    AFFIDAVIT OF GUARDIANSHIP

    I, ___________________________ of legal age, and a resident of


    ____________________________________, after having been sworn in accordance
    with law, hereby depose and state that:

    1. I am the guardian in fact of the minor __________________________


    _________________, born on ______________________ and currently enrolled and
    studying at __________________________________________;

    2. Said minor is under my care and custody; I am responsible for


    providing the minor’s supervision, well-being and basic needs; as such, I take
    responsibility for the accounts of said minor and undertake to pay any of his
    debts by reason of this Affidavit;

    3. I am not an imbecile, vagrant or vicious person, a habitual drunkard


    or a criminal; I have not abandoned, neglected, nor refused to support said minor
    or have them commit any offense against the duly constituted laws of the
    Republic of the Philippines;

    4. I am competent to receive on behalf of said minor the


    “Tablet” device provided by the Local Government Unit of Pasig City to be
    used solely for distance learning and such other related activities that the school
    may require the minor to do using such “gadget”;

    5. I undertake to ensure the safekeeping, care, upkeep and expected


    maintenance of the “Tablet” device with due diligence; moreover, I take full
    responsibility for its loss, intentional damage, and unlawful use other than the
    minor to whom it was assigned; the minor is ensured access to the device for the
    purpose of which it was issued;

    6. This affidavit is executed for the purpose of assuming responsibility


    for the “Tablet” device and in compliance with the requirements of the City
    Government for its turn over to the minor.
