NEBOSH IG1 Mock Exam

Task 1: Duties of the Workers


1. As the article 19 of C155 states that the workers performing the job should cooperate in
fulfillment of obligations placed upon by the employer, the day supervisor didn’t give the
whole information nor handed the report of machine failure to the evening supervisor rather
just left it on the control panel of machine.
2. As the article 19 of C155 states that the worker’s representatives should be given adequate
information regarding the OHS, but here the day supervisor didn’t inform properly about the
situation and its hazards to the new evening supervisor and just told the machine was not
working properly.
3. As the article 19 of C155 states that the workers and representatives are given appropriate
safety trainings but there was only 1 first aider in Whole Park which indicates not enough
workers were trained.
4. As the article 19 of C155 states that the workers should immediately reports about the
hazardous situation and workers should not be allowed to return to the work until that danger
is mitigated but here the AM didn’t even inform the new evening supervisor but also allowed
the workers to keep working rather barricading the machine area.
5. As the recommendation 16 of R164 states that the workers should take care of their own
safety and people who may be affected, but the day supervisor didn’t fully inform about the
hazards that could occur due to operating the damaged machine to the new evening
supervisor as he told him that it is not working properly.
6. As the recommendation 16 of R164 states that the workers should take care of their own
safety and people who may be affected, but the evening supervisor didn’t consult anyone else
about the damaged machine or fully shut down the machine before reaching out to it.
7. As the recommendation 16 of R164 states that the workers should report any incident or
injury occur during the work but here was no accident recorded in the accident book.

Task 2: Determining Uninsured Costs Incurred from the


Accident
1. The injured worker can make claims in court against the park for the permanent injuries he
got due to accident as his fingers will be amputated.
2. The park may have to compensate the injured worker by giving the free medical treatment,
increased income as he lost he fingers due to the injury.
3. The park may have to consult a lawyer to deal with the legal issues and lawsuits against them
which will also cost a huge amount.
4. If the park was found to have violated the health safety codes, it could be fined or penalized.
5. If these types of accidents keeps occurring, the park will have more insurance claims which
will cost more money.
6. The park will have a negative reputation among the visitors and there will be loss of
business.
7. The threat given of permanent roles reviewing and increase of accidents could decrease the
staff morale which will result in huge turnover.
8. If the staff will feel like that their jobs are at risk due to threats and they might lose it
anytime, they will try to switch their job and find another stable job.
9. If any of the visitors become injured in the process, it could result in media involvement
which will highly damage the reputation of the park.
10. If any of the visitors become injured in the process, they might make claims and file a case
against the park which be a huge loss.
11. The park will lose the award as a result of this accident and its reputation will be damaged
which will result in loss of business.
12. If these accidents keep occurring, there will be less visitor’s satisfaction and the park may
have to shutdown completely due to loss of business.

