间被移除,例如在膨胀已经减轻或随后的外科手术已经完成之后,并且脱细胞组织基质220仍然可以附着到腹壁缺损的外周边界周围的组织上。另外,可以在通过使用缝线、钉、或其它手术装置将无细胞组织基质220的剩余部分彼此附着之后移除合成聚合物材料210而闭合腹壁缺损。在不同的实施方案中,脱细胞组织基质220将支撑筋膜或缺损周围的其他组织,从而防止重新开口或裂开。另外,在现存的用于正常筋膜闭合的组织不足的情况下,脱细胞组织基质可以提供另外的组织。
在一些实施方案中,如上所述,脱细胞组织基质220可以包括开口295,并且这些开口可以用来接收用于闭合腹部开口的缝线。在一些实施方案中,开口295可以被加强,如以下进一步说明的。
在某些实施方案中提供了用于治疗腹壁缺损的装置,这些装置不包括处于薄片形式的合成聚合物材料,如上所述。这样的装置可以只包括一种脱细胞组织基质,但是在用于闭合在上述挑战(例如,膨胀、组织不足、需要多次进入外科手术部位)存在下的某些切口可能是有用的。图3展示了根据某些实施方案的用于治疗腹壁缺损的装置300。装置300包括无细胞组织基质的一个薄片310,其中薄片310包括一个长形开口340、以及在该开口340相对侧的用于接收缝线的多个孔360,并且其中这些孔360是被加强的。装置300可以被固定到伤口边缘(例如,通过使用缝线的筋膜)上,并且这些被加强的孔360可以接收向装置300和伤口边缘提供张力的缝线,从而闭合该伤口或切口。
在一些情况下,开口340可以被再次打开,例如,用来进行一个随后的操作,清洁一个伤口/腹部部位,或用于任何其他目的。另外,在一些情况下,装置300可以具有多组加强的孔360,从而允许该装置以变化的距离缝合,例如,向伤口边缘提供渐增的张力,或移除过量的材料。例如,在一些实施方案中,这些预先形成的孔360包括两排或更多排的定位于该长形开口340的每一侧上的孔365、367,并且可以安置缝线通过以选择的距离分开的孔。例如,如在图4中所示,缝线最初可能通过最靠近开口340的第一排孔365而附着,从而闭合一个切口。然而,稍后,随着腹腔内脏的膨胀减轻,或随着组织伸展,外科医生可以添加另外的缝线或更换这些缝线,使这些缝线穿过开口367。这样,随着这些缝线被拉紧或缩短,伤口或切口边缘可以被拉得更近。
如在图3和图4所示,装置300可以包括单个材料薄片。然而,在一些实施方案中,可以使用两片或更多片脱细胞组织基质310。例如,图3的装置可以沿着从线370延伸的一条线被分成两片从而产生两个材料片。这两片可以被植入到一个伤口或切口的相对侧上并且被缝合在适当位置以闭合该伤口或切口,如以上所述。
开口360(和295)可以按许多方式加强。在一些实施方案中,可以使用置于开口360的边或边缘周围的生物相容性粘合剂加强开口360。适合的粘合剂包括,例如,纤维蛋白胶、基于氰基丙烯酸酯的组织粘合剂(如,)、以及壳聚糖组织粘合剂。在一些实施方案中,可以交联开口360的边或边缘以增加它们的强度并且防止撕裂(例如,使用化学或辐射诱导的交联)。
适合的脱细胞组织基质
如以上指出的,在此使用的术语“脱细胞组织基质”,通常是指基本上不含细胞和其他抗原物质的任何组织基质。皮肤、皮肤的部分(例如真皮)以及其他组织(如血管、心脏瓣膜、筋膜和神经结缔组织)可以用来产生在本披露的范围之内的脱细胞基质。
