    I, HASIRUL BITEH, of legal age, Filipino citizen and resident of Lamitan


    City, Basilan Province, Philippines, after having been sworn to an oath in accordance
    with the law do hereby depose and say that:

    1. I personally know ARDIE JACINTO BAUL considering that he is a


    brother-in-law;

    2. However, due to inadvertence and/or oversight, his name as erroneously


    entered/recorded as ALDIE JACINTO BAUL on his Marriage Certificate
    when it should have been ARDIE JACINTO BAUL as evidence by his
    Driver’s License;

    3. I hereby certify that ARDIE JACINTO BAUL and ALDIE JACINTO


    BAUL refer to one and the same person;

    4. I make these statements under oath and in good faith in order to establish
    the fact that his true and correct name is ARDIE JACINTO BAUL;

    5. I execute this affidavit in order to attest to the truthfulness of foregoing


    narration of facts and for whatever legal purposes this may serve best.

    entered/recorded as WADI OTONG ARASID on his Hospital Record at
    Ediborah P. Yap Memorial Hospital, Lamitan City, Basilan Province, when it
    should have been WADI ARASID TOHLA as evidenced by his Certificate of
    Live Birth;

    3. I hereby certify that WADI ARASID TOHLA and WADI OTONG ARASID
    refer to one and the same person;

    4. I make these statements under oath and in good faith in order to establish the
    fact that his true and correct name is WADI ARASID TOHLA;

    Indigency issued by the Barangay Bohe-Piang, Al-Barka, Basilan Province
    when it should have been JERHAM AMIRIN HAMSALUN and MARCH
    15, 1996 as evidenced by my Certificate of Live Birth;

    3. I hereby certify that JERHAM AMIRIN HAMSALUN with birthdate on


    MARCH 15, 1996 and JERHAM A. AMSALUN with birthdate on JULY 05,
    1996 refer to one and the same person;

    4. I make these statements under oath and in good faith in order to establish the
    fact that my true and correct name and birthdate are JERHAM AMIRIN
