PURPOSE This study evaluated in vivo 2 different obturation techniques (lentulo spiral mounted in a slow-speed handpiece and hand-held) in primary teeth. METHODS The study was carried out on 24 children (mean age = 6.71 years) who had received 50 single-visit zinc oxide and eugenol (ZOE) pulpectomies in primary molars. The root canal filling materials were deposited into the canals either by a lentulo spiral mounted in a slow-speed handpiece or by a hand-held lentulo spiral. Evaluation of pulpectomized molars was performed by both authors immediately and 6 months following treatment using specific clinical and radiographic criteria. In cases of disagreement, the lower rank was selected. Introduction: An alternative method for the treatment of non-vital deciduous teeth is Lesion Sterilization and Tissue Repair (LSTR) technique in which instead of instrumentation of the root canals and obturating them with Zinc Oxide Eugenol (ZOE), the root canals are treated with a 3-Mix paste.
Case Report: In the current case report, the therapeutic outcomes of LSTR technique are presented in 3 patients with 3-month, 6-month, and 1-year clinical and radiographic follow-ups. The subjects consisted of patients referring to the Department of Pediatric Dentistry, School of Dentistry, Kerman, Iran.
Conclusions: Considering the successful outcomes of this treatment modality that was comparable to the standard pulpectomy procedure and its advantages such as one-visit treatment, no pushing of necrotic debris toward the buds of succedaneous permanent teeth and absence of ZOE in the root canals absorbed at a lower rate compared to the deciduous tooth roots were observed; hence, the technique might be an alternative technique for the standard pulpectomy procedure.

To perform a systematic review in which the clinical research question for primary teeth with irreversible pulpal pathosis was "how pulpectomies with zinc oxide eugenol (ZOE) paste performed compared to other materials in their clinical and radiographic outcomes after twelve months or more follow-up period. A literature survey of the electronic database (1950-2010) used the Medical Subject Headings and free text terms. Forty three references were retrieved and inclusion criteria were applied; 15 articles remained for full-text evaluation. From these, two were selected for data extraction regarding quality characteristics and results. Selected studies showed moderate or high risk of bias. The overall success of pulpectomy was 80.0% (Calcicur), 60.0% (Sealapex) and varied from 85.0% to 100.0% (ZOE) and 89.0% to 100.0% (Vitapex). Compare the antimicrobial effectiveness of common antimicrobial paste immediately after pulp therapy and in different storage conditions. Staphylococcus aureus, Staphylococcus epidermis, Streptococcus mutans, Streptococcus oralis, Enterococcus faecalis; Escherichia coli and Bacillus subtilis sample were used. External root resorption (ERR) is a serious complication after replantation, and its progressive inflammatory and replacement forms are significant causes of tooth loss. This retrospective study aimed to evaluate the factors related to the occurrence of inflammatory ERR (IERR) and replacement ERR (RERR) shortly after permanent tooth replantation in patients treated at the Dental Trauma Clinic at the School of Dentistry, Federal University of Minas Gerais, Belo Horizonte, Brazil. Case records and radiographs of 165 patients were evaluated for the presence, type, and extension of ERR and its association with age and factors related to the management and acute treatment of the avulsed tooth by using the logistic regression model. The fracture of an instrument is a recognized complication in endodontics. The immediate response to a fractured instrument is frequently to regard the treatment as a failure. Several factors must, however, be taken into account to evaluate the prognosis of the tooth in this situation. The objective of the endodontic treatment with or without a fractured instrument remains the same, namely to disinfect the root canal system and prevent its recontamination. The time at which file fracture occurred during treatment and the degree of canal infection should be considered when determining the potential effect of instrument fracture on treatment outcome. To find out the safe and efficient sedative agent for primary molar pulpectomy in uncooperative pediatric patients. This double blind randomized trial enrolled 40 anxious and healthy 2-6 year olds. All subjects received IV propofol (1-1.5 mg/kg) or ketofol (1-1.5 mg/kg propofol with 0.25 mg/kg ketamine) as per group assignment after oral midazolam premedication (0.5 mg/kg). Sedation maintenance was done with propofol infusion at 25-75 microg/kg/min titrated to a predefined Worse level as per Houpt's sedation rating scale. This study comprises a survey of Swedish dentists'treatment preferences in cases of carious exposure of the dental pulp in adults.The survey was conducted as part of a comprehensive report on methods of diagnosis and treatment in endodontics, published in 2010 by the Swedish Council on Health Technology Assessment. A questionnaire was mailed to a random subsample of 2012 dental offices where one dentist at each office was requested to answer all questions. Each questionnaire contained one of three sets of questions about endodontic practice routines.Thus around one-third of the subsample received case-specific questions about treating carious exposure. Introduction: Extensive dental caries in primary dentition, remains a serious problem in pediatric dental practice. Objective: To evaluate different obturation techniques of pulpectomized primary molars using pressure syringe (Map System), endodontic plugger and rotary lentulo spiral Methodology: The clinical study was carried out on 120 pulpectomized primary molars, which divided into two groups according to the material used for obturation. Group I: obturated with zinc oxide eugenol paste (ZOE). Group 2: obturated with Iodoform paste. According to obturation technique, each group was subdivided into three Subgroups(A) using Map system technique, (B) using Endodontic plugger and (C) using Rotary Lentuo spiral. All teeth were restored using stainless steel crowns. Teeth were evaluated radiographically regarding quality of obturation and presence of voids. Data was statistically analyzed using Chi-square test. Result: Good results were observed with lentulospiral technique in terms of quality of obturation (optimum filling) followed by endodontic plugger technique and then map system with no significant difference (P = 0.8). Voids were found in all subgroups with no significant difference between them. Significant difference was only between filled mesial and distal root canals of A1 and C1 subgroups (p = 0.016, P = 0.023) respectively. Conclusion: All techniques used for obturating primary root canals obturation showed good result regarding quality of obturation with no significant difference. Map syringe can be recommended as a technique of obturation as it is simple, easy to use, less threatening to the child.

