They are no longer rough and flaky, but rather smooth, REGULAR HEALTHY skin is now there (only thing that remains is a mark where the patch once was, this should be easy to get rid of with cocoa butter or some kind of skin cream). The smaller tiny patches have completely healed up like popping a pimple.. Just a mark remains and the ones on my arms are beginning to heal up very nicely. Causes of Psoriasis
The exact cause of psoriasis is not known. It is believed that a combination of several factors contributes to the development of this disease. Some common causes of psoriasis are the following:

The use of curd in the form of buttermilk has proved useful in psoriasis and the patient should drink it in liberal quantities. Do not control natural urges like vomiting, urination, bowel emptying etc. Avoid taking spicy food and consume only easily digestible food. Psoriasis affects one to two per cent of the population in the United Kingdom (i.e. upwards of a million people in the UK). It is a chronic relapsing condition, mild to moderate involvement can usually be managed in primary care. The degree of psychological and social disability that accompanies psoriasis is commonly under estimated by the medical profession and this can result in sub-optimum care. There is no cure for psoriasis although there are effective suppressive treatments aimed at inducing remission or making the amount of psoriasis tolerable for the patient. For the majority of patients, the disease follows a chronic course interspersed with periods of remission. Relapses are difficult to predict.
Well defined, circumscribed, varying sized plaques of erythematous (described as "salmon pink") skin summonted by varying thickness of silvery scale; most often located on elbows and/or knees; any skin siteI can be affected (see clinicasl features inindividual subsections on psoriasis types below). In the scalp there may be very thick scales. The finger (and toe) nails may be deformed with thickening, pitting and discolouration. Differential diagnosis includes eczema (especialy discoid and seborrhoeic eczema); tinea (fungus) infection; rarely cutaneous Tcell lymnphoma.
Suggested Investigations
The physician should make the patient aware of the possible therapeutic options including the simplest available therapies. The need for treatment will often be dictated by the patient's own perception of his/her disability. In order to advise the patient on suitable therapies, it is important to know the sites of involvement. Certain drugs are thought to precipitate or worsen psoriasis including alcohol, beta blockers, non-steroidal anti-inflammatories, some ACE inhibitors. Oral administration of lithium, chloroquine or mepacrine can be associated with the severe deterioration of psoriasis. It is helpful to record patients' views of the most upsetting aspect of their psoriasis so that management strategies can be directed appropriately within the therapeutic limitations based on risk benefit ratio. There are several forms of psoriasis and an individual during his or her lifetime may move from one type to another and the extent of involvement can range from trivial to almost total coverage. Psoriasis can change from stable plaque to unstable patterns, typified by eruptive inflammatory lesions easily irritated by topical treatments. Rarely is the diagnosis difficult as all practitioners will be familiar with the usual patterns of psoriasis but the type of psoriasis does influence the treatment as set out below.
purification takes place so effectiveness is reduced. Preparations listed tend to be purified. This is very much a last resort treatment. The improvement with tar is slow. The preparation can be applied twice daily but is best used at night, covered with old pyjamas etc. to prevent staining of bed clothes. Third line treatment: Dithranol (see short contact Dithranol patient advice sheet). An excellent preparation and is probably the most effective topical treatment for plaque psoriasis but its disadvantages are that it irritates, needs to be used accurately and carefully, stains both skin and clothing and is not suitable for flexural skin sites or the face. If compliance is good psoriasis will clear completely over four to eight weeks. Preparations include Dithrocream, micanol (purified dithranol less staining) and Psorin (tar/salicylic acid/dithranol proprietary mixture). Topical Streoids: Are not usually recommended for treatment of chronic plaque psoriasis as the skin can become thinned and the psoriasis may become unstable. However, a proprietary Vitamin D3 analogue/steroid mix is available (Dovobet) and can be used for 4 weeks for revatively limited psoriasis but should NOT BE USED FOR MAINTENANCE. Whilst the treatment response for Dovobet is quicker than for Vitamin D3 analogues alone, but the degree of clearance beyond 4 weeks is not significantly increased. Chronic plaque psoriasis but particularly thickened. Pre-treat skin with salicylic acid e.g. salicylic acid ointment, BP (2%) to reduce scaling which otherwise acts as a barrier to prevent penetration of active ingredients in other preparations. If BP formulation difficult to obtain, tar/salicycilic acid mixture (eg Psoriderm) will fulfill the same function. Usually twice daily for one week with be sufficient. Proceed as for standard plaque psoriasis. Inflamed Psoriasis If psoriasis angry, sore or itchy, use moderately potent topical steroid, e.g. Eumovate ointment each day for one week, then introduce Calcipotriol ointment (preferable to cream as more effective) or weak tar. Withdraw topical steroid. Scalp psoriasis. If mild use tar shampoo (e.g. Polytar, Capasal,T-gel) plus either Calcipotriol scalp solution once or twice daily if psoriasis uninflamed. If scalp inflamed or itchy use topical steroid lotion, e.g. Betnovate lotion, Betamousse, Diprosalic, Synalar gel once or twice daily. (Warn patient to make sure lotion/gel applied sparingly so it does not run down forehead and face). A Vitamin D3 analogue/topical steroid mixture scalp application (Xamiol) is now available - the scalp equivalent of Dovobet; this can be used over-night but when shampooing out in the morning, the hair should be shampooed DRY - just shampoo, no water then rinse out normally; use for upto one month then have a break of 1 month before reusing. Thick scalp psoriasis. It is best to remove scale over night by applying Cocois liberally over night (covered with a shower cap, both to protect bedding and to enhance effect by hydrating scalp and softening the scale). Wash out with tar shampoo in the morning as above and then use as for standard scalp psoriasis by day (Calcipotriol or steroid lotion or combination). If scale very adherent, apply Dermol 600 for 15-20minutes, massage into scalp to loosen scale then removed with fine toothed comb before washing with tar shampoo. Xamiol can be used by day (as for scalp psoriasis above as an alternative to Calcipotriol or steroid lotion) in conjunction with the overnight tar application but do not wash out before applying the tar. Facial psoriasis. Use emollients and if that is insufficient add moderate potency topical steroid ointment (e.g. Eumovate) over night or tar/steroid combination (e.g. Alphosyl HC) overnight. Guttate psoriasis (numerous scaly droplet lesions over trunk which may follow streptococcal infection most common in children and adolescents and self-limiting after four to six months). Widespread lesions best treated by emollients, Vitamin D3 analogue cream/ointment, +/- topical tar and tar solution in bath (preparations as above). Check throat swab and if strep present treat with systemic antibiotic. Flexural psoriasis (psoriasis in axillae, groins, natal cleft, sub-mammary etc. will look different because the scaling is reduced in hydrated areas). Most commonly seen in the elderly. May be secondarily infected with yeasts.
Treat with moderately potent topical steroid antibiotic combination (e.g. Trimovate cream overnight). Some patients will tolerate tar preparations or Vitamin D3 analogues but in the majority such preparations will cause soreness. Psoriasis of palms and soles (often misdiagnosed as eczema). Usually very well demarkated and usually symetrical. May present as pustular psoriasis (green/yellow pustules and red/brown macules amongst well demarcated scale). Exclude fungal infection (take mycological samples). Treat with potent topical steroid plus or minus salicylic acid, e.g. Diprosalic ointment by day. Addition of tar at night may help, e.g. Exorex (cover with gloves plus or minus socks to prevent staining of bedding. Treatment should be directed to controlling symptoms but warn patient that treatment usually does not clear rash completely (especially with pustular psoriasis).
