1. General Guidelines
All committees, candidates and person involved in Intramurals 2023 should be:
a) Fully Vaccinated with vaccination card
b) Wear Face mask all the time
c) Observe proper guidelines and social distancing (6 feet apart)
d) Practice Hand hygiene all the time by washing before, during and after activities.
e) Monitor oneself and make sure that there are no signs and symptoms of COVID-19 before going to
the venue.
f) Before entering the venue, everyone should wear a shoe cover and do hand hygiene ex. Washing
with soap and water, using 60% alcohol or hand sanitizers.
g) Participants will have to sanitize their hands every 1hr and 30 min.
h) Before the Preliminary and Coronation night, everyone should take 3 days home quarantine.
i) Everyone is mandated to bring extra clothes, personal alcohol spray, face towel, hygiene kits and
water bottle.
j) Strictly no sharing of personal things, equipment and food.
k) Only one warning will be given to those who failed to comply with the rules implemented.
i. Those who failed to follow rules shall be given punishment of dismissal or
suspension.
SIGN AND SYMPTOMS OF COVID-19:
If a person is having any of the symptoms below please refrain from participating in physical
practices for the event. Rest and recover until further notice or as the symptoms disappear.
Failure to follow will lead to disqualification for putting the people involved at risk.

g) New loss of taste or smell

LEVEL 4 and 5:
● No physical interactions. The Intramurals 2023 will be in virtual set-up or postponed until
further notice. All activities will undergo virtual setup because face-to-face interaction in
level 4 & 5 is not allowed.
LEVEL 3:
● 30% venue capacity
● Limited number of committees
● Candidates and staff will be given a schedule when to monitor and practice on the venue.
LEVEL 2:
● 50% venue capacity
LEVEL 1:
● 18 below and 65 above now allowed as long as with proper PPE
The most common symptoms of monkeypox include fever, headache, muscle aches, back pain,
low energy, and swollen lymph nodes. This is followed or accompanied by the development
of a rash which can last for two to three weeks. The rash can be found on the face, palms of the
hands, soles of the feet, eyes, mouth, throat, groin, and genital and/or anal regions of the body. The
number of lesions can range from one to several thousand. Lesions begin flat, then fill with liquid
before they crust over, dry up and fall off, with a fresh layer of skin forming underneath.
III. CERTIFICATION AND WARRANTY OF INFORMATION/STATEMENT
1. I certify that all of the following information/data and/or matters indicated in this Application Form
and such documents attached hereto are true, and that there are no false statements,
misrepresentations or omissions herein.
2. I further certify my Eligibility to Participate as stated in the Article 1 of the Revised Intramurals
rules and regulations:
Sec 1. All participants must be University of Makati students enrolled in 1st Semester
Academic Year 2022-2023
Sec 2. Sex will be the sole basis of categorization of each and every sporting events.
Sec. 3. All participants must submit a waiver prior to participating in our Intramural Sports
Program. A duly signed waiver must be submitted for every sport the participant wishes to
participate in.
(1): Students under the age of 18 may participate in our Intramural Sports Program
but must provide a consent form signed by their guardian.
Sec. 4. Participants in any sporting events under UMAK Intramurals 2022 should be by the
college.
(1): Each college including the HSU is only allowed to not have representatives in a
maximum of 3 sporting events.
Sec. 5. UMAK Athletes are not restricted to participate in the intramurals hence, they
cannot join in their specific sporting event.
(1): “X” players are allowed to join any sports category if proven that they are not a
UMAK Athlete.
Sec. 6. Anyone who is part of the working committee is not allowed to participate in all
sporting events for the Intramurals.
(1): Filing of LOA for student leaders who want to participate in the event is a must
1-2 weeks prior to the scheduled screening.
Sec. 7. Participants are required to submit all the necessary requirements below on their
respective schedule of submission otherwise their entry will be voided.
Sec. 8. Participants are only allowed to join 1 sporting category among the given 5 sporting
events.
Sec. 9. In any team sporting event such as Volleyball and Basketball, (both men’s and
women’s categories) a minimum of 8 players is enough to validate their rights to create a
team that will represent their college.