The aim of this study was to evaluate the incidence of pain of endodontic origin and its relationship with socio-economic and demographic factors among patients seeking unscheduled urgent dental care. Patients attending the Emergency Clinic of Athens Dental School, Greece, between November 2011 and June 2012, were evaluated to determine their socio-economic profile, dental problem and treatment required. The facility operated from Monday to Friday, from 8.30 am to 1.00 pm, excluding the 4 weeks encompassing the Christmas and Easter holidays. In total, 533 patients were assessed regarding gender, age, ethnicity, occupation, reason for visiting, diagnosis and treatment provided. The data obtained were recorded, reviewed, coded and analysed using Poisson regression models. Mondays and Wednesdays were the busiest days of the week. The most common occupation among the patients was 'unemployed'. Pain of endodontic origin (reversible or irreversible pulpitis, or acute apical periodontitis) was the prevailing reason for the visit. The most frequent treatments administered were pulpectomy and drainage. Prescriptions for medications were rare. Services were requested primarily by individuals who were experiencing acute pain of endodontic origin, had low or no income and were available during morning hours, probably because of the service's low cost and operational hours. Prospective studies, such as the present investigation, can provide epidemiological evidence and indicate areas in the infrastructure of emergency services which may be improved. Additionally, such studies can provide rationale for public insurance programs and can generate profiles of the patients who utilise these low-cost public services.

