Background: The past four decades have seen a steady increase in thyroid cancer in the United States (US). This study investigated the impact of the American Thyroid Association (ATA)’s revised cancer management guidelines on thyroid cancer incidence trends and how the trends varied by socioeconomic, histologic, geographic, and racial and ethnic characteristics from 2000 to 2020. Methods: We used data from the Surveillance, Epidemiology, and End Results (SEER) database to identify thyroid cancer cases diagnosed among US patients between 2000 and 2020. We employed joinpoint regression software to fit, assess, and compare thyroid cancer incidence trends over time stratified by socioeconomic status (SES), histologic type, geographic location, and race/ethnicity. Results: Between 2000 and 2009, there was an average annual increase of 5.8% in thyroid cancer incidence (average annual percent change (AAPC): 5.8, p < 0.05). Subsequently, there was a modest rise (AAPC: 1.1, p < 0.05) from 2010 to 2015, followed by a significant annual decrease of 4.8% from 2016 to 2020 (AAPC: −4.8, p < 0.05). The joinpoint regression models identified prominent inflection points around 2009 and 2015, aligning with the years of the ATA’s cancer management revisions. These intricate dynamics in thyroid cancer incidence trends from 2000 to 2020 were shaped by SES and histologic, geographic, and racial/ethnic factors. Conclusions: Thyroid cancer incidence trends over the past two decades can be partially explained by the changes in thyroid cancer screening and management recommendations. These findings underscore the importance of cancer management strategies and highlight the need for targeted interventions to address disparities in thyroid cancer incidence across minority demographic groups. Background: There has been a growing interest in using inertial sensors to explore the temporal aspects of the Timed Up and Go (TUG) test. The current study aimed to analyze the spatiotemporal parameters and phases of the TUG test in patients with knee osteoarthritis (KOA) and compare the results with those of non-arthritic individuals. Methods: This study included 20 patients with KOA and 60 non-arthritic individuals aged 65 to 84 years. All participants performed the TUG test, and 17 spatiotemporal parameters and phase data were collected wirelessly using the BTS G-Walk inertial sensor. Results: Significant mobility impairments were observed in KOA patients, including slower gait speed, impaired sit-to-stand transitions, and reduced turning efficiency. These findings highlight functional deficits in individuals with KOA compared to their non-arthritic counterparts. Conclusions: The results emphasize the need for targeted physiotherapy interventions, such as quadriceps strengthening, balance training, and gait retraining, to address these deficits. However, the study is limited by its small sample size, gender imbalance, and limited validation of the BTS G-Walk device. Future research should include larger, more balanced cohorts, validate sensor reliability, and conduct longitudinal studies. Despite these limitations, the findings align with previous research and underscore the potential of inertial sensors in tailoring rehabilitation strategies and monitoring progress in KOA patients. Effective blood glucose management is essential for individuals with type 1 diabetes, particularly when dietary adjustments involve staple foods like rice. As a primary carbohydrate worldwide, rice significantly influences the glycemic index (GI) based on its type and cooking method. This study investigated the impact of rice type and boiling duration on the GI in healthy adults using an in vivo approach aligned with ISO 2010 standards. The glycemic response to four rice types (white round-grain, parboiled medium-grain, white long-grain, and whole-grain long-grain) was measured through postprandial blood glucose levels under both standard and extended boiling conditions to assess their implications for dietary glycemic control. Ten healthy participants (mean age 25 years, body mass index (BMI) 23.0 ± 1.6 kg/m2) consumed rice samples containing 50 g of available carbohydrates, prepared under controlled boiling conditions. Postprandial glycemic response was measured at regular intervals over 2 h following ingestion, with glucose solution as a reference food. The GI was calculated based on the incremental area under the glycemic response curve for each rice sample. Extended boiling significantly increased the GI across all rice types. White round-grain rice exhibited the highest relative increase (+15.8%) in the GI, while whole-grain long-grain rice, despite showing a greater percentage increase (+25.4%), maintained the lowest overall GI due to its high amylose and fiber content. Rice types with higher amylopectin content demonstrated faster glycemic responses and higher GI compared to high-amylose types. This study highlights rice type and cooking time as critical factors influencing postprandial glycemic response. Shorter boiling durations may benefit individuals requiring strict glycemic control, particularly those with diabetes, underscoring the importance of personalized dietary guidance for managing glycemic outcomes effectively. In addition to their practical and aesthetic qualities, ceramic teapots are highly decorative and stylish. Based on the theory of perceptual engineering, this study employs eye-tracking technology and semantic-difference methods to investigate user preferences for ceramic teapot shapes. Using