Task 3: Influencing Health and Safety Culture


1. The AM’s were pressurized, threatened to work in any situation and keep the queue running
otherwise they will be given verbal warning and their roles will be reviewed, which indicates
that AM’s were anxious and nervous to perform their work.
2. The deadline to open the attraction forwarded one month and renovation staff was short, so
the park workers got involved in the process and double shifts were worked to meet the
deadline which indicates that employer is not concerned about the health of workers.
3. More temporary workers were hired for the job and given basic training, but no safety
inductions was conducted or any daily TBT record was found which indicates that the
employer is not complying with occupational health and safety.
4. Only a leaflet of “what to do in fire” is handed over to the workers on their first shift, but
none was given any physical or verbal training or any fire drill was conducted which shows
the poor involvement of employer in trainings.
5. Experienced supervisors were expected to train other workers to operate the machine but
they didn’t find any free time, which indicates the unimportance of health safety among the
supervisors.
6. The safe system of works was not fully understandable and needs clarification but the
supervisors were too busy to do so which is indicating that there was no importance of safe
system of work.
7. The supervisors were not available and could be found as they were too busy and shift ends
so no gets the trainings of got there queries answered about the hazards or risks of the
workplace.
8. When asked about the extra trainings for the workers, AM’s didn’t agreed as they consider it
a waste of time which indicates the unimportance of health safety among AM’s.
9. The workers keeps requesting the supervisors for 3 days but supervisor didn’t train the
workers which shows that supervisors are not concerned about the safety of the workers.
10. The day supervisor didn’t fully communicate/inform the new evening supervisor about the
situation rather just told him that “it is not working properly” and evening supervisor didn’t
ask further, which shows poor communication among supervisors.
11. The day supervisor didn’t handed the report of machine failure to the evening supervisor,
rather just placed it on control panel and didn’t inform the evening supervisor, which shows
the poor coordination, communication gap among supervisors.
12. The day supervisor leaves quickly and didn’t fully inform the evening supervisor about the
machine failure and evening supervisor didn’t ask about it either, which shows that
supervisors do-not follow any procedure.
13. The evening supervisor start self-inspecting the machine rather finding the report and didn’t
consult any AM regarding the machine, which shows supervisor is not concerned about his
own safety.
14. The control panel area was dim and the report could not be read which is a negative indicator
as the control panel should be well lighted enough.
15. The supervisor didn’t fully shutdown the machine rather he tried to the read the report from
the light of safety gate, which shows the supervisor was not trained enough and negligent.
16. There was only 1 first aider among the 950 total staff, who was already busy during the
accident, which shows enough people were not trained.
17. There was no emergency reporting procedure at the park, as no one recorded or reported the
injury which is a negative health safety indicator.
18. The park CEO was more concerned about the award rather employee safety, as he only
padlocked the machine and keep the haunted house running.
19. The CEO suggested that the investigation could be conducted in the morning and keep the
park running, which shows poor commitment of health safety.
20. CEO suggested to carry out the quick investigation, which shows that he is not taking the
health safety seriously and just wanted the thorough investigation only for record purposes.
21. CEO rather than hiring evening health officer/solving the current situation, blamed the delay
in investigation to health officer as he does not work evening shift which shows poor
commitment to health safety.
22. There was no accident reporting procedure developed at the park as there was no accident
recorded in the accident book.
23. There was no record or any safety policy mentioned which shows that there was none to
begin with.
Task 4: Accident Investigation
(a)
1. Barricade the accident area and secure the scene to prevent any further access and ensure the
safety of workers.
2. Ensure that the machine is not moved or tampered with to preserve the immediate
surroundings for evidence collection.
3. Interview any witnesses who saw the accident or were present when accident occur.
4. Gather the information about the workers involved/injured and require their name, role and
responsibility.
5. Inspect the hydraulic machine and safety gate to check they are working properly or any
failure/malfunction occurred.
6. Evaluate the working environment by checking lighting, ventilation, accessibility, identify
any hazards and deficiencies.
7. Check and review the training records involved in the operation of hydraulic monster.
8. Check and review the safe system of work to find any gaps and deficiencies.
9. Examine the report left by day-supervisor on control panel and analyze its contents.
10. Check the communication record between the day supervisor and maintenance team as well
as evening supervisor and check shift hand over record.
11. Review the health safety policy of the park including the procedures, trainings maintenance
record and emergency response.
12. Review the work done by contractors and in house maintenance team during the renovation
of the haunted house.
13. Verify the park’s compliance with national regulations and standards related safety and
amusement parks.
(b)
1. The safety protection device on the hydraulic machine stopped working which led the
workers to access the machine without disabling it first.
2. The day supervisor didn’t wait for the machine to be fixed and left early neither did he fully
inform the evening supervisor about the situation.
3. The day supervisor very poorly handed over the shift to evening supervisor as he left the
report on the control panel without informing anyone and left.
4. The evening supervisor attempted to operate the machine without proper training and
understanding.
5. The evening supervisor opened the safety gate to access the hydraulic machine without
disabling or confirming it is disabled.
6. There was lack of adequate lighting in the control panel area making it difficult to read and
see surroundings.
7. The evening supervisor didn’t consult or found anyother experienced supervisor for the
clarification.

Task 5: Improving the Emergency Procedure


1. Develop a comprehensive emergency plan that includes the potential risk/hazards, develop a
response team for its implementation as previously there was none to be found.
2. Conduct safety audits to identify the potential hazards, review and update the emergency
procedures as no previous record of audits was mentioned.
3. Conduct regular trainings of the workers especially safety induction for the new workers, fire
drills and emergency evacuation drills and first aid as they are given basic training and leaflet
rather than comprehensive training.
4. Provide the adequate resources for emergency situations, emergency equipment, first aid box
and trained personnel.
5. Develop an incident reporting system that allows the employs to record and report risk,
incident and near misses.
6. Conduct regular risk assessment to evaluate and mitigate potential hazards that could cause
harm.
7. Ensure the employees are trained for operating the machinery, tools and emergency
equipment.
8. Provide the basic appropriate PPE’s to all employees and ensure they are trained to use it.
9. Ensure that all employees are aware of emergency exits, fire extinguishers and other safety
equipment.
10. Establish the system to monitor and evaluate the emergency procedures and make
adjustments as necessary.
11. Establish a system for regularly reviewing and updating the emergency procedures
depending on the feedback from employees, risk assessments and incidents.
12. Ensure that all employees receive adequate training and information about the emergency
procedures.
13. Develop communication plan for emergency situations including communication with
employees and visitors.