Sec. 10. Every individual will go through screening hence, it will not guarantee that every
aspiring participant will be allowed to participate in the Intramurals 2022. If the Screening
Committee and/or Panel can find a valid ground/s to void the application, therefore it can
be nullified.
3. I hereby authorize USC to verify the truth of all my assertions herein and the genuineness/validity
of all documents I have submitted, thereby waiving for these purposes the provisions of all laws,
rules and regulations, inside or outside the campus, relating to the privacy or confidentiality of the
matters hereunder.
4. I warrant that the documents I have submitted are public documents in my possession or which
came into my possession legally and/or are public documents attested to by the officer/s having the
legal custody of the records, and that, where applicable, the entries therein were made in the
regular performance of an official duty, and that I will hold USC free and harmless from any breach
of this warranty.
Section 1: Any team that allows an ineligible participant to play on their team will be
automatically disqualified from the competition.
Sec. 2. All participants are required to be at the venue 30 minutes before the scheduled game. The
grace period for accepting late participants is only 15 minutes.
(1): If the team has an incomplete number of players, the late players will not be allowed to
participate in the said sporting event as a sanction
(2): On a solo and duo sporting event, the late player will be automatically disqualified, and their
opponent will move on to the next round of the competition instead. If both players are late,
none of them will move on to the next round of the competition.
Sec. 3. Participants should submit all the requirements with the complete forms on the list
provided in Article 1, Section 6 of Eligibility to Participate. Otherwise, they will not be allowed to
play in any sporting events. The application of anyone with incomplete requirements will
automatically be voided.
Sec. 2. Any participant, who has been drinking alcohol, taking drugs, or any form of substance
abuse will be removed from the game and will be automatically disqualified.
Sec. 3. Any participant playing in a manner that is endangering other intramural sports and/or
participants, will be removed from the game and will be automatically disqualified.
Sec. 4. Starting on the day of the scheduled screening, during, and after the Intramurals 2022,
participants and committees are not allowed to post or publicize any malicious content and
confidential information using their social media profiles which will result in tarnishing other
participants and committees’ image and the image of the event as well. Disobedience towards
this rule will automatically become a subject for disqualification and a separate complaint will be
submitted to the Prefect of Discipline with the grounds of Cyber Libel and/or Slander.
Sec. 5. All concerns, appeals, queries, and other matters with regard to the event should only be
raised to the Jury of Appeals.
IV. UNDERSTANDING OF CONDITIONS
1. I understand that I am applying as a participant in the Intramurals 2023 and that the
requirements and/or limitations with regard to age, height, residency, and report grades may vary,
that accordingly my qualifications are further subject thereto.
2. I am aware that USC, in its sole discretion, may require me to submit additional and sufficient
proof of age, height, residency, report of grades, health and other documentation/information
regarding my qualifications. Should I be required to do so, I undertake to provide USC with such
requested documentation/information, and failing to comply within the period to submit, as
determined by USC, I understand that my application will not be considered complete and
submitted.
3. I understand that I have to pass the screening and meet all the requirements and qualifications
of USC before I can qualify as a participant in the Intramurals 2023. I further understand that the
basic requirements relate to information obtained from me as contained in this Application Form.
4. In considering my application to be a participant in the Intramurals 2023, I agree that all issues
relating to my eligibility as an applicant shall be determined solely by USC and that I shall abide
by its determination, which shall be final and non-appealable.
5. I am aware that if I have been chosen as an official participant in Intramurals 2023, I have no
intention to withdraw to this competition, if I do so, I will undertake sufficient sanction given by the
USC.
6. In filling out and signing this Application Form, I further understand that USC is under no
obligation to accept me as a participant. My acceptance as an official participant shall commence
only when I am formally and specially notified thereof by USC.
7. If I am chosen as an official participant, this Application Form shall be considered part and
parcel of the Official Entry Form/Contract which I shall be required to sign.
8. I understand that in signing this Application Form, I have accepted the sole discretion of USC
to accept or reject me as a participant in the Intramurals 2023 and that the decision of USC on
this application is final and non-appealable.
9. I finally certify that I have read this Application Form carefully, that I have understood the same,
and that no promises or representations were made to induce me to sign this Application Form.
