WHAT SHOULD I DO WHEN?


A. In the event of accidents/injuries: Attach copies of any relevant documents.

What are the schools injury policies? (e.g. What should you do if a child gets
injured?) Send them to the nurse on campus
What documents must you complete? N/A but if is the head type of injury
then there will injury forms to complete ex: concussion
Where is the first aid kit? In the Cafeteria or with the nurse
Who should you contact in the event of an injury? Ms. Monique

B. In the event of fire/evacuations: Attach copies of any relevant documents.

What are the schools fire procedures? If there is a fire, take them to the grass
area and line them up
Where are the fire exits? Through the office door, side gate, or grass
Where is the meeting point? Grass area and or black top

C. In the event of unauthorized access of children: Attach copies of any relevant


documents.

What are the schools procedures should an unauthorized individual attempt to


access the children? Walkie-Talkie. Take them to the cafeteria, have a room 37
if needed

D. In the event of school lockdown: Attach copies of any relevant documents.

What are the schools procedures should a school lockdown be enacted? Room
37. Cafeteria or computer room. Restroom if needed to hide

ABOUT THE ACTIVITY SPACE:

A. Risk assessment of activity space: Identify all potential hazards in the


OUTDOOR activity space (draw a map of the area and indicated all potential
hazards). There are small holes in the grass. On the big blacktop space, the
basketball poles can be hazardous if they run into them
B. Risk assessment of activity space: Identify all potential hazards in the
INDOOR activity space (draw a map of the area and indicated all potential
hazards). The cafeteria has all the tables that will have to pushed to the side if
we need to use indoor space
C. Where are the closest restrooms and water fountains?
There are bathrooms and water fountains right outside the activity space. If
teachers need to use the bathrooms, the teacher lounge can be used.

ABOUT MY STUDENTS:
A. Risk assessment of students medical needs:
- Identify all students with specialized medical needs. Give a sheet with
student and medical
- Be aware of student profile/medication and identify school medical
personnel responsible for these students

PRE-PROGRAM PREPARATION:
A. Where is the equipment located and how will I access it? Equipment in the
shed and the key is in the gym
B.
C. Where will I go if it rains? Cafeteria
D. How will I make sure the students are adequately protected from the
weather (sun/rain/wind etc)? Have the students go in the shade. Give
them water breaks
E. What will I do if a student is not appropriately dressed for Physical
Education? Will be prepared, if not they can sit out and watch the lesson
F. Where can I leave my personal belongings? On the bench
G. What access, if any, do I have to the teachers lounge/facilities? N/A
