Most Western chronic diseases are closely tied to lifestyle behaviors, and many are preventable. Despite the well-distributed knowledge of these detrimental behaviors, effective efforts in disease prevention have been lacking. Many of these chronic diseases are related to obesity and type 2 diabetes, which have doubled in incidence during the last 35 years. The Complete Health Improvement Program (CHIP) is a community-based, comprehensive lifestyle modification approach to health that has shown success in addressing this problem. This pilot study demonstrates the effectiveness of CHIP in an underserved, rural, and vulnerable Appalachian population. Two hundred fourteen participants in CHIP collectively demonstrated significant reductions in body mass index, systolic and diastolic blood pressure, and fasting blood levels of total cholesterol, low-density lipoprotein, and glucose. The most common types of supraventricular tachycardia are caused by a reentry phenomenon producing accelerated heart rates. Symptoms may include palpitations (pulsation in the neck), chest pain, lightheadedness or dizziness, and dyspnea. It is unusual for supraventricular tachycardia to be caused by structurally abnormal hearts. Diagnosis is often delayed because of the misdiagnosis of anxiety or panic disorder. Patient history is important in uncovering the diagnosis, whereas the physical examination may or may not be helpful, and usually necessitates use of a Holter monitor or an event recorder to capture the arrhythmia and confirm a diagnosis. Type 2 diabetes is over-represented in vulnerable populations. Vulnerable patients managing diabetes are challenged with less-than-optimal processes and outcomes of care; thus, and the American Diabetes Association have renewed the focus on social determinants of health with regard to the management of chronic diseases such as diabetes. This study explored the correlations between A1C and social and personal factors, including diabetes knowledge, diabetes numeracy, and food security. The Diabetes Numeracy Test-15, the Spoken Knowledge in Low Literacy Diabetes Scale, and the U.S. Department of Agriculture Food Security Questionnaire were administered to a Caucasian study population ( = 96) receiving diabetes care at a federally qualified health center. The American Diabetes Association (ADA) annually updates its Standards of Medical Care in Diabetes to provide clinicians, patients, researchers, payers, and other interested parties with evidence-based recommendations for the diagnosis and management of patients with diabetes. For the 2018 standards, the ADA Professional Practice Committee searched MEDLINE through November 2017 to add, clarify, or revise recommendations on the basis of new evidence. The committee rated the recommendations as A, B, or C depending on the quality of evidence or E for expert consensus or clinical experience. The standards were reviewed and approved by the Executive Committee of the ADA Board of Directors, which includes health care professionals, scientists, and laypersons. In this issue of Journal of Diabetes Science and Technology, Barnard and colleagues evaluate the use of the ACCU-CHEK® Aviva Expert blood glucose meter/bolus advisor system in patients with type 1 diabetes mellitus. Hypoglycemia is a major limiting factor to intensive glucose control, and fear of hypoglycemia, especially in those who have experienced severe reactions, is a major barrier. The bolus advisor improved overall glucose control and increased adherence by overcoming the patients' fear of hypoglycemia, giving them more confidence to give adequate doses of insulin to control hyperglycemia. In this review, we discuss other human factors that become barriers to intensive control, which can benefit from new technologies, including numeracy literacy, information overload, time required for diabetes self-care, and device incompatibility.

IN BRIEF Limited diabetes numeracy may be an important factor affecting diabetes care and treatment adherence. This study assessed the relationship between the Diabetes Numeracy Test (DNT-15 score) and patient and treatment variables. Patients who had type 1 diabetes and those who received care from specialty centers had higher levels of numeracy, but this did not translate into improved glucose control.
