WHAT SHOULD I DO WHEN?


A. In the event of accidents/injuries: Attach copies of any relevant documents.

What are the schools injury policies? (e.g. What should you do if a child gets
injured?) there is a first aid kit with band aids
What documents must you complete? If the injury is bad we will fill out an
incident report
Where is the first aid kit? In the cafeteria closet
Who should you contact in the event of an injury? Ms. Monique an she will
contact the parents if it is serious

B. In the event of fire/evacuations: Attach copies of any relevant documents.

What are the schools fire procedures? If a fire drill then go to the grass,
actual fire then the front gate
Where are the fire exits? The front gate
Where is the meeting point? Grass area

C. In the event of unauthorized access of children: Attach copies of any relevant


documents.

What are the schools procedures should an unauthorized individual attempt to


access the children? Go to the closet open door and get the kids inside

D. In the event of school lockdown: Attach copies of any relevant documents.

What are the schools procedures should a school lockdown be enacted? Get
the kids into the nearest place with a door, the restrooms would be the
safest

ABOUT THE ACTIVITY SPACE:


A. Risk assessment of activity space: Identify all potential hazards in the
OUTDOOR activity space (draw a map of the area and indicated all potential
hazards). Map of school is attached. Potential hazards of kids running into
the basketball poles
B. Risk assessment of activity space: Identify all potential hazards in the
INDOOR activity space (draw a map of the area and indicated all potential
hazards). Inside hazards could be not enough space for activities
C. Where are the closest restrooms and water fountains?

ABOUT MY STUDENTS:
A. Risk assessment of students medical needs:
- Identify all students with specialized medical needs.
- Be aware of student profile/medication and identify school medical
personnel responsible for these students to my knowledge none of my
kids have medical needs or disabilities

PRE-PROGRAM PREPARATION:
A. Where is the equipment located and how will I access it? Equipment is in
the shed and the keys are in the box or the staff will have them
B. Where will I go if it rains? Into the cafeteria or the classroom
C. How will I make sure the students are adequately protected from the
weather (sun/rain/wind etc)? Try to keep them in shaded areas
D. What will I do if a student is not appropriately dressed for Physical
Education? if the kids are not appropriately dressed then they will be sent to
ms. Monique
E. Where can I leave my personal belongings? I can leave my things in ms.
Monique
F. What access, if any, do I have to the teachers lounge/facilities? We can use
the bathroom in the cafeteria and there is water in the faculty lounge