The aim of this study was to evaluate the microbial distribution in the root canal system after periapical lesion induction in dogs' teeth using different methods. Fifty-two root canals were assigned to 4 groups (n=13). Groups I and II: root canals were exposed to the oral cavity for 180 days; groups III and IV: root canals were exposed for 7 days and then the coronal openings were sealed for 53 days. The root apices of groups I and III were perforated, while those of groups II and IV remained intact. After the experimental periods, the animals were euthanized and the anatomic pieces containing the roots were processed and stained with the Brown & Brenn method to assess the presence and distribution of microorganisms. The incidence of microorganisms at different sites of the roots and periapical lesions was analyzed statistically by the chi-square test at 5% significance level. All groups presented microorganisms in the entire root canal system. A larger number of microorganisms was observed on the root canal walls, apical delta and dentinal tubules (p<0.05), followed by cementum and cemental resorption areas. In spite of the different periods of exposure to the oral environment, the methods used for induction of periapical periodontitis yielded similar distribution of microorganisms in the root canal system.

The aim of this study was to measure root-dentin microhardness after instrumentation with two types of files and using irrigation with 2.5% sodium hypochlorite. Thirty roots were instrumented with irrigation: 10 roots had the pulp extirpated only; 10 roots were instrumented with stainless steel files, and 10 roots were instrumented with rotary nickel-titanium (NiTi) files. Additionally, five roots were extirpated only without irrigation and served as a control. The roots were sliced and root-dentin microhardness was determined at depths of 500 microm and 1000 microm from the pulp-dentin interface. Significant differences were found between the microhardness at 500 microm and 1000 microm (p &lt; 0.001) in all groups. Instrumentation with NiTi rotary files affected dentin microhardness significantly to a lesser extent.

This in vivo study compared the effect of mineral trioxide aggregate (MTA), IRM, Super EBA and ZOE in a puttylike consistency, used as retrofilling materials, in the healing process of periapical tissue of pulpless dogs' teeth submitted to a conventional retrofilling technique. Twenty-four premolars obtained from three dogs were used. At the first intervention, the animals were anesthetized, coronal access was obtained and pulpectomy was done. Root canals were kept open to the oral environment for 180 days to induce the formation of apical lesions. After surgical removal of the lesions with curettes, 2 mm of the apical root was cut out perpendicular to the long axis of the teeth, and root-end cavities were shaped with a low-speed round bur. The bone cavities were irrigated and dried, and the root-end cavities were filled with MTA, IRM, Super EBA and ZOE in a puttylike consistency. The bone cavities were passively filled with blood and flaps were sutured. The coronal access openings were cleaned and double-sealed with ZOE and amalgam. After 180 days, the animals were killed by anesthetic overdose, maxilla and mandible were removed and the pieces were processed for histomorphologic analysis. Data were evaluated blindly on the basis of several histopathologic events and the scores obtained were analyzed statistically using the Kruskal Wallis test. No significant differences were observed among MTA, Super EBA and IRM (p>0.05). However, ZOE had a significantly more negative influence on the apical healing (p<0.05). In conclusion, MTA, Super EBA and IRM had similar histopathologic effects among each other and better performance than ZOE used in a puttylike consistency. Furthermore, only MTA stimulated hard tissue deposition in direct contact with the retrofilling material, even when it was inserted under critical conditions.

Vitamin D-resistant rickets (familial hypophosphatemia) is a systemic disease secondary to defective renal-tubular reabsorption of phosphate. The major oral manifestations are spontaneous abscesses in a caries-free dentition. Pulpotomies or extractions were frequently described in the dental literature as the therapies of choice. This article presents a new prophylactic approach utilizing zinc oxide--eugenol pulpectomies and full crown coverage in an attempt to retain the primary dentition and prevent abscess formation.

Congenital Rubella Syndrome is a rare disorder comprised of a constellation of physical abnormalities that develop in infants as a result of maternal infection and subsequent fetal infection with rubella virus. The congenital lesions involve vital organs such as heart, eye, ear, brain and endocrine system and less frequently, teeth. The severity of systemic involvement depends on the stage of gestation at which maternal rubella infection occurs. With the implementation of immunization programs worldwide, its incidence has been dramatically reduced during the past half century. This article provides an insight into the prolonged effect of the virus on ameloblasts by highlighting the presence of hypoplastic enamel in primary teeth and erupting permanent teeth in a female child diagnosed with congenital rubella syndrome.
