Subject: NON – COMPLIANCE ( by not using lock nut )

Dear Mr. Barry;

Good day!

In view of our internal inspection of two elevator system done last February 4, 2020 to the above mentioned project.
We are wondering “WHY” the installation of the lock nut was not noticed by your authorized representative during the
joint inspection with our elevator supplier last Jan. 07, 2020.

In line with this, we are asking your technical expertise if these lock nuts are necessary to installed to all the
components of the elevator or in the selected section of the structure of the said elevator.

Please see attached photos for non-compliance with regards to fastening of elevator components, by not using lock
nuts;

We are hoping for your urgent and immediate action on this matter, prior to the final inspection and turn-over of the
two units elevator.
