A client was brought to the ER following a vehicular accident. Patient is a 50 year-old male who
was hit on his left side of the car by another motor vehicle, he was ambulatory with no visible
injuries. Initial interview reveals that the client had no pain associated from the accident but is
currently experiencing chest pain. You are a nurse who was asked to further assess the client's
chest pain.
Instruction: Find any partner who will act as a patient as described by the situation. Perform the
pain assessment as indicated by the scenario given.
*Patient is lying in the hospital bed and hears a knock from the door and sees the nurse enters

Nurse: Hello sir, I am Michael and I will be your nurse for today. May I know your name sir?
Patient: Hi Nurse Michael, I am Alexander.
Nurse: Hello Mr. Abas, so how are you feeling right now? Do you feel any pain or discomfort?
Patient: Actually a while ago I was feeling ok, I was not hurt from the accident according to my
doctor, but right now I'm feeling some pain in my chest.
Nurse: Well sir what kind of pain do you feel in your chest? Is it a dull pain like there is tightness
in your chest or a sharp pain like a stabbing kind of pain?
Patient: I feel a dull pain like my chest is getting tight.
Nurse: Ok sir can you tell me the intensity of the pain, from a scale of 1-10 with 1 being least
painful and 10 being most painful how does your pain feel?
Patient: the pain is around 7-8.
Nurse: In what part of the chest do you feel the pain? Does it go anywhere else? Does it
radiate?
Patient: I feel the strong pain in my left chest and it does not radiate to other parts.
Nurse: How frequently do you feel this pain? Do you feel it hourly or more or less frequently
than that?
Patient: I just felt it a while ago again, I feel this pain less frequently about 3x a week.
Nurse: How long does the pain last?
Patient: I feel the pain for around 10-15 mins.
Nurse: When did the pain start?
Patient: I started feeling this pain last month, but I forgot the exact date
Nurse: It's ok sir, now, what do you believe causes the pain? Are there any activities that you do
when the pain occurs?
Patient: Most of the time I feel the pain when I’m stressed and too tired also when I exercise or
do extraneous activities.
Nurse: What have you been doing to alleviate this pain, and what medications have you been
taking if ever?
Patient: When I feel the pain, I take some rest and drink a lot of water. When it is too intense I
sometimes take pain relievers like advil.
Nurse: Do you feel anything else when experiencing this pain? Something like trouble breathing,
dizziness, nausea, anything like that?
Patient: Yes, i feel like i have trouble breathing, sometimes i feel super tired, very weak and
dizzy.
Nurse: Does this pain and other symptoms have any effect on your average day?
Patient: Sometimes I can't function really well because of the pain and dizziness that it delays
my work.
Nurse: Prior to this chest pain have you been experiencing any other types of pain in the past
year?
Patient: No, this is the first time I felt this. I am a very healthy person you know.
Nurse: Ok sir, so, that concludes our assessment on your pain. Thank you for answering the
questions. This will give us better insight into your condition and help us improve your care. I will
get back to you with the doctor after giving this assessment. If you need anything else or feel
the pain again, I will be at the nurse’s station just press the call button and I will come
immediately.
Patient: Ok, Thank you nurse michael.
Nurse: thank you sir.
