REMINDERS:
1. Always review your submitted requirements and read and understand carefully the details being asked in the
application form, affidavit of undertaking, as well as the instructions per item in the Establishment Categorization
Checklist form to avoid returning your application.
2. The name of establishment and establishment address of the PCO and the Managing Head in the Application Form
and in the Affidavit should be the same as they belong to one establishment and address.
3. The home address of the PCO in the application form and residence in the Affidavit should be the same.
4. For reference as to the correct establishment category, always refer to Table 1 section 5 of DENR Administrative Order
(DAO) 2014-02 or the Revised Guidelines for Pollution Control Officer Accreditation. You may google and download
it.