通常,在一种脱细胞组织基质的产生中所涉及的步骤包括从供者(例如,人尸体或动物来源)收获组织以及在保存生物和结构功能的条件下移除细胞。在某些实施方案中,该过程包括化学处理,以使该组织稳定以及在细胞移除时或移除前避免生物化学降解和结构性降解。在不同的实施方案中,稳定溶液阻止并且防止渗透性、低氧性、自溶性以及蛋白水解性降解,防止微生物污染,并且减少可以发生于含有例如平滑肌部件(例如,血管)的组织的机械损伤。该稳定溶液可以含有一种适当的缓冲液、一种或多种抗氧化剂、一种或多种渗透剂(oncoticagent)、一种或多种抗生素、一种或多种蛋白酶抑制剂、和/或一种或多种平滑肌弛缓剂。
该组织之后被放入脱细胞溶液中以便从该结构基质中移除活细胞(例如,上皮细胞、内皮细胞、平滑肌细胞以及纤维母细胞)而不损坏胶原基质的生物和结构完整性。该脱细胞溶液可以含有一种适当的缓冲液、盐、一种抗生素、一种或多种去污剂(例如TRITONX-100TM、去氧胆酸钠、聚氧化乙烯(20)失水山梨醇单油酸酯)、一种或多种防止交联的试剂、一种或多种蛋白酶抑制剂、和/或一种或多种酶。在一些实施方案中,该脱细胞溶液包含在RPMI培养基中1%的TRITONX-100TM,该培养基含有庆大霉素以及25mMEDTA(乙二胺四乙酸)。在一些实施方案中,将组织在37°C下在该脱细胞溶液中孵育过夜,同时在90rpm下轻轻振摇。在某些实施方案中,可以使用另外的洗涤剂从该组织样品中移除脂肪。例如,在一些实施方案中,将2%的去氧胆酸钠加入到该脱细胞溶液中。
在脱细胞过程之后,用生理盐水将该组织样品彻底清洗。在一些示例性的实施方案中,例如,当使用异基因材料时,用脱氧核糖核酸酶(DNase)溶液在室温条件下处理该脱细胞的组织过夜。在一些实施方案中,用制备在脱氧核糖核酸酶缓冲液(20mMHEPES(4-(2-羟乙基l)-1-呱嗪乙磺酸)、20mMCaCl2以及20mMMgCl2)中的脱氧核糖核酸酶溶液来处理该组织样品。任选地,可以将抗生素溶液(例如,庆大霉素)添加到该脱氧核糖核酸溶液中。可以使用任何适合的缓冲液,只要该缓冲液提供适合的脱氧核糖核酸酶活性即可。
虽然可以从与该脱细胞组织基质移植物的受者相同的物种的一个或多个个体制造一种脱细胞组织基质,但情况不必一定如此。因此,例如,可以从猪的组织制造一种脱细胞组织基质并且将其移植到人类患者中。可以作为脱细胞组织基质的受者以及用于生产该脱细胞组织基质的组织或器官的供者的种类包括但不限于:哺乳动物,例如人类、非人灵长类(例如,猴子、狒狒、或黑猩猩)、猪、奶牛、马、山羊、绵羊、狗、猫、兔、豚鼠、沙鼠、仓鼠、大鼠、或小鼠。
从该含胶原的材料中消除α-gal抗原表位可能减少对抗该胶原材料的免疫反应。α-gal抗原表位表达于非灵长类哺乳动物以及新世界猴(南非猴)中连同大分子(如细胞外成分的蛋白聚糖)上。U.Galilietal.,J.Biol.Chem.263:17755(1988)。这种抗原表位不存在于旧世界灵长类(亚洲和非洲猴以及类人猿)与人类中,然而,相同的抗gal抗体作为对在胃肠细菌上的α-gal抗原表位碳水化合物结构的免疫应答的结果而产生于人类和哺乳类中。U.Galilietal.,Infect.Immun.56:1730(1988);R.M.Hamadehetal.,J.Clin.Invest.89:1223(1992).
