    5W e 1H Simple 6 questions (the name comes from English language, W and H letters)

    What? - What thing / product did you see the problem on?
    When? - When did the problem occur? Which phase in the process? Start, run, stop, …
    Where? - Where did you see the problem? Equipment or product?
    Who? - Is the problem related to skill? Are one or more operator involved?
    Which? - Which trend (pattern) did you see the problem?? When is more frequent, Monday
    morning? After shift?
    How? - How is the state changed from Optimal? How much is frequent?

    It is the tools for problems description.


    The 6 questions help to evaluate every aspect of the phenomena. An detailed and careful
    description is vital in root cause analysis.
