Notes (Enrolling Japan Distributor)


To enroll a Japanese distributor, the sponsor must purchase a Japan Business Information Packet.
The Information Packet will include all Japan business information and a Gaiyoshomen. The
Gaiyoshomen contains an 11 digit unique enrollment number which must be entered at the top
portion of the enrollment page.
Under Personal Information, if you can't enter Kanji, Romaji will be accepted.
Bank Account Information must be entered.
Purchasing Distributor Kit, there are two types: Simplified* and Complete/Full

National ID may/ may not be required, depending on country.

3. Click Virtual Office and log onto your VO with your distributor ID number and password

Compliance Notice:
New distributors must submit proof of identity within 45 days of their enrollment or before
your commissions are paid. Failure to do so will result in an automatic suspension of the
distributor account and all pending commissions until proof has been received.

You must submit a photocopy of your Hong Kong Identity Card (HKID).
Include your distributor ID and contact phone number along with the photocopy.

The MonaVie Product Purchase Agreement can be printed from your Virtual Office. To print off the
document, log in to your Virtual Office at www.monavievo.com. Click the “Corporate” link located on
the left margin. Click the “Document Library” link. The Product Purchase Agreement can also be picked
up in person at the MonaVie Corporate Office.

1. Fax your completed MonaVie Purchase Agreement to 1566-2622.


Please allow 24 hours for processing.
2. You may also deliver your MonaVie order form to the MonaVie Korea office.

Enrolling in AutoShip
AutoShip is a stress-free program that allows you to establish a standing monthly order with MonaVie.
Your product will be delivered to you every 28 days. Initially, AutoShip may be set up via phone or fax to
MonaVie Distributor Support.

Korea distributors will also be allowed to use our “cascading payment” feature that is available for the
AutoShip program. You can enter a maximum of three separate payment methods and they can control
which sequence to have their listed payment methods used. To set up the “cascading payment” feature
after the AutoShip profile is created, please follow the instructions below.

How to manage the “cascading payment” feature:


Access your Virtual Office.
Click on “Manage AutoShip” (left-hand navigation).
Click on “Add Payment Method.”
Enter your additional payment method information.
Click “Save.”

d. Print the last page and mail or hand deliver directly to Taiwan office with signature and Taiwan

Option 2 - Enrol through the official MonaVie website:


a. Log onto Monavie.com
b. Select Country
c. Select Enrol (Placement is placed to sponsor’s default setting)
d. Enter Sponsor’s ID number
i. Make sure Sponsor’s ID number is correct
e. Select Country
f. Have your sponsor assist you through the online application process
g. Print the last page and mail or hand deliver directly to Taiwan office with signature and Taiwan
ID Card

a. Visit our Taiwan office with a copy of your Taiwan ID Card to fill out your application form

Distributors Enrolling as a Business (Office Only):


All Distributors enrolling as a business must follow these steps:

1. Visit our Taiwan Office


2. Fill out the Taiwan Business Application document
3. Submit your application with a copy of your principal’s Taiwan ID Card and one of the
following:
Company registration card
Business letter of approval
4. Submit your weekly GUI Invoices to the Taiwan office in order to be compensated for your
following week’s commissions. Invoices are due by the end of each Friday (TWN) before
your weekly commissions can be paid.

Compliance Notice:
New distributors enrolling without proof of identity are placed automatically on commissions hold.
These distributors must submit proof of identity within 30 days before your commissions are paid.
Failure to do so will result in an automatic suspension of the distributor account and all pending
commissions until proof of identity has been received.

Submitting proof of identity:

You must submit a photocopy of your Taiwan Identity Card


Include your distributor ID and contact phone number along with the photocopy.
o Send your proof of identity via email (Taiwan@monavie.com) or via fax (0080-114-
8987) to the Taiwan Office

b. Click link to place a personal order
c. Follow page prompts to complete process

AutoShip is a stress-free program that allows you to establish a standing monthly order with
MonaVie. Your product will be delivered to you every 28 days.

Activating AutoShip through the Virtual Office (VO):


a. Log onto Monavievo.com with your distributor ID number and password
b. Click Manage AutoShip
c. Choose the product you want to receive every 28 days
d. Choose AutoShip Date by coloured week
e. Complete Application

NOTES:
AutoShip payment is charged to a distributor’s credit card every 28 days.
To manage your AutoShip, select same coloured week as your initial order to remain active.
Coloured weeks help distributors distinguish AutoShip week.
The week of your AutoShip, active status will show “not active” until AutoShip is processed
and shipped.
Cancellation of AutoShip must be done three business days prior to the next AutoShip date.
Requests made after that may not be effective until the following shipment date.
