Three-port sleeve gastrectomy: complete posterior approach

Background Laparoscopic sleeve gastrectomy (LSG) is considered a standard bariatric procedure, which is easy to perform. However, there are pitfalls to be aware of, because complications from the operation, on the other hand, pose great challenges. Methods We describe our innovative three-port-technique of LSG in a step-by-step instruction. To investigate the safety and efficacy, a retrospective evaluation and analysis was performed on consecutive 116 patients. For this purpose, the operation time, complication rates and weight loss were recorded.Results Mean BMI was 35.47 ± 4.53 kg Mean procedure duration was 90.13 ± 5.68 mins. Mean weight loss achieved was 11.57 ± 5.09 kg at first month; 19.75 ± 6.57 kg at third month; 26.85 ± 7.60 kg at sixth month, and 28.86 ± 11.23 kg at one year. Background: Up till now, there is no standardized technique for performing Laparoscopic sleeve gastrectomy (LSG). Aim of Study: The aim of this study was to assess the feasibility of LSG by means of three port technique and reporting short term results. Patients and Methods: This was a prospective study donefrom February to December 2019. Twenty-eight morbidly obese patients (18 females and 10 males) were included. Mean age of 31.03 ±8.89 years, mean preoperative body mass index of 44.38±5.32kg/m . This study discussed the patients' demographics, operative technique, operative records, post-operative outcomes concerning excess weight loss, comorbiditiesresolution, complication rates, pain assessment and patients' scars satisfaction. Introduction. Laparoscopic sleeve gastrectomy is a widely utilized and effective surgical procedure for dramatic weight loss in obese patients. Leak at the sleeve staple line is the most serious complication of this procedure, occurring in 1–3% of cases. Techniques to minimize the risk of sleeve gastrectomy leaks have been published although no universally agreed upon set of techniques exists. This report describes a single-surgeon experience with an approach to sleeve leak prevention resulting in a progressive decrease in leak rate over 5 years. Methods. 1070 consecutive sleeve gastrectomy cases between 2012 and 2016 were reviewed retrospectively.
