FMEA
The Failure Modes and Effects Analysis, also called as FunctionsComponentsParameters Analysis, is a highly effective method for carrying out analysis of failure of engineering systems, both complex and simple. FMEA is a systematic procedure for which standard forms have been designed and are provided to the analyst.

FMEA

Developed by the Aerospace industry in the 1960s Spread to the Automotive industry Now used extensively across all industry sectors QS9000 FMEA reference manual(SAE J 1739)

Failure

Failure is the partial or total loss or change of those properties of a device (or system) in such a way that its functioning is seriously or completely stopped Or Simply a component fails when it stop doing its required function. Causes of Failure can be intrinsic such as weakness in the component and extrinsic such as mishandling of component or process etc. eg. corrosion, oxidation etc.

Mode

Failure Mode defines the manner or mode in which failure occurred. eg. cracks in the component, seizure etc.

Effect

Effect stands for consequences of the failure. Consequence can be nonrelevant, partial, complete or critical failure.

FMEA functions :

FMEA establishes the criticality of the sub-systems, sub-subsystems, of the system. Helps to reduce failures. Helps to separate out those failure modes that causes the maximum damage to the product or process. Such failure modes are called as Critical Failure Modes. To find the failure of any item will affect other items in the product or process. Helps to provide training to needful employees. Helps to produce a product that meets the customer requirements.

Steps in FMEA :

The location of the item or equipment in the system is identified for reference purposes. The function details of the item are specified. The conditions to which the item is subjected under normal and abnormal circumstances are specified, e.g. high vibration and stress, acid environment. All conceivable modes of failure are listed. This is an important phase in the analysis of failures. The grading of failures is carried out based on the severity of the failure. Identify the cause of failures. Recommend corrective action and report to the concerned authority.

Structure of FMEA document :


Product/Process : Clearly indicate the exact product or process that is being analyzed. Date and prepared by : FMEA number and reference number Potential Failure Modes : Clearly indicate the manner in which failure has occurred. Potential Effects of Failure : For each failure, indicate the potential impact of failure. Severity : Indication the severity of the failure. Potential Cause of Failure : Indication of various causes of failure. Current Design Controls : Record of existing design controls to reduce failures. Recommended actions : Record of list of actions to eliminate failures.
