Rekonstruksi sendi pinggul pada orang dewasa dengan telah menemukan kesuksesan dan sekarang diterima sebagai prosedur operatif yang aman. Apabila kita berbicara mengenai Total Hip Replacement, bagaimanapun kita harus menetapkan apakah kita mengarah pada sistem femoralis yang konvensional atau pada penambahan terbaru dari sistem sebelumnya. Konsep dari setiap sistem berbeda dengan jelas, kebutuhan kebanyakan pasien sangat bervariasi dari tiap orang, teknik operasi dan keahlian dibutuhkan untuk melapisi kembali bagian yang rusak sesuai dengan banyaknya permintaan.
Selama 19 tahun (1962 – 1981) sejak Charnley mengadopsi pembuatan komponen acetabular dari polietilen densitas tinggi, secara sistematis telah melewati 3 generasi dari sistem penggantian sendi pinggul konvensional. Seperti yang kita ketahui dan seiring meningkatnya pengalaman, terlalu banyak modifikasi pada desain prostetik dan kemajuan dari material dan juga teknik operasi telah banyak berperan.

Das Brugada-Syndrom gehört zu den primär kardialen Leitungsstörungen ohne erkennbare strukturelle Herzerkrankung und ist mit lebensgefährlichen ventrikulären Rhythmusstörungen assoziiert. Es ist eine autosomal dominante Erbkrankheit mit unterschiedlicher Penetranz und Bevorzugung des weiblichen Geschlechts. Eine endemische Häufung findet sich in Südostasien, im Mittelmeerraum sowie in Japan. Die jährliche Mortalität liegt weltweit bei etwa 30 pro 100.000 Einwohner. Der Gendefekt liegt auf dem SCN5A-Gen, auf dem kardiale Natriumkanäle kodiert sind. Zurzeit gibt es lediglich Empfehlungen für die Diagnosestellung, es liegen keine strengen Diagnosekriterien vor. Die Diagnose beruht auf einem auffälligen Ruhe-EKG (ST-Strecken-Hebungen in V1 und V2 mit inkomplettem Rechtsschenkelblock, häufig PQ-Zeit-Verlängerungen), der klinischen Symptomatologie und einer positiven Familienanamnese. Die derzeit empfohlene Therapie besteht in der Implantation eines internen Defibrillators. Zusammenfassend sollte bei Patienten mit auffälligem Ruhe-EKG und einem anamnestischen familiären plötzlichen Herztod das Brugada-Syndrom als wichtige Differenzialdiagnose in Betracht gezogen werden. A 42 year old male patient presented with atypical ECG pattern at rest and reported that his brother died suddenly of unknown reasons at the age of 40. A performed pharmacological testing with ajmalin discovered strong ST-segment elevations in lead V1 and V2 in combination with an incomplete right bundle branch block. As a result, the diagnosis Brugada syndrome was established. The Brugada syndrome is a primary cardiac conduction disturbance without structural heart disease and is associated with an increased risk of sudden cardiac death caused by life threatening ventricular tachyarrhythmia. It is an inherited disease displaying an autosomal dominant mode of transmission with an incomplete penetrance, especially prevalent in females. In some regions like the southeast of Asia and Japan it is endemic. The mutation is linked to the sodium channel gene SCN5A. The annual mortality rate is estimated 30 per 100.000 persons. Today, exact diagnostic criteria do not exist. The diagnosis is based on the typical ECG pattern, combined with clinical symptoms, and a family history of sudden cardiac death. The currently suggested therapy consists of the implantation of an internal defibrillator.

1. Diagnosis/ Gambaran Klinis:
Seorang  laki-laki usia 41 tahun datang dengan keluhan muntah. Muntah dirasakan sejak 1 hari sebelum masuk rumah sakit. Muntah timbul mendadak, dan  terus-menerus lebih dari 10kali/hari sehingga tidak ada intake makanan sama sekali dalam sehari. Muntah tidak dipengaruhi oleh makanan, kecemasan, perubahan posisi tubuh, atau konsumsi obat-obatan. Muntah disertai nyeri perut kiri dan kanan atas, kembung, juga dirasakan nyeri kepala, dirasakan sensasi seperti badan yang berputar, serta mata terasa panas. Muntah tidak didahului dengan mual, walaupun pasien juga mengeluh mual. Keluhan dirasa cukup mengganggu aktivitas sehari-hari. Pasien juga merasa lemas dan dirasa penurunan nafsu makan. Pasien tidak mengeluh demam. Nyeri perut dirasakan mendadak dibagian kanan atas dan ulu hati yang besifat ringan, dan timbul menetap,nyeri dipengaruhi oleh aktivitas, nyeri perut tidak dipengaruhi oleh makanan. Penurunan berat badan disangkal, batuk tidak, pilek tidak, nyeri telan tidak,dan  tenggorokan terasa sakit tidak. Telinga keluar cairan disangkal, dan berdenging disangkal. Buang air besar warna kuning, ampas, lendir tidak, darah tidak, tinja berlemak tidak, tinja  hitam tidak, tinja warna dempul tidak, dan bau amis disangkal. Buang air kecil tidak ada keluhan, warna kuning jernih, buang air kecil warna seperti teh disangkal.
2. Riwayat Pengobatan: Riwayat mondok dengan keluhan serupa(-), riwayat minum obat disangkal

