Heavy alcohol use is a significant problem in the military. Personnel often use alcohol in an attempt to cope with stress, boredom, loneliness, and the lack of other recreational activities. The easy availability of alcohol, ritualized drinking opportunities, and inconsistent policies contribute to a work culture that facilitates heavy and binge drinking in this population. Prevention strategies such as alcohol use policies combined with campaigns focusing on alcohol deglamorization, personal responsibility, and health promotion currently are being implemented to help reduce heavy alcohol use, but further research is needed to evaluate the effects of these efforts. Purpose To examine the prevalence of exercise in head and neck cancer survivors and determine preliminary associations with quality of life (QoL), fatigue, and depression. Materials and methods Fifty-nine of 65 (91%) eligible head and neck cancer survivors recruited from an academic oncology clinic completed a self-administered survey including the modified Godin Leisure-Time Exercise Questionnaire and Functional Assessment of Cancer Therapy-Head & Neck (FACT-H&N), which includes physical, social, emotional and functional well-being (FWB) as well as additional concerns, and the FACT-General (FACT-G). Medical variables were obtained by medical record review. Results The majority of participants were men (83%) and were Caucasian (92%), with mean age of 58±12.8. Cancer sites were primarily the oral cavity (24%), oropharynx (37%), or larynx (25%), with 20% being stage I, 7% stage II, 19% stage III, and 54% stage IV disease. Chemotherapy and/or radiation were ongoing in 14% of the participants. Half of the participants (51%) were diagnosed <6 months ago. Only three (5%) participants reported any vigorous exercise minutes (M=7.3±35.4), and only seven (12%) participants reported any moderate exercise minutes (M=19.5±70.6). Light exercise was reported by 26 (44%) (M=83.4±147.1). Only five (8.5%) participants were meeting current public health exercise guidelines. Partial correlations adjusting for age, medical comorbidity, and alcohol use showed that the total exercise minutes (i.e., light + moderate + vigorous) was positively associated with FWB (r=0.30, p=0.027), FACT-G (r=0.25, p=0.071), and FACT-H&N (r=0.26, p=0.064), was negatively associated with fatigue (r=−0.27, p=0.051), and had no association with depression (r=0.10, p=0.500). Conclusions Few head and neck cancer survivors are participating in any moderate or vigorous exercise, and over half are completely sedentary. Meaningful and potentially beneficial associations between total exercise minutes, QoL, and fatigue were demonstrated. An exercise intervention may have utility in this understudied cancer survivor group. Further research is warranted.

The importance of conducting substance abuse research among ethnic minorities is underscored by findings that members of many ethnic minorities in the United States report higher rates of heavy drinking and alcohol-related problems than do Whites and have increased rates of illicit drug use, abuse, and dependence. It is important to better understand ethnic-specific substance use from a public health perspective. Recent data suggest that the prevalence of past month alcohol use and heavy alcohol use among Hispanics is lower than and the prevalence of binge drinking and alcohol abuse or dependence is comparable to that of non-Hispanic Whites. These estimates vary among Hispanic subgroups and across gender and age groups. The prevalence of past month illicit drug use is also lower among Hispanics than that of several other groups, including non-Hispanic Whites. These trends are consistent among both men and women, although the prevalence for men is nearly twice that of women in nearly all subgroups. Conversely, the prevalence of illicit drug abuse or dependence among Hispanics is slightly higher than that of non-Hispanic Whites. This article describes national level epidemiological data on the prevalence of alcohol and illicit drug use, abuse, and dependence among Latinos/Hispanics in the United States in comparison to other race and ethnic groups. Previous findings in the literature will be reviewed and new analyses using the 2002 National Survey on Drug Use and Health will be presented. Given the heterogeneity of Hispanics in the United States, data for Hispanics will be broken down by national groups whenever possible.