Task 6: Working within a Health and Safety Management


System
1. A formal health safety management system will identify the risks and hazards and take
measures to eliminate them to prevent them from reoccurring as there was previously a
similar accident at haunted house.
2. It will raise awareness among workers and visitors regarding the safety and they will be more
careful taking their own safety into account as the injured worker in this totally ignored the
safety.
3. Such system will provide a safe working environment, which will increase the morale of the
workers and they will perform their job more confidently.
4. Such system will ensure that the park compliances with all the legal requirements for health
and safety as currently the park does not comply with any of the health safety standard.
5. This system will help in reducing the costs of accidents and injuries and medical treatment as
in this case the park have to bear the treatment cost of injured worker and its replacement.
6. This system help avoid the legal issues and litigation by complying with all the legal
requirements.
7. It will increase the park reputation whole and make park safe and enjoyable place for people
to enjoy.
8. By providing the safe and enjoyable place for the visitors, it will increase their reputation and
it may win the annually awards.
9. This system will improve the risk management of park enabling it to identify and manage
risk effectively as currently no risk assessment record was found because none was done
before.
10. This system will provide a framework that will continually review and improve the current
health safety standards as previously there was no system to evaluate the safety standards.
11. This system will increase the efficiency of the park by reducing time and cost for the
accidents as due to accident the park has to temporarily shut-down the monster machine.
12. This system will increase the communication between the workers by creating awareness
among them and creating a sense of responsibility as the day supervisor didn’t fully
communicate with the evening supervisor before handing over the shift thus neglecting his
duties.
13. By providing the safe working environment, there will be less accidents and less absentees
that will increase the productivity of the park and the injured can’t continue his job for
temporary period.
14. It will increase the emergency preparedness of the park that will let them handle the
emergencies more efficiently as currently there was very poor emergency response.
15. This system will promote sustainability by ensuring that park operations are eco-friendly and
socially responsible.
16. This system will increase the park reputation and bring more business which will let them
win them award which a mission of CEO.
17. This system will create awareness among workers regarding health safety and ensure that all
workers are trained enough to operate and handle the emergencies as in this case the injured
worker was not trained and there was only 1 first aider present.
18. This system will keep the workers involved in health-safety decision makings and make them
feel valued.

Task 7: Benefits of Combined Worker and Employer


Involvement
1. It will improve the overall health safety culture by promoting a sense of ownership and
responsibility among the workers and employees as injured worker was not concerned about
his safety.
2. It will help to identify and manage the risks associated with park operations especially those
that could cause harm to workers and visitors as there was no risk assessment conducted on
the hydraulic machine.
3. Enhancing the workers knowledge and awareness of potential hazards associated with their
jobs and to control them as the injured worker was not knowledgeable about the hazards
which lead to the accident.
4. Encouraging the workers to report any safety concerns/near misses/accidents/hazards without
the fear of retaliation as the day supervisor didn’t report any hazards or risks associated with
the hydraulic machine.
5. Providing an opportunity to the workers to share their experience and expertise in identifying
hazards and implementing control measures to improve the current health safety standards.
6. Promoting the positive public image by showing park’s commitment to safety and security.
7. Reducing the numbers of injuries and absentees among the workers thus reducing the cost of
treatments and medical expenses as the injured worker lost his fingers and couldn’t continue
work and park to bear its expenses and replacement.
8. Increasing the likelihood of winning the annually award which was the ultimate goal of CEO.
9. Increasing the customer satisfaction by providing a safe and enjoyable environment for the
visitors thus increasing the business and award winning chances.
10. Ensuring that the safety policies are realistic and achievable increasing the workers
confidence in safety management system and decreasing their stress and anxiety levels as
workers were under pressure to perform well otherwise their roles will be reviewed which
lead them ignore the health safety.
