Chapter 26, Skill 26-04: Obtaining a Wound Culture

1. Assesses the wound and the type of dressing,
if there is one, to determine supplies needed to
replace the dressing
2. Gathers the necessary supplies for the dressing
change and supplies for the wound culture: clean
gloves, sterile culturette tube with cotton-tipped
swab.
3. Follows the “Initial Implementation Steps.”
4. Performs wound irrigation before obtaining
wound culture.
5. Holds the proximal tip of the culturette swab
and swirls the cotton-tipped end only on the
granulation tissue in the wound bed. Avoids
touching the cotton tip to the outer edges of the
wound.
6. Quickly places the distal cotton-tipped end of
the swab back into the culturette container.
7. Applies a new dressing if appropriate.
8. Labels the container with the patient’s name,
room number, and ID number, the physician’s
name, the date and time, and the name of the test
to be performed.
9. Follows the “Ending Steps.”
10. Documents procedure correctly.
