Priapism

Priapism affects up to 50% of all males with sickle cell disease, and there is no standard treatment. Delayed and unsuccessful treatment leads to corporal fibrosis and impotence. It is therefore necessary to determine the best treatment methods for this complication in order to offer effective interventions to all affected patients. Herein we report an 11-year-old patient with sickle cell disease that presented with priapism 72 h after onset, and was successfully treated with automated red cell exchange and hyperbaric oxygen following unsuccessful surgical and conventional interventions.

Objective: This study aimed to evaluate the predictive value of platelet volume indices (PVIs), such as mean platelet volume (MPV), platelet distribution width (PDW), and plateletcrit (PCT), as prognostic parameters of detumescence in acute ischemic priapism (IP) patients with sickle cell anemia (SCA) in steady-state who received intracavernosal injections of phenylephrine with aspiration and saline irrigation. Methods: Fifty-six SCA patients with acute IP and 54 healthy male control subjects were included in the research. Priapism was diagnosed by penile Doppler ultrasound and corporal blood gas tests before intervention. Measurements of PVIs (MPV, PDW, and PCT) and TLC were ordered for all participants. Priapism is a disorder of persistent penile erection unrelated to sexual interest or desire. This pathologic condition, specifically the ischemic variant, is often associated with devastating complications, notably erectile dysfunction. This case is about a 35 year old gentleman who presented with four day old priapism. He was tried with irrigation and aspiration in another hospital with unsatisfactory resolution of the condition. Following admission duplex study was done which confirmed low flow priapism. After thorough evaluation glanulocavernous shunt shunt was done and detumescence of the penis was established within first 24 hour.

Priapism is a prolonged pathologic erection situation that occurs after sexual stimulation or without sexual stimulation. This condition is divided into two types, ischemic (low-flow, veno-occlusive) and non-ischemic (high-flow, arterial). A 68-year-old male patient applied to our clinic with the complaints of hardness and pain in the penis. Three days before he applied to our clinic, he was prescribed a single dose of 200 mg quetiapine by a psychiatry polyclinic for the complication of insomnia. Nearly 6 hours after the first dose of quetiapine, an involuntary erection occurred with accompanying pain in the penis. Introduction Novel treatments options for refractory erectile dysfunction, including penile shockwave therapy, have recently been introduced. Despite uncertain clinical efficacy, direct-to-consumer marketing has resulted in increased patient interest in these treatments, often associated with high out-of-pocket costs. Objective In the current study, we sought to assess outcomes in men with refractory erectile dysfunction undergoing penile shockwave therapy, as well as patient's perception of treatment outcomes. Methods Community-dwelling men receiving care at a local county hospital were prospectively recruited to undergo penile shockwave therapy for treatment of refractory erectile dysfunction. Background: Ischaemic priapism is one of rare presentation of Chronic myeloid leukaemia due hyper viscosity and it needs urgent urological intervention. Objective: To evaluate the need of urological intervention in Priapism with Chronic Myeloid Leukaemia (CML). Methodology: This is a retrospective study from January 2014 to July 2019 in the department of Urology, DMCH. Among 126 patients 6 were diagnosed as Priapism with CML. Records of these patients were evaluated and analyzed. Results: Among 126 patients 6 patients were diagnosed as priapism with CML Mean age of the patient was 46.7 years, 2 of them were farmer and 2 had history of taking herbal drugs immediate before sexual intercourse and developed Priapism. Following a bite from a juvenile Russell’s viper (Daboia russelii), a priapism (painful erection) developed rapidly in a 16-year-old male and only subsided after administration of antivenom 3 hours later. Potential mechanisms for this snakebite-induced priapism are unclear but likely due to venom toxins causing nitric oxide (NO) release and subsequent vasodilation of endothelium in the corpus cavernosum, although the possible involvement of other mechanisms cannot be ruled out. We strongly believe that this unusual case report may lead to further scientific research in order to improve the clinical understanding of the pathophysiology of envenomation due to Russell’s viper bites. Although it is too early to speculate, further research may also discover the possibilities of developing venom-based candidate molecules to treat sexual dysfunction in males and females.

High flow priapism is a quite rare condition. It is usually painless and occurs after a trauma. Nevertheless duplex ultrasound and selective arteriography remain to give us the final diagnosis. Here we present a young men with high flow priapism after perineal trauma.

The clinical diagnosis, ultrasonographic findings, surgical management, outcome, and survival rate of perineal or prescrotal urethral dilatation in 12 male calves are described. All calves were crossbred and intact males. The most noticeable clinical presentations were perineal (n= 10) or prescrotal (n= 2) swellings and micturition problems. The main ultrasonographic findings were oval shaped dilatation of the urethra in all animals with dimensions of 40-75 X 30-62 mm. The calves with perineal urethral dilatation were treated by perineal urethrostomy (n= 4) and partial penile transection including the dilated urethra and urethral fistulation (n= 6). Prescrotal urethral dilatations were treated by penile transection proximal to the dilatation site (n= 2).
