    I, BERNARD F. DOMINGO of legal age, Filipino citizen, a resident address at Blk 78 Barrio San Isidro
    Tala Brgy. 188 Dist. 1 Caloocan City, do hereby NAME, APPOINT and CONSTITUTE---- ROXANNE ALLION of
