    Objectives: To gauge hospital pharmacists preparedness to become accredited to perform medication reviews, with a view to implementing a sustainable and innovative model of practice of hospital pharmacist‐led medication review for discharged patients and outpatients.Method: Clinical pharmacists at St Vincent's Public Hospital, Darlinghurst, were requested to complete a semi‐structured questionnaire. The questionnaire asked pharmacists about their understanding of the medication review process, whether it was relevant to their current and future practice and their perceived barriers to accreditation. Pharmacists were also asked to rate their level of skill and/or confidence with a number of processes utilised in the medication review on a 5‐point Likert scale.Results: Questionnaires were completed by sixteen pharmacists.
