Certificate of origin/manufacturing date certificate shall be provided with each equipment.


We shall be responsible for checking all units after delivery, installation, calibration, comissioning, service
during warranty period, provide spares during warranty, and also provide service back up after warranty
within the frame of an annual maintenance contract.

Operation manual/service manuals shall be provided with each unit supplied.


All responsibilities of the supplier shall be fulfilled as per the terms and conditions.

Sub: Service Team

We confirm that we have a group of service personnel whose details are as under. They are trained by the
companies either locally or at various places. They are capable to carry out the service & maintenance for
products supplied or those would be supplied in future. The Complete list of our Service team members
are attached in a separate document attached along with this tender form.

Sub: Warranty
As per the tender terms we hereby confirm and agree to provide a warranty as mention in the specification
from the date of installation & commissioning. We shall also provide operational/service manual with
every equipment to be supplied. We shall also provide comprehensive user training at site and also carry
out the installation at site free of cost.
All goods to be supplied shall be new, unused, of current models incorporating the latest features as stated
in the brochure/technical specification sheet. All equipment are compatible with the power systems
within Nepal viz. (220-240volts 50-60HZ) and shall be provided with 3pins fused up to 16amps where
applicable.
We also warrant that the goods shall be free from defects arising from any act or omission of the company,
or arising from design, materials, workmanship, under normal use in the conditions prevailing here. Upon
receipt of information of any defects, we shall expeditiously repair or replace the defective good or part
thereof at no cost to yourselves. Such repairs/replacement shall be carried out within the stipulated time.

Spares shall be made available for 10yrs for major equipment and 1yrs for other items after the end of the
warranty period.

All the equipment quoted and to be supplied are appropriate in the local weather condition.

Proposal for AMC


AMC PROPOSAL:
AMC proposal can be submitted as and when requested. Service shall be offered both as preventive and
corrective. Spares shall be charged as per the prevailing prices and would be made available from us as
per requirements. There shall be 12 preventive visits in a year unless more visits are essential.

TENDER DOCUMENT

CONDITIONS OF CONTRACT
1. Nomenclature

Following words used in the contact document shall mean the following unless
otherwise stated clearly.

The Hospital means United Mission to Nepal, United Mission Hospital Tansen,
Palpa.

eTender Contact Person means the person assigned by United Mission to Nepal
to administer the eTender process and manage all queries.

Tender Return Date and Time means the strict date and time that the completed
tender bid documentation must be emailed to the eTender Contact Person. Any
bid tender which is emailed to another member of UMN or which is outside the
stated date and time will be deemed invalid.

Tender Return Date: Friday 24th July 2020


Tender Return Time: 12:00 noon

Bill of Quantities means the priced and completed Bill of quantities forming part
of the Bid.

The Tenderer means the eligible firm who intend to bid for the Tender under this
Tender Document.

The Contract is the Contract between the employer and the contractor to
execute, complete and maintain the works.

The Contract Price is the price stated in the letter of acceptance and thereafter
as adjusted in accordance with the provisions of the contract.

The Goods means all the items and their quantity mentioned in the Bills of
Quantity of this Tender document for which the suppliers have tendered or
accepted to complete or any other addition or deduction of items made by the
user thereof.

The user means the person deputed for supervising the goods for the time period
allocated by contract agreement.

Contract Period means the period between the official work order given by the
Hospital to the contractor and the time within which all the works must be
completed as per the Contract Agreement.

Days are calendar days; months are calendar months.

2. Eligible Tenders
Tenderers are based on a closed select list determined by United Mission to
Nepal.

3. Electronic Tender Document Issue


The Tender Documentation shall be issued by eTender Contact Person assigned
by the United Mission to Nepal. The contact person will handle all
communication and queries.

4. Completion of Tender Form


The Tenderer shall study the tender documentation and complete the Bill of
Quantity form as indicated. The rate of different items in the package should be
typed in figures. The Tenderer shall quote the unit rate of each item which should
include all necessary taxes such as Contract tax, local taxes etc.

5. Submission of Tender Document
The Tenderer shall study and complete the tender documentation as set out in
these instructions. The tender document should be converted to a ‘pdf’ format
and emails to the eTender Contact Person by Tender return date and time as
described above. The ‘pdf’ completed bid document and all other relevant
certificates, catalogues etc must be emailed to the eTender Contact Person
before the noted return date and time. The email reference should include the
following:

“eTender for Medical Equipment for United Mission Hospital Tansen – July
2020”

6. Necessary Documents along with Tender Document


a. Completely and properly filled Tender form issued by the Hospital
b. A copy of VAT Registration certificate and Company registration Certificate.
c. The sole authorisation letter from the manufacture company.

Note: Certificates as mentioned on 6 (c) above are to be presented at the


time of purchasing tender form.

7. Invalid Tenders
Tender lacking any of the following shall be considered invalid and thus shall be
rejected.
a. If the submitted tender form is not the one issued by the Hospital.
b. If the tender is not submitted within official due date stated in this document.
c. If such conditions that are not desired by the Hospital, are mentioned while
filling the tender form.
d. If the certificate as per article 6b of this condition of contract is not submitted.
e. If all the items in the package are not quoted.

8. Opening of Tenders

The opening and evaluation of tenders will be by the UMN appointed


Procurement Review Committee, which consists of UMN staff members who are
not in different line management of the Hospitals or Finance Team. The tender
opening will be organised for one hour after the tender return date and time. All
tenderers will be invited to this Microsoft Team Meeting. The eTender Contact
Person will share the submitted tenders to the committee at the start of the
meeting.

9. Evaluation of Tenders

The Procurement Review Committee shall check whether the tenderer has filled
the tender form issued by Hospital, whether the given information on the
documents are valid and correct, whether the demanded documents or other
obligatory documents are authentic or not. The Procurement Review Committee,
UMN or the Hospital reserves the right to accept or reject fully or partially any of
the tenderer without giving any reason thereof.

10. Contract Period


The Contract Period will be started from the Date of Agreement.

11. Warranty
Complete warranty must be of one year and additional service warranty of one
year must be provided.

12. Delivery Terms
