Purpose/Objectives: To determine the relationship between
fatigue and physical activity in older patients with cancer.
Design: Targeted analysis using data from a prospective
longitudinal study.
Setting: A cancer care facility in southeastern Ontario,
Canada.
Sample: 440 patients, aged 65 years and older, seeking
consultation for cancer treatment at a regional cancer clinic
for lymphoma or leukemia or lung, breast, genitourinary,
head or neck, gastrointestinal, or skin cancers.
Methods: Self-report questionnaires were mailed to consenting
participants and completed at baseline and three
and six months after consultation for cancer treatment.
Main Research Variables: Participants rated fatigue and
physical activity and reported comorbidities and personal
demographic characteristics. Clinical measures of disease
and treatment factors were obtained through chart abstraction.
Findings: Fatigue was the most prevalent symptom reported.
Higher fatigue was associated with lower physical
activity levels. Physical activity level significantly predicted
fatigue level, regardless of age.
Conclusions: Physical activity level is a modifiable factor
significantly predicting cancer-related fatigue at three and
six months following consultation for cancer treatment. The
results suggest that physical activity may reduce fatigue in
older patients with cancer.
Implications for Nursing: Physical activity interventions
should be developed and tested in older patients with
cancer.

This article presents a brief overview on the importance of adopting healthy behavior during and after the completion of primary cancer treatment. Increasing evidences are advocating physical activity engagement in cancer survivors due to its convincing beneficial outcomes. Today, outcomes from numerous trials confirmed the need to examine beyond physical activity engagement, into physical inactivity as an independent factor for cancer recurrences. Reducing cancer-risk related behaviors via increase physical activity and reduce inactivity is now receiving much attention in the field of cancer survivorship. Both realms of activity and inactivity are now acknowledged as influential independent factors contributing to better care in the field of cancer survivorship.

PURPOSE Adverse effects of breast cancer treatment can negatively affect survivors' work ability. Previous reports lacked detailed clinical data or health-related patient-reported outcomes (PROs) and did not prospectively assess the combined impact of treatment and related sequelae on employment. METHODS We used a French prospective clinical cohort of patients with stage I-III breast cancer including 1,874 women who were working and $ 5 years younger than legal retirement age (# 57 years) at breast cancer diagnosis. Our outcome was nonreturn to work (non-RTW) 2 years after diagnosis. Independent variables included treatment characteristics as well as toxicities (Common Toxicity Criteria Adverse Events [CTCAE] v4) and PROs (European Organization for Research and Treatment of Cancer [EORTC] Quality of life Questionnaires , Breast cancer module [QLQ-BR23] and Fatigue module [QLQ-FA12], Hospital Anxiety and Depression Scale) collected 1 year after diagnosis. Logistic regression models assessed correlates of non-RTW, adjusting for age, stage, comorbidities, and socioeconomic covariates. RESULTS Two years after diagnosis, 21% of patients had not returned to work. Odds of non-RTW were significantly increased among patients treated with combinations of chemotherapy and trastuzumab (odds ratio [OR] v chemotherapy-hormonotherapy: for chemotherapy-trastuzumab, 2.01; 95% CI, 1.18 to 3.44; for chemotherapy-trastuzumab-hormonotherapy, 1.62; 95% CI, 1.10 to 2.41). Other significant associations with non-RTW included grade $ 3 CTCAE toxicities (OR v no, 1.59; 95% CI, 1.15 to 2.18), arm morbidity (OR v no, 1.59; 95% CI, 1.19 to 2.13), anxiety (OR v no, 1.47; 95% CI, 1.02 to 2.11), and depression (OR v no, 2.29; 95% CI, 1.34 to 3.91). CONCLUSION Receipt of systemic therapy combinations including trastuzumab was associated with increased odds of non-RTW. Likelihood of unemployment was also higher among patients who reported severe physical and psychological symptoms. This comprehensive study identifies potentially vulnerable patients and warrants supportive interventional strategies to facilitate their RTW.

Psychological distress is especially common with any cancer diagnosis. This study examined the psychological distress in a cohort of women newly diagnosed with breast cancer. A longitudinal study of a cohort of women (n=147) diagnosed with breast cancer (within the past 1 year), were conducted at University Malaya Medical Centre, Kuala Lumpur. Data were collected at baseline and at post 4-week intervention. Analysis of variance was conducted to examine for any significant differences in the change-scores between the experimental group (n=69) and the control group (n=78). Using the change scores, analysis of variances shows significant differences between groups for stress, F(1,140) =13.68, p<0.0001, anxiety, F(1,140) = 8.44, p<0.004, and depression, F(1,140) =11.57, p<0.0001. Levels of stress, anxiety and depression generally decreased significantly in the experimental groups (p<0.05), but either maintained or increased in the control group. This study indicates that the level of psychological distress of women with breast cancer can be ameliorated with a 4 week self management intervention. Lower stress was also found in women who reported engagement in higher physical activity than women with low physical activity. Future studies may examine role of physical activity in ameliorating distress.
