Gingival recession is frequently occurring in patients coming to dental clinic which could lead to pain or hypersensitivity, esthetic problem, retention of plaque hence inflamed gingiva, root caries, abrasion and fear of tooth loss. Gingival recession can be managed by surgical or non-surgical approaches. Non-surgical approaches include-restorations, crowns, veneers and gingival masks whereas surgical management includes various techniques of increasing the width of keratinized tissue such as frenectomy in case of high frenal attachment & root coverage procedures. Lateral pedicle graft (LPG), is a technique where graft is elevated from donor site which remains attached at its base for nourishment and is transferred to adjacent site in isolated denuded root. A case of gingival recession is presented that was managed in our department using LPG technique. This technique was selected because of the advantages such as – single surgical area, preservation of blood supply of flap, the postoperative color being in harmony with surrounding tissue.

Gingival recession is a complex phenomenon resulting from the apical migration of the junctional epithelium with exposure of root surfaces which may cause hypersentivity problems and may also pose esthetic concerns for the patient, thus may be a major therapeutic challenge to the clinician. Though there are many surgical techniques advocated to treat recession, root coverage using Lateral Positioned Flap technique as compared to other technique have many potential benefits for patients with localized recession defects. The lateral positioned flap is commonly used to cover isolated, denuded roots that have adequate donor tissue lateral and vestibular depth. By using the tissue adjacent to the recession defect, the lateral positioned flap allows for correction of the defect without the discomfort encountered during other grafting techniques associated with palatal donor sites. This procedure is a time efficient, less invasive, and highly aes-thetic treatment option for managing isolated recession defects. This article highlights a case report in which a lateral positioned flap technique was used for root coverage of a labially placed tooth which should have ideally been corrected by orthodontic treatment but due to patient’s unwillingness, root coverage procedure was attempted to resolve the esthetic concern of the patient.

Gingival recession is defined as the apical migration of the gingival margin from its physiologic level which further causes pathologic exposure of root surfaces. Mucogingival esthetic surgery includes the treatment of mucogingival esthetic alterations. The Mucogingival problems may be caused by gingival recession which is often associated with non-carious cervical lesion or by root caries or by altered passive eruption. Root coverage is achieved by many procedures like free gingival autografts, pedicle graft and connective tissue grafts. Recession can be treated by surgical or non-surgical means. Surgical procedure includes various methods of increasing the width of keratinized tissue such as frenectomy in case of high frenal attachment & root coverage procedures and nonsurgical approach include-restorations, crowns, veneers and gingival masks. Lateral displaced pedicle flap was first described by Grupe and Warren in 1956 which was indicated in area to cover isolated apical migration of gingiva, & denuded root surfaces that have adequate amount of donor tissue adjacent to the recipient site. In few clinical studies it is seen that the success rate with Lateral pedicle graft is 70%, it is preferred over free gingival autografts in narrow and shallow gingival defects. This case report present the predictability of root coverage by using Lateral pedicle grafts in single tooth narrow and shallow gingival recession.
The desire for improved esthetics and the consequent demand for cosmetic dentistry have increased tremendously in recent times. Cosmetic procedures have become an integral part of periodontal therapy. Probably one of the most common esthetic concerns associated with the periodontal tissues is gingival recession. Various approaches have been shown in clinical studies to achieve predictable root coverage. This case report has described two cases of root coverage by connective tissue autograft combined with partial thickness double pedicle flap technique. The advantages of this technique are excellent color matching, triple blood supply to the graft and very predictable results. The two treated cases demonstrated adequate root coverage at six months follow-up.
