Amaala International Airport – WP -01 HSE Observation Report

Personel lifting equipment placed on loose


soil

SRM:

Observed that SRM boom truck outrigger was


placed on loose soil without outriger pad while the
equipment was being operated. This could affect
the stability of the boom truck and lead to
unwanted incident on site

Category: (MEPI).

Recommendations:

Outrigger pads were provided and used


immediatelyimmediately. Advised SRM crane
operator to always make sure that outriggers are
fully extended and places on compact soil with
outrigger pads before lifting personel

Expired competency card

SRM:

Observed that competency card of wheel loder


operator (Esam mugahed) has expired.

Category: (training and competency).

Recommendations:

Advised SRM plant supervisor to arrange renewal


for the operator card as soon as possible

Soil contamination:

SRM:

Soil contamination as result of diesel sprayed to


lubricate SRM concrete pump.

Category: (Environmental pollution).

Recommendations:

Advised to dispose contaminated soil as per site


procedure. Spill protection (drip tray/ plastic
sheet) should be provided to control the spillage
subsequently.

Unprotected excavation edge:

TMC:

Observed that evcavted area were not barricaded


and vehicles were moving in the area. This may
lead to equipment or persons falling into the
excavation. .

Category: (Excavation).

Recommendations:

Advised TMC team to barricade the area as soon


as possible and make sure that adequate barricade
is provided for all excavatied areas going forward.

Poor Housekeeping/ Waste management:

TMC:
Observed poor housekeeping and accumulated
wood waste at taxi way from TMC. This could lead
to slip, trip or fire related incident

Category: (Housekeeping).

Recommendations:

Advised TMC to conduct proper housekeeping and


dispose wood waste as per site waste
management procedure

GOOD PRACTICES:

1. Site nurse visitingg site regularly to chech workers welfare and familiarize with site
layout in case of emergency response
2. Personel carrying out welding activity using the required task specific PPE
3. Housekeeping conducted at SRM batching plant
4. Inspection tags and new signages installed at employers office
