TOPIC: Personal Protective Equipment Checklist Employee Participants:


PPE is important to your everyday safety. Use this checklist to determine
if any corrective action is needed to make sure the personal protective
equipment being used is sufficient and safe.

□ The work place has been assessed for hazards that require the
use of personal protective equipment (PPE).
□ The work place assessment has been documented with the
name of the work place, the person doing the assessment,
and the date of the assessment.
□ The work place is re-assessed as needed when changes to the
work place or PPE are made.
□ PPE is selected based on the hazards identified in the
assessment.
□ PPE that is selected meets recognized industry standards.
□ The hazards of hazardous materials use are considered when
doing the assessment.
□ Efforts are made to ensure that employee PPE is properly
fitted.

they perform.
□ Employees have been trained on the required PPE for the job
they perform.
