Hal ini antara lain disebabkan karena populasi pasien yang semakin banyak dengan konsekuensi meningkatnya penggunaan obat dan adanya obat-obat baru yang lebih mahal. Angka kejadian kasus bedah sesar di negara maju maupun negara berkembang dari tahun ke tahun diketahui semakin meningkat. Di Rumah sakit X kab jombang dalam kurun waktu 1 tahun jumlahnya mencapai 900 lebih kasus bedah sesar yang di sebabkan oleh berbagai faktor. The treatment of peritoneal dialysis (PD)-related peritonitis has been a matter of extensive investigation, frequently generating therapeutic trials. Several combinations of antibiotics have served as newer protocols and tended to be efficacious, comfortable, and cost-effective. According to the more recent recommendations from the International Society for Peritoneal Dialysis, the rationale for empirical initial therapy of clinically detected peritonitis in PD patients has been to follow the bacterial profile derived from cultured specimens of PD effluents. Despite over 40 years of worldwide usage, relatively few data have been published on the pharmacokinetics of cefazolin in pediatric surgical patients. The primary objectives of this study were to examine the pharmacokinetics and safety of cefazolin in children 10 to 12 years of age (inclusive) receiving 1 or 2 g of cefazolin, based on body weight. This multiple-center, open-label study enrolled pediatric patients electively scheduled for surgical procedures who required cefazolin as part of their routine clinical management. Patients weighing ≥25 to <50 kg received a 1-g dose, and patients weighing ≥50 to ≤85 kg received a 2-g dose. Postdose pharmacokinetic and safety assessments were conducted following drug administration. Cefazolin concentration-time data were analyzed by using both noncompartmental and population pharmacokinetics methods. To investigate the utility of additional microbial analyses to detect the causative microorganism of microbial keratitis and to determine the strength of association between clinical variables. This retrospective study included 125 eyes of 123 people presenting with presumed infectious keratitis associated with contact lens wear, and examined between January 2001 and November 2011. Sixty-three male and 60 female patients with a mean age of 24.14 ( ±6.7) and 26.7 (±10.0) years, respectively, were included in the study. Among the samples tested, 38 (40%) corneal cultures, 85 (92%) contact lens cultures, and 49 (80%) contact lens case cultures yielded positive results. In the samples for which all microbial investigations were performed and corneal culture was positive, cultures of contact lenses and corneas yielded the same microbes in 94% of cases, and for contact lens case and corneal cultures 77% of cases yielded the same microbes. Importantly, these were monocultures of the same microbe isolated from the cornea in the contact lens and lens case cultures. The odds of having a positive corneal culture was 37 times higher (p = 0.008) when patients presented to the clinic 2 weeks after the onset of symptoms. Pseudomonas aeruginosa was the predominant organism isolated, and most isolates were susceptible to commonly used antimicrobials. Contact lens culture might be an effective test to perform to detect and determine the causative microorganism when corneal cultures are unavailable or yield no microorganisms upon culture. A longer duration of symptoms before presenting to the clinic was significantly associated with obtaining a positive corneal culture.

Background Peritoneal dialysis (PD)-related peritonitis is a common and morbid complication of PD. Bacteria are able to create a biofilm on the PD catheter, which can be a source of recurrent infection. Biofilms undergo a phenotypic change resulting in increased antibiotic resistance. Methods 21 clinical isolates of different patients with PD peritonitis secondary to Staphylococcus aureus were collected. They were analyzed for their antibiotic susceptibility in the planktonic form using the standard minimum inhibitory concentration (MIC) and in a biofilm using minimum biofilm eradication concentration (MBEC). Chi-square was used to compare the sensitivity results. Results The isolates were susceptible to all the antibiotics tested using MIC. Every antibiotic except gentamicin lost its efficacy when the bacteria were grown in a biofilm ( p > 0.05). Deep sternal wound infection is a severe complication after cardiac surgery. Insufficient antibiotic target site concentrations may account for variable success of perioperative prophylaxis. Therefore, we measured perioperative penetration of cefazolin and of linezolid into sternal cancellous bone after sternotomy in coronary artery bypass grafting (CABG) patients by in vivo microdialysis. Nine patients underwent CABG using a skeletonized left internal mammary artery. Standard antibiotic prophylaxis consisted of 4 g cefazolin prior to skin incision and additional 2 g during skin closure. In addition, 600 mg of linezolid were administered prior to skin incision and after 12 h for study purposes. One hundred and fifty-nine cases of clinical Staphylococcus aureus mastitis were analyzed to detect factors associated with bacteriological cure after therapy. On 100 Dutch dairy farms, data were collected from four clinical trials with five intramammary treatment regimes designed to treat beta-lactamase-positive pathogens. Infected quarters were treated three times, with a 12-h interval between treatments. Treatment was extended for 2 d if results of the trial treatment were, according to the owner, not satisfactory. The overall bacteriological cure rate was 52%. The bacteriological cure rate of clinical beta-lactamase-negative S. aureus mastitis was significantly higher than that of clinical beta-lactamase-positive S. aureus mastitis. Bacteriological cure was also significantly higher if somatic cell count of the cow was low at the milk recording prior to the onset of the clinical mastitis. Peritoneal dialysate fluid (PDF) is a bacteriostatic medium that compromises the antibacterial activity of cell wall-active agents. By use of an in vitro static model, methicillin-resistant Staphylococcus aureus (MRSA), methicillin-susceptible S. aureus (MSSA), methicillin-susceptible Staphylococcus epidermidis (MSSE), and Streptococcus sanguis were exposed to daptomycin at concentrations of 10, 30, and 100 mg/liter, cefazolin at 125 mg/liter, and vancomycin at 25 mg/liter in cation-adjusted Mueller-Hinton Broth or Todd Hewitt Broth (for S. sanguis ) and PDF at pHs of 5.5 and 7.4. The pH had no effect on antibacterial activity. Neither cefazolin nor vancomycin produced a bactericidal or a bacteriostatic effect versus MRSA, MSSA, MSSE, or S. sanguis in PDF, while all concentrations of daptomycin were bactericidal against all organisms in PDF. Higher doses of cefazolin are required in obese patients for preoperative antibiotic prophylaxis, owing to its low lipophilicity. An ultra high performance liquid chromatography-tandem mass spectrometry method was developed to quantify cefazolin in serum and adipose tissue from 6 obese patients undergoing cesarean delivery, and using stable-isotope labeled cefazolin as an internal standard. The method has a 2μg/g lower limit of quantitation. The concentration in adipose tissue was 3.4±1.6μg/mL, which is less than half of the reported minimum inhibitory concentration of 8μg/mL for cefazolin. Serum cefazolin concentrations were more than 30-fold higher than in adipose tissue.