Riwayat kesehatan/Penyakit: Riwayat keluhan serupa disangkal, riwayat mata atau badan kuning disangkal, riwayat minum jamu-jamuan disangkal, riwayat transfusi disangkal, riwayat narkotik suntik disangkal, riwayat minum minuman keras disangkal, riwayat keluarga atau lingkungan sekitar dengan keluhan serupa disangkal, riwayat diabetes disangkal, riwayat hipertensi disangkal
4. Kondisi Lingkungan Sosial dan Fisik : pasien tinggal dengan istri dan anak, pekerjaan pasien sebagai pekerja swasta. Riwayat kontak dengan orang yang sakit kuning disangkal

A recent survey conducted by the publicly funded Competence Network Sepsis (Sep- Net) reveals that severe sepsis and/or septic shock occurs in 75,000 inhabitants (110 out of 100,000) and sepsis in 79,000 inhabitants (116 out of 100,000) in Germany annually. The author had hypothesized that the prevailing Momentary Intracellular Ion-Pattern as a whole has an essential, perhaps the primary signaling function. (See Part 1.) The concentration of intracellular H + (pHi) is the most critical of all ions to maintain homeostasis; that is why regulation wants to preserve the permanence of pHi. Recent researches have shown that caged rodents can be well-modelizing for human hypoarousal diseases of civilization, which may correspond to Metabolic Syndrome or Depression. Repeated Social Defeat results in such psychic mechanisms that induce persistent hypoventilation among most of the observed rodent populations. Hypoventilation continued for weeks, even after stress, and presumably associated with hypercapnia. It may correspond to a larval version of the freeze response well known in the animal world, but it becomes chronic at people. The author., et al. previously presented in their papers how social or mental stress can be converted into an increased or decreased arousal by the body's closed-loop systems. Human 'low-grade hypercapnia' usually lasts for decades-to the end of life. One of the results is the Metabolic Syndrome, which often tends to the classic Type 2 Diabetes or other Related Disorders. Persistent elevation of the pCO 2 level launches a compensational cascade through chemical and hormonal-humoral alterations that can lead to various forms of readjustment. The Metabolic Syndrome is a latent, stealth catabolic state of the body that develops due to slight but chronic hypercapnic acidosis. Although chronic hypercapnia is usually compensated, a moderate intracellular acidotic state persists in continuing. Metabolic Syndrome is often the result of a learned 'civilized behavior'; the Social Defeat can also develop in captive animals due to chronic or repeated stress. The point is that this type of load does not increase but decreases arousal through hypercapnic acidosis, which can also cause Depression. Slight cytosolic acidosis also results in reduced metabolism, which is associated with moderately reduced anabolic and increased catabolic activity in the body with insulin resistance and reduced ATP producing capacity. As a consequence, the whole metabolism has become remodeled and dysregulated because of the disturbed Intracellular Ion Pattern.

Die Datenlage zu Sarkoidosereaktivierungen unter Interferontherapie ist ungenügend. Wir berichten hier den Fall einer Patientin mit anamnestischer Sarkoidose vor mehr als 20 Jahren. Aufgrund einer chronischen Hepatitis C wurde eine zunächst unkomplizierte antivirale Therapie mit pegyliertem Interferon α und Ribavirin durchgeführt. Ab der 32. Therapiewoche kam es zu einer Sarkoidosereaktivierung. Die Weiterführung der antiviralen Kombinationstherapie in unveränderter Form erfolgte bis zur 48. Therapiewoche ohne systemische Gabe von Steroiden. Sechs Monate nach Therapieende befand sich die Sarkoidose in Remission und die Hepatitis C war ausgeheilt. The data on reactivation of sarcoidosis during treatment with interferon-alfa is unsatisfactory. We here report the case of a female patient with a history of sarcoidosis more than 20 years ago. Due to chronic hepatitis C the patient was treated with pegylated interferon-alfa and ribavirin. Beginning in the 32nd week, the patient developed a reactivation of sarcoidosis. The antiviral therapy was continued until week 48 without administration of systemic steroids. Six month after the end of antiviral treatment the sarcoidosis was in remission and the hepatitis C was cured.

Eine zunehmende Anzahl von therapeutisch eingesetzten Substanzen und exogenen Toxinen führt zu strukturellen und funktionellen Veränderungen der Skelettmuskulatur. Das klinische Syndrom toxischer Myopathien variiert von symptomfreier Kreatinkinaseerhöhung über Muskelschmerzen mit Belastungsinsuffizienz, Schwäche und Atrophie bis hin zur akuten Rhabdomyolyse. Aufgrund der potenziellen Reversibilität einer toxischen Myopathie sollte der kausale Zusammenhang möglichst rasch erkannt werden und dann die Diagnosestellung und Elimination der Noxe erfolgen. Häufigste Ursache einer exogenen toxischen Myopathie ist die Alkoholkrankheit, die sowohl zu akuten als auch chronischen Neuropathien und Myopathien mit konsekutiver Muskelatrophie führt. Unter den medikamentös induzierten Myopathien hat neben der steroidinduzierten Myopathie zurzeit die durch Lipidsenker ausgelöste Myopathie die höchste Relevanz.