eye-movement experiments, the study first determines users’ visual attention to different morphological regions. Using the orthogonal alignment method, nine styling samples were developed by combining expert classification of classic and traditional teapot styling elements. By combining a semantic perception questionnaire with a satisfaction questionnaire, the study evaluated users’ visual attention to these samples and their satisfaction with them. It was found that shapes characterized by classic, rounded, and proportional coordination were more in line with consumers’ aesthetic preferences despite the differences in semantic evaluations among consumers of different genders and ages, which led to relatively consistent consumer satisfaction. The purpose of this study is not only to provide a scientific basis for styling ceramic teapots but also to assist designers in grasping the laws of consumer preference in order to create better products. Objectives: This study aimed to evaluate the effectiveness of a novel hybrid aligner system, Geniova Technologies™ (GT), for arch expansion and to compare the predictability of its virtual setup with a conventional clear aligner system (CA) after the first treatment phase. Materials and Methods: Forty (mean age: 31.3 years for GT, 38.4 years for CA) adult patients with maxillary dentoalveolar compression and anterior crowding >3 mm were enrolled and assigned to GT and CA groups. Transverse changes for canines and premolars were measured at the cusp and cervical levels. Results: No significant baseline differences in transverse dimensions were found between groups. Treatment duration (4.25 months for GT vs. 9.75 months for CA) and the number of aligners (4.25 in GT vs. 28.25 in CA) significantly differed (p < 0.001). At the cusp level, mean transverse expansions for the maxillary first premolars were 2.78 mm (GT) and 2.44 mm (CA). However, effectiveness comparisons revealed no significant differences in expansion outcomes, with both groups showing similar accuracy. Conclusions: The GT group achieved comparable dentoalveolar expansion of canines and premolars in significantly less time and with fewer aligners than the CA group. The predictability of virtual setup measurements was similar for both systems, confirming their comparable performance in achieving planned expansion. Full article

The development of the subway system in Shenyang City, China, plays a vital role in alleviating traffic congestion and promoting sustainable societal growth. However, the deformation of the surface caused by the tunneling of the shield presents a significant threat to the structural integrity of Shenyang Subway Line 2 and adjacent geotechnical structures. To tackle this challenge, a set of FEA (finite element analysis) simulations were carried out to examine surface deformation under various construction scenarios for Line 2. These simulations were compared with empirical formulas and numerical analyses conducted using Midas GTS NX 2019 software, in addition to actual site measurements. The outcomes of the finite element analysis (FEA) demonstrated a closer alignment with the empirical data than with traditional formulas. The maximum deformation was observed to be approximately twice as large as the equivalent diameter at the back of the excavation face. The analysis indicated that surface deformation is inversely correlated with overburden thickness (H), soil elasticity (E), and the grout filling rate of the shield tail (ψ), while it is directly proportional to the shield’s outer diameter (D). This study provides important methods used in the shield tunneling process employed in the Shenyang subway and suggests that the developed methodologies may be applicable to similar subway projects. Background: Promoting physical activity among people living with dementia is critical to maximise physical, cognitive and social benefits; yet the lack of knowledge, skills and confidence among health professionals, informal care partners and people with dementia deters participation. As the initial phase of a larger feasibility study, co-design was employed to develop a new model of community care, ‘Changing the Focus’, to facilitate the physical activity participation of older people living with mild dementia. Methods: Co-design methodology was utilised with nine stakeholders (with experience in referring to or providing physical activity programs and/or contributing to policy and program planning) over three workshops plus individual interviews with four care partners of people with dementia. Insights were gathered on the physical activity for people with mild dementia, referral pathways were explored and ‘personas’ were developed and refined. Materials and resources to support exercise providers and referrers to work effectively with people with mild dementia were finalised. Results: Three ‘personas’ emerged from the co-design sessions, aligned with stages of behaviour change: (1) hesitant to engage; (2) preparing to engage; and (3) actively engaged. Referral pathway discussions identified challenges related to limited resources, limited knowledge, access constraints and individual factors. Opportunities were classified as using champions, streamlining processes, recognising triggers for disengagement, influencing beliefs and attitudes, and means of communication. Conclusion: This study captured the views of physical activity referrers and providers and informal care partners in an inclusive and iterative manner. The use of co-design ensured a robust approach