During an audit on May 13, 2015, the auditor could not find details recorded in the logsheet for a lot running on machine BAL04 or ball placement accuracy and stencil conversion buyoff results for a customer order. As immediate containment, the responsible party checked and found the auditor referred to an old logsheet. The root causes were identified through questioning, with the last answer leading to a systemic or technical issue. The corrective actions to prevent recurrence included always recording lot details in the logsheet and compiling and storing logsheets by equipment in the retention room. The completion date was set within 30 days of issuing the NCR