由于非灵长类哺乳动物(例如,猪)产生α-gal抗原表位,来自这些哺乳动物的含胶原材料到灵长类体内的异种移植经常导致排异反应,这是因为灵长类抗gal结合到该含胶原材料上的这些抗原表位上的缘故。该结合导致了通过补体结合和通过抗体依赖性细胞毒性的该含胶原材料的破坏。U.Galilietal.,ImmunologyToday14:480(1993);M.Sandrinetal.,Proc.Natl.Acad.Sci.USA90:11391(1993);H.Goodetal.,Transplant.Proc.24:559(1992);B.H.Collinsetal.,J.Immunol.154:5500(1995).此外,异种移植导致了该免疫系统的显著激活从而产生增加量的高亲和力抗gal抗体。因此,在一些实施方案中,当产生α-gal抗原表位的动物用作该组织来源时,从细胞以及从该含胶原材料的细胞外成分中基本上除去α-gal抗原表位,并且防止细胞α-gal抗原表位再表达可以减少针对与结合到α-gal抗原表位上的抗gal抗体相关联的含胶原材料的免疫应答。
为了除去α-gal抗原表位,在用盐水彻底清洗该组织以除去脱氧核糖核酸酶溶液之后,可以使该组织样品经受一次或多次酶处理从而除去某些免疫源性抗原(如果在该样品中存在的话)。在一些实施方案中,可以用α-半乳糖苷酶处理该组织样品以消除α-gal抗原表位(如果在该组织中存在的话)。在一些实施方案中,用制备在pH值6.0的100mM磷酸盐缓冲液中的300U/L浓度的α-半乳糖苷酶处理该组织样品。在其他实施方案中,α-半乳糖苷酶的浓度增加到400U/L,用于从收获的组织中充分除去α-gal抗原表位。只要完成抗原的充分移除,任何适合的酶浓度和缓冲液都可以使用。
可选择的,已在遗传上改良从而缺乏一种或多种抗原表位的动物可能被选作该组织来源,而不是使用酶处理该组织。例如,已经被遗传工程化而缺乏末端α-半乳糖部分的动物(例如,猪)可以被选作组织来源。对于适当的动物的说明参见共同未决的美国申请序列号10/896,594和美国专利号6,166,288,其披露内容通过引用以其全文结合在此。另外,某些对组织加工以产生具有或不具有减少量的α-1,3-半乳糖部分或缺乏α-1,3-半乳糖部分的脱细胞基质的示例性方法描述于XuHui等人的“APorcine-Derived Acellular Dermal Scaffold that Supports Soft TissueRegeneration:Removal of Terminal Galactose-α-(1,3)-Galactose and RetentionofMatrixStructure,”TissueEngineering,Vol.15,1-13(2009)(“一种支持软组织再生的衍生自猪的脱细胞真皮支架:末端半乳糖-α-(1,3)-半乳糖的去除以及基质结构的保留,”组织工程,第15卷,1-13(2009)”)”中,通过引用以其全文结合在此。
在脱细胞组织基质形成之后,可以任选地将组织相容性活细胞接种到该脱细胞组织基质中,以产生可以被宿主进一步重塑的一种移植物。在一些实施方案中,可以通过移植之前的标准体外细胞共培养技术或通过移植之后的体内再增殖而将组织相容性活细胞添加到这些基质中。通过受者的自身细胞迁移到脱细胞组织基质中或者通过将来自受者的细胞或来自其他供者的组织相容性细胞注入或注射到原位脱细胞组织基质中而实现体内再增殖。可以使用不同的细胞类型,包括胚胎干细胞、成体干细胞(例如,间充质干细胞)、和/或神经元细胞。在不同的实施方案中,这些细胞可以恰好在移植之前或之后直接施用于脱细胞组织基质的内部分。在某些实施方案中,这些细胞可以被置于有待被植入的脱细胞组织基质之内并且可以在植入之前将其培养。
Claims (18)
1.一种腹部或筋膜治疗装置,包括:
一种合成聚合物材料薄片,其形成该腹部或筋膜治疗装置的中心部分;以及
一种附着到该合成聚合物材料薄片的一个外周边界上的脱细胞组织基质,使得该脱细胞组织基质能被固定到围绕体腔的壁中的一个缺损的组织上,从而将该体腔闭合而没有将该合成聚合物材料薄片附着到组织上,
其中该脱细胞组织基质界定了该腹部或筋膜治疗装置的外周部分,
其中所述合成聚合物材料薄片的至少一部分与所述脱细胞组织基质不重叠,并且
其中在该中心部分处,该脱细胞组织基质与该合成聚合物材料薄片不重叠。
2.如权利要求1所述的装置,其中该合成聚合物材料薄片的极限强度与该脱细胞组织基质的极限强度的差异小于20%。
3.如权利要求1至2的任一项所述的装置,其中该合成聚合物材料薄片的弹性模量与该脱细胞组织基质的弹性模量的差异小于20%。

其中该脱细胞组织基质薄片被附着到该合成聚合物材料的一个外周边界上并从该外周边界延伸,使得该脱细胞组织基质薄片能被固定到围绕体腔的壁中的一个缺损的组织上,从而将该体腔闭合而没有将该合成聚合物材料附着到组织上,
其中该合成聚合物材料的至少一部分与所述脱细胞组织基质薄片不重叠,并且