to facilitating participation in formal and informal physical activity options for people living with mild dementia. This study has provided the necessary framework from which to develop and test training and resources for the next stage of intervention (a feasibility trial) to improve physical activity participation for people with dementia. Background: This observational study investigates the efficacy of combining local muscle vibration (LMV) therapy and kinesiotaping using the McConnell method (KMcCM) in patients with patellofemoral pain syndrome (PFPS). PFPS is a prevalent knee condition characterized by anterior or medial knee pain exacerbated by activities that overload the patellofemoral joint. Objective: The primary aim of this study was to evaluate the effectiveness of LMV combined with KMcCM in reducing pain and improving function in PFPS patients. Methods: A total of 52 participants, aged 25–85, with PFPS were included. Participants underwent LMV and KMcCM treatments three times weekly for three weeks. Pain and function were assessed using the Visual Analog Scale (VAS) and the Knee Injury and Osteoarthritis Outcome Score (KOOS) at baseline (T0) and six months post-treatment (T1). Radiological assessments of patellar alignment and biomechanics were also conducted through dynamic MRI. Results: Significant pain reduction and functional improvements were observed across all age groups. Notably, younger participants showed greater improvement compared to older participants. Among women, those in the younger age group experienced more substantial reductions in VAS scores compared to their older counterparts. KOOS scores improved significantly, indicating enhanced knee function overall. A significant decrease in VAS scores from T0 to T1 was observed across all patellar alignment groups, signifying a reduction in pain levels. However, Group 2 (Laxation and Subluxation) experienced the most substantial reduction in VAS scores at T1 compared to the other groups. These results suggest that the combination of LMV and KMcCM may be particularly effective in addressing biomechanical abnormalities associated with patellar maltracking and enhancing VMO muscle contraction, leading to more substantial improvements in these patients. Conclusions: The combination of LMV and KMcCM demonstrates promising efficacy in reducing pain and improving knee function in PFPS patients, with age and gender influencing treatment outcomes. The most significant improvements were observed in younger individuals and those with specific patellar alignment issues, highlighting the potential of this combined approach for the targeted treatment of PFPS. Background: Anterior choroidal artery (AChoA) aneurysms pose a challenge for both endovascular and clipping procedures. The eloquent territory supplied by the parent vessel has limited collateralization and its compromise can lead to significant morbidity. This study aims to analyze the clinical outcomes and procedure-related complications of clipping and endovascular treatment of AChoA aneurysms to aid physician decision making. Methods: Thirty-two ruptured and unruptured AChoA aneurysms that underwent catheter angiography at a single neurovascular center between January 2000 and December 2023 were included. Either conservative management, clipping, and/or endovascular treatment were performed. Clinical outcomes and complications were analyzed retrospectively. Results: Twenty-four endovascular treatments and seven clipping procedures were included. Of the total 24 endovascular procedures, 46% were primary coiling, 25% were balloon-assisted coiling, 13% were flow diverting stent, 8% were combined balloon-assisted coiling and flow diverting stent, and 8% were combined balloon-assisted and stent-assisted coiling. There was no procedure-related mortality in both groups. No intra- or post-procedural ruptures/re-ruptures occurred during follow-up in both endovascular and clipping cohorts. AChoA territory infarcts occurred in 4% of the endovascular and 29% of the clipping cohorts. Other thromboembolic complications occurred in 4% of the endovascular cohort. The recurrence rate requiring retreatment was 12.5% for the endovascular and 43% for the clipping cohort. A favorable clinical outcome (mRS ≤ 2) was 78% for the endovascular cohort and 67% for the clipping cohort. Conclusions: Endovascular and clipping outcomes align with prior studies, with endovascular showing a favorable safety profile. Both approaches are viable, though they present distinct risks and advantages. Full article

This study aimed to enhance the nutritional value of jellies by fortification with polyphenol extracts derived from Pinus sylvestris L. shoots at various maturation stages. Pinus sylvestris L., a coniferous species, is widely used in traditional medicine and functional foods due to its antioxidant, anti-inflammatory, and antimicrobial properties. Its needles, bark, and shoots are commonly used to extract bioactive compounds such as phenolic acids and flavonoids. In the current study, extracts were derived from young shoots collected directly from natural forest environments and processed using a decoction method to preserve bioactive compounds. The novel jelly formulations were prepared using pine shoots harvested at three maturity stages: stage I (4 cm), stage II (8 cm), and stage III (12 cm). All determinations were conducted both on the pure decoction extracts and the jelly samples to ensure a comprehensive analysis. High-performance liquid chromatography coupled with