Assessing Axillary temperature with an Electronic or Digital Thermometer
1. Check the physician’s order or nursing care plan for frequency and route.
2. Identify patient.
3. Explain procedure to the patient.
4. Gather equipment.
5. Make sure the electronic or digital thermometer is in operating condition.
6. Perform hand hygiene and don gloves if appropriate or indicated.
7. Select the appropriate site.
8. Ensure privacy by closing door or curtain.
9. Place bed at an appropriate working height to reduce back strain during skill.
10. Remove the digital thermometer from its container.
11. Wipe the thermometer using cotton moistened with alcohol from the tip to
base and discard cotton ball in appropriate receptacle.
12. Turn on the thermometer.
13. Move the patient’s clothing to expose axilla.
14. Place the tip in the center of the axilla. Have patient bring his or her arm
down and close to the body.
15. Hold the thermometer in place until you hear a beep, and then carefully
remove. Note the temperature reading.
16. Wipe the thermometer correctly by using cotton moistened with alcohol
from the base to tip and discard cotton ball in appropriate receptacle.
17. Turn the thermometer off and place it back to its container.
18. Perform hand hygiene. If wearing gloves, discard them in proper receptacle.
19. Record temperature on paper flow sheet or computerized record. Report
abnormal findings to the appropriate person. Identify assessment site if other
than oral.