The trajectory from alcohol use to alcohol use disorders in adolescence is yet to be understood. Momentary sampling may assist in capturing &amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;#39;real-time&amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;#39; data on young people&amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;#39;s alcohol use and associated motivational factors. This paper aims to review the feasibility and usefulness of a mobile phone momentary sampling program to capture data about alcohol use and related behaviours. Two studies were conducted: a school-based study with 18 Year 9 and 11 students and a clinical study with eight high-risk adolescent drinkers. Participants answered questions about their daily activities, alcohol use, stressors and negative mood four times a day for 1 week using a mobile phone momentary sampling program. In the school-based study, 61% of participants reported drinking alcohol. On drinking days participants spent less time studying (14% vs. 26%), more time sleeping or resting (35% vs. 12%) and more time hanging out (21% vs. 11%) than on non-drinking days. In the high-risk sample, 88% of participants reported drinking alcohol. On drinking days, these participants spent a greater proportion of their waking time with their boyfriend or girlfriend (19.2% vs. 6%) and generally had higher negative mood than on non-drinking days. The current mobiletype program was well suited to capturing data on alcohol use in younger, school-attending adolescents. However, to capture alcohol use in older adolescents who lead less routine lives, it is necessary to make some amendments including targeting particular behaviours and symptoms. Recommendations for future studies are proposed.

The pattern of pedestrian crashes varies greatly depending on lighting circumstances, emphasizing the need of examining pedestrian crashes in various lighting conditions. Using Louisiana pedestrian fatal and injury crash data (2010-2019), this study applied Association Rules Mining (ARM) to identify the hidden pattern of crash risk factors according to three different lighting conditions (daylight, dark-with-streetlight, and dark-no-streetlight). Based on the generated rules, the results show that daylight pedestrian crashes are associated with children (<15 years), senior pedestrians (>64 years), older drivers (>64 years), and other driving behaviors such as 'failure to yield', 'inattentive/distracted', 'illness/fatigue/asleep'. Additionally, young drivers (15-24 years) are involved in severe pedestrian crashes in daylight conditions. This study also found pedestrian alcohol/drug involvement as the most frequent item in the dark-with-streetlight condition. This crash type is particularly associated with pedestrian action (crossing intersection/midblock), driver age (55-64 years), speed limit (30-35 mph), and specific area type (business with mixed residential area). Fatal pedestrian crashes are found to be associated with roadways with high-speed limits (>50 mph) during the dark without streetlight condition. Some other risk factors linked with 'high-speed limit' related crashes are pedestrians walking with/against the traffic, presence of pedestrian dark clothing, pedestrian alcohol/drug involvement. The research findings are expected to provide an improved understanding of the underlying relationships between pedestrian crash risk factors and specific lighting conditions. Highway safety experts can utilize these findings to conduct a decision-making process for selecting effective countermeasures to reduce pedestrian crashes strategically.

To examine the influence of alcohol consumption, gender, and psychological risk and protective factors on college students' experiences of negative and positive consequences, the present study of 181 students assessed frequency and quantity of alcohol consumption, negative and positive consequences of alcohol use, positive alcohol expectancies, constructive thinking, and positive and negative affect. Results indicated that men and women differed in their experience of some consequences and that while alcohol consumption was generally more strongly related to consequences for women than for men, it was unrelated to most consequences. Further, when controlling for alcohol consumption, positive alcohol expectancies and negative affect were positively related to experiencing positive and negative consequences while constructive thinking was related to fewer positive and fewer negative consequences. Results indicate that consequences are much more strongly related to psychological risk and protective factors than to alcohol consumption. The article concludes with a discussion of implications for intervention efforts.

Discrimination has been conceptualized as a stressor that may be more negative than generic stress because it is tied to valued and unchangeable social identities. As with other stressful events, the experience of sexism has been related to poorer mental health and some physical health outcomes. This study of 179 female college students (79% of whom were White) showed that the relation between perceived sexism and binge drinking and smoking was mediated by psychological distress. In addition, there was a direct relation between sexism and smoking for weight control. Given the large toll that tobacco and excessive alcohol use exact on the U.S. population, we suggest that policy makers aim to reduce discrimination as one method to improve overall health.