electrospray ionization mass spectrometry (HPLC-ESI-MS) allowed the identification of eight phenolic acids and six flavonoids in the samples. Significant differences were observed between the pine shoot extracts and jellies at different development stages. Notably, stage II exhibited optimal polyphenol content (312.2 mg GAE/100 g), DPPH free radical scavenging activity (94.9%), dry matter content (79.5%), and acidity (0.79% citric acid/g). A similar pattern emerged in the jelly samples (jelly2 (pine decoction stage II) > jelly1 (pine decoction stage I) > jelly3 (pine decoction stage III)). All extracts demonstrated antioxidant potential in DPPH free radical quenching assays. FTIR analysis evaluated structural changes in phenolic compounds during jelly formulation, focusing on key absorption bands at 1600 cm−1 (C=C stretching) and 3336 cm−1 (-OH stretching) using a Shimadzu IR Prestige-21 spectrophotometer. Compared to extracts, jellies showed diminished band intensities, indicating thermal degradation of phenolic compounds during processing. This aligns with observed reductions in antioxidant capacity and phenolic content, suggesting partial destabilization of these bioactive compounds. However, their integration into the jelly matrix highlights the potential for functional applications. The textural attributes of jellies were also assessed, and differences were attributed to the changes in acidity and moisture content of the pine shoots during maturation. Pine shoot extracts at specific maturation stages are valuable sources of antioxidant and polyphenol compounds and were successfully employed in functional applications belonging to the food or nutraceutical industry. In arid Turkana County, over 90% of the population is food insecure, and wild edible plants (WEPs) provide 12–30% of dietary intake. However, climate change and overexploitation threaten these crucial resources. This study employed sequential qualitative methods to investigate community perceptions, conservation priorities for WEPs, barriers, and necessary actions in Turkana. It combined participatory community workshops and expert validation interviews. The research revealed critical threats to WEP availability, including climate change, shifting cultural practices, and a lack of natural regeneration. Key conservation barriers included intergenerational knowledge gaps, inadequate policy implementation, and conflicts between immediate needs and long-term conservation goals. In developing conservation plans, the stakeholders identified and prioritized WEP species based on food value, medicinal properties, cultural significance, utility, and drought resistance. The co-developed conservation strategy emphasized both in situ protection measures, such as community awareness programs and local policy enforcement mechanisms, and restoration actions that include planting prioritized WEPs in home gardens and community spaces. Collaborative roles for communities, non-governmental organizations, researchers, and government actors were identified to provide training, resources, and technical support. This strategy also emphasizes the need for incentivization through food/cash-for-work programs and small business grants to promote alternative livelihoods. The strategies align with some of the most-utilized conservation frameworks and principles, and present new ideas such as integrating indigenous knowledge. Expert validation confirmed the feasibility of proposed actions, highlighting the importance of multi-stakeholder approaches. This study contributes to expanding our knowledge base on community-based conservation and provides insights for policymakers, emphasizing WEPs’ critical role in food security, cultural preservation, and ecological resilience. The findings could serve as a model for similar initiatives in other arid regions facing comparable challenges. Full article

Mining activities lead to significant environmental degradation, including soil erosion, water pollution, and biodiversity loss. In South Korea, abandoned coal mines cause considerable ecological disturbances in mountainous regions. Forest rehabilitation has been proposed as a strategy to mitigate these impacts, but its effectiveness depends on successfully integrating ecosystem services (ES). This study assesses the social value of ES in post-mining rehabilitation by incorporating perspectives from local communities and experts in forestry and mining sectors. A mixed-methods approach involving surveys and interviews was employed to gather stakeholder views on 18 ES, including provisioning, regulating, cultural, and habitat services. Results indicate that local communities prioritize cultural and regulating services, such as mental health, aesthetic value, and climate regulation, while experts emphasize regulating services like soil erosion control and carbon sequestration. This divergence highlights the need for a balanced approach that integrates both ecological and socio-cultural benefits, suggesting that community needs have not been adequately reflected in current practices. The study findings underscore the importance of incorporating community input into forest management to ensure both ecological outcomes and social value, offering a framework for adaptive management that aligns ecological goals with community needs, ultimately promoting sustainability and resilience in post-mining landscapes. This article draws on doctoral research conducted at the University of Glasgow and examines the role of youth work and participatory research