Objectives: This study aims to investigate whether cefazolin-sodium has any adverse effect on fracture healing in an experimental rabbit model. Materials and methods: The study was performed on 50 male New-Zealand white rabbits. Under general anesthesia, closed double fracture of middle one-third of the tibia-fibula of the left lower extremity of the subjects was produced by manual compression followed by closed reduction of fracture and long leg circular cast was applied. Subjects were divided randomly into five groups including 10 rabbits in each group. The first and second group were administered ciprofloxacin 50 mg/kg SC bid and cefazolin-sodium 50 mg/kg IM on the seventh day of fracture. The third group was applied a single high-dose of vitamin D (50.000 IU/kg) IM following fracture. The fourth group was applied daily vitamin E (alpha tocopherol) 20 mg/kg IM for five days from one hour before the production of fracture. Control group did not receive any treatment before and after fracture. Initial and control X-ray examinations were performed immediately and four weeks after production of fracture, respectively. At the end of the fourth week, animals were sacrificed and a histological examination of the fracture site was performed. Results: Histological evaluation showed that the histological grade of the fracture healing was significantly lower in the ciprofloxacin group, while it was significantly higher in the cefazolin-sodium, vitamin D and vitamin E groups, compared to control group (p<0.005). Conclusion: Significantly improved histological grade of the fracture healing in subjects treated with cefazolin-sodium than controls suggest that it may be reasonable to choose cefazolin-sodium as an antibiotic therapy for the treatment of infection in patients with bone fractures.

To evaluate nine eyes that developed corneal infection after radial keratotomy (RK) and their subsequent management. Dr. Rajendra Prasad Centre for Ophthalmic Sciences, New Delhi, India. The parameters evaluated were interval between RK and development of the ulcer, number of keratotomy incisions, nature of surgery (primary or secondary), status at presentation, and best corrected visual acuity (BCVA). Corneal scrapings were taken from all eyes for microbiological evaluation. Initial therapy was based on clinical impression and subsequent therapy, on the microbiological report. Staphylococcus species were the most frequently isolated bacteria followed by Pseudomonas aeruginosa. Two eyes had fungal growth, and no organisms were isolated from one. All but one eye responded to medical therapy; healing took 23 to 26 days. Therapeutic penetrating keratoplasty (PKP) was done in one eye. Presenting BCVA of hand movement to 6/36 improved to hand movement to 6/18 after the ulcer healed. Final BCVA was 6/36 to 6/9 after lamellar keratoplasty or PKP or with contact lens of spectacle correction. Our study shows that corneal infection is a potential complication of RK. Therefore, persistent postoperative irritation should be carefully observed to ensure early diagnosis and prompt therapy.

The objective of this study was to describe the population pharmacokinetics of cefazolin in plasma and the interstitial fluid of subcutaneous tissue of post-trauma critically ill patients and provide clinically relevant dosing recommendations that result in optimal concentrations at the target site. This was a pharmacokinetic study in a tertiary referral ICU. We recruited 30 post-trauma critically ill adult patients and collected serial total and unbound plasma cefazolin concentrations. Interstitial fluid concentrations were determined using in vivo microdialysis. Population pharmacokinetic analysis and Monte Carlo simulations were undertaken with Pmetrics(®). Fractional target attainment against an MIC distribution for Staphylococcus aureus isolates was calculated.
