PROCEDURE CHECKLIST

Chapter 38, Skill 38-01: Administering IV Medication via prn Lock or IV Line

1. Reviews the physician’s orders and the medication
administration record (MAR) for the correct patient,
medication, dose, route, and time.
2. Reviews a drug book for actions, required
assessments, correct dosage, route, side effects, expected
results, and the period of time over which the medication
may be safely administered IV. Checks for compatibility
with IV fluids.
3. Assesses the patient’s allergies.
4. Assesses the type of IV or prn lock the patient has, to
determine type of connector that will be required to
administer the IV medication.
5. Follow s the “Initial Implementation Steps.”
6. Selects the correct medication, checking for the
medication name, dosage strength, dosage volume, dosage
route and its appropriateness, and expiration date.
Assesses for cloudiness or color changes.
7. Obtains a normal saline (NS) flush syringe and checks
label three times.
8. Performs any necessary physical assessments or
checks any required laboratory results.
9. Washes hands.
10. Performs at least three medication label comparisons
to the MAR.
11. Calculates correct volume of medication to draw up.
Rechecks calculations.
12. Uses the correct syringe and gauge and length needle.
13. Cleans the ampule or vial stopper with alcohol swabs.
14. Withdraws the medication from the ampule or vial,
maintaining sterility of appropriate pieces of equipment
and medication. Dispels any air from the syringe.
15. Labels the syringe with drug, dose, route, and patient’s
name; recaps the needle before walking to the patient’s
room.
16. Once in the patient’s room, washes hands again.
17. Uses two methods of checking patient identification
and follows the “Initial Implementation Steps” found in
the text.
18. Assesses the patient’s IV site for infiltration and
phlebitis.

If Injecting Into the IV Line of a Continuous Infusion of IV Solution:
19. Cleans Y-injection port closest to the insertion site
with an alcohol wipe.
20. Aspirates to determine catheter placement in vein.
21. Assesses for air in flush syringe, removes any air
present, and then flushes line with NS. Takes 1 minute to
flush line.
22. Injects medication at the appropriate rate according to
manufacturer’s instructions, maintaining aseptic
technique.
23. If there is an IV line attached, pinches off the IV
tubing during medication administration.
If Injecting Into a prn Lock:
19. Cleans the prn lock injection port with alcohol.
20. While maintaining sterility of the syringe tip, dispels
air from the Luer-Lok syringe and attaches the syringe
with 3 to 5 mL NS to the Y-injection port of the prn lock
(or inserts a needle or blunt-tipped adapter into the
injection port).
21. Opens the slide clamp on the prn lock.
22. Aspirates with the syringe to verify blood return into
the prn lock. If blood return is noted, flush the prn
lock with 2 to 3 mL of the NS flush solution. After
completing the flush, disconnects the NS flush syringe
from the prn lock injection port.
23. Instills medication at the recommended rate according
to manufacturer’s instructions.
For Both the IV Line of a Continuous Infusion of IV Solution and prn Lock:
24. Follows medication with additional 2 to 3 mL NS
flush, continuing to monitor site for infiltration.
25. Discards the syringes in the sharps container.
26. Follows the “Ending Steps.”
27. Documents in MAR (and in nurses’ notes).
28. Evaluates effectiveness of medication.
