    5. Proof of the appointee’s qualification such as
    Certificate of Training of Basic PCO Training
    6. Certificate of eight (8) hour training on environmental
    management of the Managing Head
    7. Valid Professional Regulation Commission license, if
    applicable, for Category B Establishments
    8. Duly accomplished Establishment Categorization
    checklist form (download at r7.emb.gov.ph>PCO
    corner>New Application>Form 3)
    NOTE: APPLICATION IS STILL SUBJECT FOR SUBSTANTIAL REVIEW

    REMINDERS:

    1. Always review your submitted requirements and read and understand carefully the details being asked in the
    application form, affidavit of undertaking, as well as the instructions per item in the Establishment Categorization
    Checklist form to avoid returning your application.
    2. The name of establishment and establishment address of the PCO and the Managing Head in the Application Form
    and in the Affidavit should be the same as they belong to one establishment and address.
    3. The home address of the PCO in the application form and residence in the Affidavit should be the same.
    4. For reference as to the correct establishment category, always refer to Table 1 section 5 of DENR Administrative Order
    (DAO) 2014-02 or the Revised Guidelines for Pollution Control Officer Accreditation. You may google and download
    it.
    5. If you are in doubt or could not understand the remarks in the screening form, kindly call us at (032) 260-9777 local
    118, 09662705718, 09365475931, or 09365475943 and look for the undersigned or any staff from the Air and Water
    Permitting Section (AWPS) for clarification.