in shaping young people’s wellbeing in post-COVID-19 Scotland. This study focuses on how participatory methods, such as photovoice and photo-elicitation, can foster socio-political awareness and agency among young people, engaging Scottish youth and local stakeholders. Using empowerment theory, critical pedagogy, and feminist theory, this research reveals how youth participation contributes to personal and collective empowerment, offering insights into the complexities of supporting youth wellbeing. The findings indicate that young people gained control and awareness of community needs through youth work and research engagement, reflecting critical consciousness. However, this study highlights challenges in achieving sustainable change, as structural barriers often constrain the transformative potential of these initiatives. Despite the safe spaces for connection and skill-building provided by youth work, some participants experienced disillusionment when their contributions did not lead to tangible outcomes. This study recommends more inclusive and responsive youth work practices, emphasising the importance of genuine youth-led decision-making and aligning stakeholder efforts with youth aspirations. It contributes to discussions on youth agency, community engagement, and the critical role of supportive relationships in fostering resilience post-pandemic. Full article

Background/Objectives: In our society, as well as in many other parts of the world, sexuality is shaped through gender-differentiated socialization. This process compels individuals to align their desires, behaviors, emotions, and thoughts with the expectations of normative sexuality, especially hegemonic heterosexuality. The primary objective of this current research was to examine the influence of hegemonic masculinity on the sexuality of men struggling with addiction. Method: This study employed a qualitative approach, specifically using conventional content analysis. To ensure research quality and transparency, the study adhered to the Standards for Reporting Qualitative Research (SRQR) and the Consolidated Criteria for Reporting Qualitative Studies (COREQ). Fourteen participants from a therapeutic community for addiction treatment in Spain were selected through purposive sampling. Data collection involved semi-structured interviews, supplemented by participant observation. The sample size was determined based on the principle of data saturation. Results: The findings reveal that factors such as gender-differentiated socialization, particularly the values associated with hegemonic masculinity (e.g., the pressure to maintain constant sexual availability and sexual initiative), alongside excessive engagement with prostitution and pornography, are closely linked to substance abuse, the development of addictions, and the emergence of sexual dysfunctions in men. The study also highlights the role of challenges related to oppression rooted in heteronormativity—understood as the imposition of rigid norms governing sexual orientation and behavior—in shaping sexual problems and in the origin and maintenance of addictions in men. Conclusions: It is essential to implement an intervention strategy that promotes egalitarian masculinities within the treatment of men with addiction issues, emphasizing a biopsychosocial approach to sexuality that integrates a gender perspective. Additionally, it is crucial to incorporate gender-sensitive interventions into rehabilitation programs for both men and women, ensuring a comprehensive understanding of each group’s specific needs and fostering the development of healthy, equitable relationships in the context of addiction treatment. ChatGPT’s Performance in Spinal Metastasis Cases—Can We Discuss Our Complex Cases with ChatGPT?

Background: The integration of artificial intelligence (AI), particularly large language models (LLMs) like ChatGPT-4, is transforming healthcare. ChatGPT’s potential to assist in decision-making for complex cases, such as spinal metastasis treatment, is promising but widely untested. Especially in cancer patients who develop spinal metastases, precise and personalized treatment is essential. This study examines ChatGPT-4’s performance in treatment planning for spinal metastasis cases compared to experienced spine surgeons. Materials and Methods: Five spine metastasis cases were randomly selected from recent literature. Consequently, five spine surgeons and ChatGPT-4 were tasked with providing treatment recommendations for each case in a standardized manner. Responses were analyzed for frequency distribution, agreement, and subjective rater opinions. Results: ChatGPT’s treatment recommendations aligned with the majority of human raters in 73% of treatment choices, with moderate to substantial agreement on systemic therapy, pain management, and supportive care. However, ChatGPT’s recommendations tended towards generalized statements, with raters noting its generalized answers. Agreement among raters improved in sensitivity analyses excluding ChatGPT, particularly for controversial areas like surgical intervention and palliative care. Conclusions: ChatGPT shows potential in aligning with experienced surgeons on certain treatment aspects of spinal metastasis. However, its generalized approach highlights limitations, suggesting that training with specific clinical guidelines could potentially enhance its utility in complex case management. Further studies are necessary to refine AI applications in personalized healthcare decision-making. Full article
