The guidelines of the University of Cartagena for comprehensive professional training, provide for the inclusion of the research component. In the medical program, research is the cornerstone for the construction of new knowledge, that today, directs the medical act. However, to achieve these objectives educational work in the teaching of medical research, requires that teachers employ innovative strategies for education. An important step in the appropriation and application of these strategies is the identification and understanding of the barriers or “paradigms” that have the medical students during their training. It identifies three: the paradigm of the profession, clinical specialty and general physician, in which addresses the reason for the subjects considered “filler”, the reductionism of the specialties and the passivity of the general physician to the need for research. Knowledge of these paradigms leads to the identification of teachers’ barriers such as unidirectional conception of teaching-learning process, lack of feedback and research to enrich their classroom work. Enseñar y aprender en los escenarios clínicos, sean estos formales o informales, es una tarea fundamental del personal de salud. Estos espacios son propicios para el establecimiento de relaciones de enseñanza y de aprendizaje entre el personal de la salud y los estudiantes de pregrado y posgrado. Las oportunidades de aprendizaje en entornos clínicos son mucho más diversas y realistas que las que se pueden encontrar en textos, en las aulas o en herramientas digitales, pues nada iguala a la experiencia real del aprendizaje clínico que se da “junto a la cama del paciente”. Además, ciertas maniobras prácticas, aunque sencillas (como tomar una muestra de sangre venosa), solo pueden terminar de enseñarse en un espacio clínico. El Uso de Ejemplares Paradigmáticos en la Formación MédicaResumen Resulta dificultoso definir el término "enfermedad, mas aun si lo que se busca es un conjunto de caracteres suficientes y necesarios que de manera uniforme se repitan en cada una de sus instancias, o cuyas características caigan bajo un principio en el que todos sus miembros posean propiedades idénticas o con algún tipo de naturaleza común; problema que obliga a explorar otros principios que permitan categorizarla. Wittgenstein alejándose de la pretensión racionalista de representar objetivamente el mundo, analiza esas otras alternativas. Dice que el uso de un término se aprende con ejemplos, o señalando al objeto específico en el que se aplica. Si la pretensión es enseñar acerca de una enfermedad, necesitamos que se nos muestren ejemplos y para ello precisamos de maestros. El objeto de este trabajo es ayudar en esa demarcación, considerando a la "enfermedad" como un constructo teórico-clínico. Seguimos a Lorenzano cuando dice que cada enfermedad es una teoría clínica y cada paciente es un ejemplar de esa teoría, que se aprende mediante mostraciones ejemplares que los maestros enseñan de manera práctica; a estos los llamamos ejemplares paradigmáticos, necesarios para que el médico pueda incorporar la estructura teórica de la enfermedad, que junto a los modelos de los casos clínicos, enseñan los distinto modos que el paciente puede presentarse a la consulta. Este modo de instruir busca una vía automática para que el médico novel acceda rápidamente a la información práctica que necesita, la recupere y utilice como si fuese ya un experto. ¿Cómo el médico elabora y justifica epistémicamente su diagnóstico? ¿Cómo seleccionará de todas las enfermedades, la que encaja mejor como hipótesis para su paciente? Lo explicamos mediante el razonamiento abductivo de Pierce, el agregado de elementos de la metateoría estructuralista y el uso de ejemplares paradigmáticos.
The objective of this work is to present a model for the direction of the educational strategy of the medical career, which surpasses the mere orientations that, in this respect, gives Curriculum D; although, at the same time, it takes them into account. The proposed model was designed through the application of the strategic planning method to the educational-educational process. In this way, a model is obtained that constitutes a genuine strategy for the direction, planning, control and evaluation of the educational work at the level of the race; Which is characterized by its objectivity, integrity and scientific rigor.
