Scope and Format of Inspection

The inspector should advise the Master whether the inspection will be
conducted as a tablet-based inspection, a partial paper-based inspection or full
paper-based inspection.

The SIRE 2.0 Tablet and Camera

The inspector should advise the Master:


• Of any restrictions on the use if the tablet or camera imposed by port or
terminal regulation.
• Whether the camera is activated or deactivated for the inspection.

The Master should advise the Inspector of any documented restrictions in the
vessel’s SMS on the use of tablet outside the accommodation and camera at
any time during the inspection.

Any restrictions on the use of the tablet and camera should be noted.

Order of Inspection

The inspector should identify the expected officers and ratings relevant to the
questions assigned to the CVIQ.

The Master should advise the identity of the accompanying officer for each
area of the inspection and any additional officers or crew that will be needed to
support the accompanying officer.

Defect List

The Master should provide the inspector with a list of all open defects entered
in the vessel’s defect reporting system (as required by VIQ.2.4.1).

All defects existing aboard the vessel at the time of inspection, except sudden
failures on the day of the inspection, should be included in this list.

Defect Reporting & Management

• Master should provide a list of ALL open defects entered in the defect reporting system to
inspector during the opening meeting.

• All defects existing onboard the vessel at the time of inspection, except sudden failures on
the day of the inspection, should be included in the list of open defect reports.

• If a defect is properly recorded in the defect reporting system and mitigating actions are
documented in place, there are no grounds for a negative observation to be raised. If not, a
negative observation (High Risk) will be recorded in the report.

• Notifications should be made in accordance with Class and Flag requirements.

• Where an observed substandard condition or defect indicates an applicable SSSCL


question, a negative observation will be recorded.

Recording Negative Observation & Closing Meeting

The inspector should advise the Master that:


• Any negative observations. will be pointed out to the accompanying officer
and recorded in the inspection Editor at the time of finding
• Any corrective action taken to rectify a negative observation will be recorded
as an additional remark within the supporting negative comment.
• The addition or removal of negative observations is tracked in the inspection
Editor. Errors will be corrected where warranted, but no negotiation will be
undertaken.

Observation Declaration

• A wireless printer will be required to print the Observation Declaration prior


to the Closing Meeting.
• If no wireless printer is available, the content of the Observation Declaration
will be dictated to the Master.
• Screen shots of the tablet are not permitted.

Observation Declaration (Cont.)

If no wireless printer is available, the inspector must review the content of the
declaration with the Master in sufficient detail so that the Master can make
notes for each observation recorded as follows:

Negative observations
• The question number.
• The type of negative observation (hardware, process, human or photo
comparison)
• The nature of concern (NOC).
• The subject of concern (SOC).
• The essential details of the negative comments.

Human observations that exceeded normal expectations:


• The question number.
• The essential details of the comment.

The definition of Subject of Concern (SOC):


A term used within the Inspection Editor application to identify what is being
reported on:

Hardware: The deficient vessel structure, machinery, outfitting or equipment


identified through the standard classification coding.

Process: The deficient procedure and/or document identified through the


TMSA-based classification coding.

Human: The rank grouping of the Observed Person (OP) or Responsible Team
(RT).

Photograph Comparison: The location of the standard photograph.

The definition of Nature of Concern (NOC):


A term used within the Inspection Editor application to identify what has been
observed.

Hardware: Identified through the standard hardware cause analysis tree.

Process: Identified through the standard process cause analysis tree.

Human: Identified by one or more Performance Influencing Factors (PIFs).
