I, NOMER RAMBAC CURIBANG, Resident of San Vicente, San Pablo, Isabela,

after having duly sworn in accordance with law do hereby declare that:
1. That, I am the same person executing this affidavit;
2. That when a copy of my Certificate of Live Birth was issued to me by NSO, It

appears that the entry pertaining to my full name bears “NOMER RAMBAC
CURIBANG JR” which should have been “NOMER RAMBAC CURIBANG” and
the date of my birth appearing therein is “ MARCH 22, 1993” which should be
“FEBRUARY 22, 1993” and the place of my birth is “SIMANU SUR SAN
PABLO,ISABELA” which should be “SAN VICENTE SAN PABLO, ISABELA”;
3. That, I had been using as my correct name “NOMER RAMBAC CURIBANG” the
correct date of my birth which is “ FEBRUARY 22, 1993” and the correct place of
my birth which is “ SAN VICENTE, SAN PABLO, ISABELA” as my full name in
all my Public and Private transactions;
4. That those misspelled entries and which I had been using in all public and private
transactions refer to one and the same person;
5. That, I am executing this affidavit in order to attest the truthfulness of all the
foregoing statements and for all lawful intent and purposes it may serve.

AND “MAMAWAG” instead of “MAGAUAY” and “MAMAUAG”;
4. That likewise appear therein that my father’s middle initial was entered as
“MALANAO” instead of “MALLANAO”;
5. That, I had been using “MAMAUAG” as my middle initial, MAGAUAY and
MAMAUAG as my mother’s maiden name, and “MALLANAO”as my father’s middle
initial in all my Public and Private transactions;
6. That those misspelled entries and which I had been using ion all public and private
transactions refer to one and the same person;
7. That, I am executing this affidavit in order to attest the truthfulness of all the foregoing
statements and for all lawful intent and purposes it may serve.
